Good question! You use the same formula though. So for the work module you add up the 4 questions to get a total number, you divide that by 4 (the number of questions answered), subtract 1, then multiply by 25 to get you final score. If that doesn’t make sense let me know!
I also should have made it clear that you do calculate it separately and not together. So don’t add the original 11 questions with the work module or the sports/performing arts module.

You didn't include what the scores actually represent....
The test/questionnaire is looking at percentage of disability/impairment of the upper extremity. Of course, this is subjective though.
