I think witchers having an american accent was to divide them from all the other people in the witcher world. To highlight that they don't belong anywhere. With other characters their accent tells right away what region they're from. In medieval times american accent didn't exist so I think it's representing them not having an accent.

In the books, Geralt says that he fakes a Rivian accent in order to fit the backstory. Whether that’s what we’re hearing in the games, I’m unsure.
