Stroke Leg Exercise: The BEST way to stand from a chair

  • If you have hemiplegia (weakness on one side of the body) you may be using a compensation strategy to get up out of a chair. If so, there IS a better way. In this video you will learn the BEST progression to learn how to stand up from a chair using a “facilitation” strategy. This will help you to regain normal movement in the weaker leg.
    It is so important to remember that as the brain is healing, your brain will develop NEW pathways. If you compensate with the stronger side of your body, you miss out on the opportunities to gain new pathways on the damaged side of the brain.
    This progression will teach you how to use your weaker leg in an easy to follow progression regardless of where you are at in the recovery process.

    Dr. Tara Tobias, thank you. I love your videos which I just discovered today and have just spent nearly two hours studying with you. I had a stroke nearly 8 years ago and still have many problems. I envy the people who get to directly work with you. They are lucky and blessed to have the best. I am an American who resides in Japan now but have also had rehab in the US for awhile. I still have rehab in Japan and work with therapists three times weekly, but they are not as knowledgeable , skilled and articulate as you. I am so glad that I happened upon your videos. Thank you very much. I have many questions which I will pose to you later. Now I can have my rphysical therapy rehab sessions every day.

    Tara I appreciate your positive can do approach I had a major stroke and the greatest thing to factor in is fear I've had my left knee simply fold out from under me and dropped down you'd be surprised in a typical facility at how hard the floor is and how sharp furniture contains corners that I just don't want to hit it's called fear the easiest way to get up I must admit is that without an armrest on a chair it is simply easier to get up so perhaps I should not be so dependent upon the armrest thank you for all of your work it's appreciated a stroke is a constant struggle I don't care what anybody says and I believe you know that but you do make it easier!

    I am so glad that I don’t do any compensatory strategies. People are always surprised when I stand up because I do it very fast. Thank God the hard work that I do every single day is paying off. 😀

    Wow doc! I found this after years of chronic pain in my leg. And it's by far the most accurate and perfect excersices. I had Legg Calve Perthes during my childhood, and a Chiari Osteotomy at the age of 21 (wighting 130 KG), after that plus my overweight docs said to take it easy, which made me do just the opposite. By the age of 24 I was weighting 75 KG after training 8 hours a day, and runing my first thriatlon, seemed I recovered and got my health back.. but I was a little wrong. I neglected for YEARS (im 34 now) the focused rehab work on my affected leg. And day after day I got used to compensate, even when training under the hardest conditions. So I basically did amazing things, with just one strong leg, I couldnt explain what was going on, I knew the involved leg lacked of power, but is just when seeing this video or the Trendeburg Gait one you posted that I know how much I messed up of 10+ years of intense training, commited and focus, but never paying attention to something as important as this. Results? My gait is a disaster, I can barely stant a day with the ammount of pain my hip has. If I walk 500 meters I get spasms and can't use anymore the lag (unles is warmed up) so I cycle everywhere. I will start now with this special excersices and a new hope that I might actually be able to recover my hip without going through a hip replacement. I will keep you posted doc!

    Thank you I am 2 1/2 years post stroke and have the same problem with my knee going out in the left side of my foot been tilted physical therapy at my doctors office has been unsuccessful. This has been very helpful.

    Dr. Tobias, I suffered a stroke on July 8th 2023, I have been in acute rehab for a month and will be moving into a subacute rehab this week. My team has been great and I'm making constant progress every week. That said I can't wait to put many of your exercises to work. In addition to having had 2 strokes, I have also had Spinal MS for 40 years. Your videos give me hope and inspiration that I can make a full recovery if I put in the work. My biggest challenges are muscle tone complicated with MS balance issues.

    Hello Dr. Tara thank you for sharing your video's it helps a lot , i'm stroke patient from the Philippines ..my 2nd. year right now my situation ..i'm doing good getting there ..doing my homework seriousley 2x a day ..i know theres no time table just keep on doing work for the best...the most important for me building my confidence..now i walk straight..hands down..speech good. I'm getting there. Stairs? I still need hand bars for safety precaution , my PT said im on 85% looking good👍😊

    I had a brain bleed stroke on the right side just before Thanksgiving, 2020. I had two drains put in to relieve the pressure. Due to Covid issues in the facility I was in I only got one therapy session in and really had to learn to leave the walker behind on my own at home. So I came home after a month in the hospital but was able to climb our 14 stairs without much of an issue. I was able to get into PT, OT and speech therapy here back home about two months after leaving the hospital.
    I can do the standing rather easily, my issue is that I have numbness and tingling from the left side shoulder down throw the left leg. The top of my thigh to the shoulder is stiff. I walk about 20 minutes a day currently some inclines involved too and can do it without a cane until or unless I get tired or unsteady. Generally I carry the cane horizontally across my beltline, almost as an aid and reminder to stand tall- I do tend to lean forward. It's like my left leg weighs some 50 pounds more that what it should and that my left shoulder is bound up. I can move both but the stiffness is crazy. If my left side could move like my right side can, I'd be fine. You may even have videos already that could address these issues. I've also wondered about acupuncture or TENS to help 'reroute' those nerves or to 'wake them up'. Anyway, thanks, especially for the encouragement to never give up as I had one person tell me (something I heard you 'preach' against, that whatever you gain in the first 12 to 18 is the best I should expect to get. I'll be 72 in August, so really didn't want to hear that! The family doctor gave my muscle relaxants as it feels like my muscles are 'turned on' and never off, tight in the shoulder and below and just above the knew with lots of tingling. Thanks for any ideas that could help!

