You need to duct tape it all down, so that it doesn't get wet

This is so bad and completely ridiculous rubbish
    1) all that rubbish for the nappy is not needed all you need I one nappy over the cast and that’s it
    2) a child in a spica be it for a break or hip dysplasia they should NEVER EVER be in any kind of car seat ESPECIALLY rear facing as this does more damage to the hips etc

Where did you get your medical degree?
Your doctor might've told you that your child shouldn't be a car seat with his/her spica, and that you only need one diaper, but this video was not made by your doctor. A lot of hospitals, including Arkansas Children's Hospital, do things this way. If you look at any number of videos by families about dealing with a spica, or educational material by medical professionals, either their child fits in their existing car seat, or they had to rent one from the hospital, and an additional diaper is used inside the cast. The harness is an option, but I haven't found a video of a family that prefers it. Just because you do it differently from everyone else, doesn't mean everyone else is wrong.
