Ranni, Malenia, and Miquella are demigod empyreans and are able to become gods. I think when Miquella first made the promise with Radahn, Radahn agreed on the unlikely basis of Miquella achieving godhood. Marika is the only actual god we know of. It probably took a long time for Miquella to even discover how Marika became a god, and even the existence of the shadow lands.
During this time he is trying cure Malenia, and create the Hiligtree. By the time he figured it out, it was probably during the chaos of the Shattering War.
