Fliegenbein leading the dance and everyone else following as though they were in a trance symbolises the spreading of the plague. Paracelsus identifies him as a carrier, because he has seen him in every city where the plague broke out. When he tumbles around and the people want to grab him he yells "Don't touch him! He has the plague!" The rest is probably self explanatory, the grim reaper appears to claim him, but Paracelsus fends him off. He intends to cure Fliegenbein.
Since I don't speak German, I have no idea what anyone is saying here - but people gradually being drawn into a jerky, spasmodic mass bewitched dance seems very subversive for the strict Nazi censorship to have allowed. Maybe the man who started it all was revealed to be a dangerous element who was fortunately identified before he could do more damage to the good German people in this tavern. They certainly fled from him in terror once the other man examined him.
Bin hier, weil Daniel Kehlmann diese Szene in "Lichtspiel" so ausdrucksstark schildert. Toll, dass man es hier sehen kann!
