He blew out his vocal cords but he's been working with a vocal coach to come up with new techniques to keep his voice healthy as he's aging, that's why he sounded so bad for a while but has gotten so much better. If you listen to the most recent live performances of A Perfect Circle on tracks like Talk Talk, he found a cool combination of effects that allow him to pull off the screams and it sounds super good. He's really just hitting a note hard and then processing it in real time.
My guess is he probably can't while playing the riff.. there's rarely one spot where the guitarist or bassists can take their mind off the music they are playing.. a lot of it seems simple but there's a lot of nuance to the parts and they are rarely in standard time.. that upload was awesome

Had I listened to them a bit beforehand, I think I would have enjoyed it more, but their music isn't available digitally. They're hard to get into at first, to me at the time it just sounded like they were playing the same song over and over again, and I didn't really feel any connection. I wasn't a fan of all the visual stuff either, I prefer to focus on the band and the music they're playing, but that's just personal preference.

They only seem to do the extended Schism and Stinkfist at their own, arena shows. At festivals they just play the album versions. No "Parabol" at festivals either.

And by the 8th joint we were at war. This current tour set list has no jambi, schism, vicarious or parabola. Or the pot, or lateralus. Was kinda disappointed at the O2 London date recently (10 may). But still the war waited for no one.

You'll have to hunt down individual songs, I think someone managed to cobble together some footage of the set but there isn't any Proshot footage of it since Tool are infamous for not liking their shows filmed so no official cameras were rolling.