    I ride a recumbent trike. With a recumbent trike you only use your leg muscles. Is riding the recumbent trike a detriment to my progress? I also have a recumbent exercise bike for in the house. While I like the freedom to travel up to 20 miles,I will,curtail my riding if it is inhibiting my progress. My right leg muscles are getting stronger but I believe my involved leg is not gaining any strength. Thanks to your excellent training ,I realize now that I have become dependent on compensatory strategies. This is something that No therapist has ever pointed out to. You young Lady are exceptional, I a. Sure that I am not the only person that recognize so your excellent guidance.

    Sometimes when I stand from a seated position my involved foot/leg wants to rise off the floor which can make me somewhat unstable. Why is that and what's a fix, just repetitions?

    Hello,
    I was watching your bonus video about home gym equipment, then I watched the really interesting compensatory strategy versus facilitation strategy.
    I was in a nursing home that had a rehabilitation bicycle that had a display option that I loved.
    I was able to set the resistance for my weak leg to a higher resistance level than for my strongest leg.
    Plus, I could roll my wheelchair up to the machine because it did not have a seat. My wheelchair could be strapped securely to the machine.
    It also allowed for different resistance levels for each arm, as well.
    I have been trying to find such a machine for my home gym but so far, no luck.
    Maybe you can help?

    I cannot pull back my involved foot (left for) to stand up from the chair. unless use right foot to push left foot back to the pulled back position .

    Dr. Tara, I've dealt with MS since 1998. My right arm and leg are weak. The right leg really needs help because of muscle loss. Is performing exercises wise before I attempt to build muscle in the leg, if that's even possible? I'd appreciate your input.

    Love your videos. I am not a stroke victim, but I have right side weaknesses caused by transverse myelitis. This has significantly affected my gait, and my right side strength (waist down). Are there any specific videos you would recommend for me. Foot drop and hyper extension are most noticeable issues.

My recommendation is you watch videos that describe your movement problem. Try starting with the “stroke hip and/or knee control” playlists. Also comment with any suggestions for future videos!

    My husband had central nervous system lymphoma CNS lymphoma in 2014 he recovered fine walked right was able to hold a hammer. It affected his right side of his body. Then in 2015 lymphoma came back with a vengeance. Which left him with him locking his knee and his arm and hand curls. We’ve been to therapy and nothing seems to work. In 2020 he had a brain bleed. He’s okay except just recently this year 2022 he was getting therapy twice a week. One therapist worked on his leg and the other on his hand. And now they said he finished his therapy but he can’t even walk anymore. They left him worse than when he started. Why would you think he got worse? I know that after therapy he would come to bed and sleep and never got up to walk. Therapist did say that his muscles weren’t locked yet that he could get back his movements. But why is he not able to walk anymore?

    🔴🔴 Hello & Thank You, SEPT 2023 74 yr old HAD covid vaccination, stroke 2 years later, mild bleed in thalamus, diagnosed with involuntary muscle movement ARM & LEG 🔴PROBLEM IS : LEG wiggles around alot SLOWLY ..like some1 else is controlling his LEGS & ARMS, leaving him susceptible to falling.     Dr. Tara Tobias, what strategy for standing up do you suggest for a person with ACA artery stroke, she only knows to pull with her hands on the walker and pushes with strong leg. Also she has a "magnetic foot" as in the foot sticks to the ground when walking, and abulia. Thank you!

      Thanks for watching! that is a great question. It is always difficult once the muscles have shortened. First is your AFO articulate or solid? (does it bend at the ankle)? It is best to have an articulated AFO. Two, start wearing your night splint 😊(I am sure that goes without saying). And watch the series on ankle spasticity. The standing portion of the progression.       Orlando Neuro Therapy my AFO has a hinge and it can bend only slightly, I’m going to get fitted for a new AFO tomorrow, I’ll make sure it can bend more, I’ll practice the exercises and stand more to stretch the ankle, thank you so much!

    I wear an AFO most of my day but do take it off to exercise. Would I take it off for this also?

      remember "perfect practice makes perfect". you want to make sure you are practicing how you want the movement to look down the road. So, if your foot rolls or you can control your ankle with this exercise, yes, use your brace. I hope this helps. :)

    I had a stroke on March 8, 2 months ago, I'm recovering. I every single day why do my legs feel if though my muscles are tight and stiff. I thought I would feel loosen and better.

      Yes, I do not use the word "weak leg" as I feel this creates a counter-productive mindset. In reality the leg is not weak. It if you have hemiplegia, the involved leg is the leg that has been impacted by the damage to the brain. I hope that clears things up!

      The glutes allow you to be able to extend, rotate and abduct your leg.

    Is there s somehow I can test my balance, to see just how bad/good it is. So I know where I am balance wise

      Thanks for asking! I know you have said in the past that you have a physical therapist. I would recommend going back even if it is for 1-2 visits and ask them specifically to give you this information. I know in the US, most insurances cover PT and it usually "resets" every year. I think you live in Canada so I am not sure about that but still worth a try :)

      Orlando Neuro Therapy The therapist I was referring to was one I use to go to before my insurance ran out. Now I have to pay out of pocket, which is to expensive for me to afford, on a fixed income. So I stopped going. You now are my only therapist that I have :)

      You can do these with or without. The function of the AFO is to prevent the foot from pointing down due to the spasticity.
