- People with knee osteoarthritis are usually guided by healthcare professionals to focus on doing thigh exercises to help improve knee arthritis.
If exercise is supposed to help knee arthritis, why is there more pain during and after exercise?
Are these really the right muscles to focus on?
Could focusing on the wrong muscles possibly be the reason exercising is making your knee hurt more?
In this video, Dr. David explains to you 7 reasons why stronger thigh muscles make knee arthritis worse.

You are the reason I cancelled my knee surgery. I saw your videos on how to stand, walk and do glute exercises 4 weeks before my scheduled surgery. ( pre-rehab had me doing these exercises you are talking about now, my pain was worsening!) On the very first day of doing what you suggested. I got up easily with no pain from a standing position!!! I continued your exercises, its now 2 months since I was scheduled for surgery and I am walking, no pain, no walker and no cane!! You are a God send ❤❤❤ and I thank Jesus everyday for pointing me to your videos!!! God bless you Dr David!!!!
My surgeon asked, what made you cancel surgery? I said," The Lord God intervened and showed me these videos."
Never under estimate the power of God!,
Glad to hear you got better and no surgery! I’ve been going to PT, and my knees is worse and now in bed with leg up, trying to get better!
@@alleynancitry his exercises, start slow,. Watch all his videos about knees. 🙏🏼God bless , praying you find relief. I understand the pain.
Thank you for sharing your experience. This gives me hope.
@@dianecaine4529 never give up hope. Work hard on the right exercises, it makes a real difference! God bless!
Dearest Dr. David, You have turned me into a glute-maniac! Glutes while brushing teeth, baby glue squats while on the phone, etc. etc. And in 4 months of following your advice I went from severe knee pain to zero knee pain. And just in time because I tripped and fractured my hip on the same leg. I was ever so so grateful that the knee pain had gone or what would I have done? So after surgery, and bed bound I searched again to find my glutes and little by little got back in touch with them and started tightening while in bed, This glute work did hurt my hip flexors a bit but they have gradually recovered. I am a 77 year old female who is ever so grateful to you for sharing your wisdom.
Hiya,
I am a practised MSK Adult Band 6 Physiotherapist based in London, UK
I want to say thank you for your help in changing fully my understanding of treating patients with knee pain.
I can t see how my patients are getting better without strengthening the quadriceps mainly focusing on glutes.
Thank you and good luck.
This is why you are a God sent individual!❤ I feel like no one where I live knows what they are doing! Your the only reason I can walk without pain and limping.
You really have a knack of putting yourself into the patients position and make this concept understandable for the lay person. I so much appreciate what you are doing. I am already benefitting from your Hip Arthritis Recovery Program.
Could you please do a video on the proper way to walk with knee pain to keep it from rubbing in the first place? I had a botched knee replacement which later caused both my hips to wear out and had 2 hip replacements. I was walking like a duck. I lost all my leg muscles. Now that I'm better, I had to learn to walk all over again. I'm an aggressive walker. I push up with my toes... I think that might be giving my last real knee quick pain. I go out and sit on my truck bed tailgate and swing my knees which helps, but I need to know how to properly walk so I don't get myself in that situation. I'm also working on my glutes, but at age 70, everything is such a struggle. I walk barefooted a lot, too... seems shoes do help some but I get tripped easily with shoes on. I'm in and outside a lot as I still manage a small suburban garden and farm. Help?
Thank you, my issue is sciatica, but all your videos help me to prevent other problems while trying to get better. God Bless you and Texas.
Dear Dr. David, as always you give us the best advice that no doctor has told us. You are the best 😃😃😃👍 Thank you so much from the bottom of my heart 🙏🙏🙏❤️
Warm greetings from Vesna from Italy ❤️🌹
Today, I'm 8 weeks out from torn meniscus surgery. Love my doc, but he pushed quad exercises and physical therapy, too. I ignored them and have been going gluteous to the maximus. I can't imagine having recovered this well if I hadn't gone in search of someone, like you, providing alternatives. You make sense and I'm enjoying the results. Doc did smooth out some arthritis at the time, and it's reassuring to be prepared for possible issues with my other knee. I'm trying hard to not overdo it, but won't settle for less than full recovery. I'm in San Antonio, and the yardwork has been calling, but it'll have to be patient, as well. 😜 Much thanks for being a knowledgeable, charitable, and valuable resource.
I agree. I find I can't do leg extension, squat and deadlift without causing knee pain. But i can do romanian deadlift which focus more on glute and hamstring and less on quad without knee pain. And doing so allow me to walk and cycle long distance painless. Many thanks for your advice.
All true. I stopped working my quads over year ago after watching one of your videos. No more knee pain. I tell everyone who will listen to me.
Totally agree! Glutes rule the knees. Also I have tight quads and loose hammies and since I’ve strengthened the ham/glute complex my anterior knee pain is soooo much better!
So helpful ❤
I got doc, we have to have a good sense and learn about it! I will work my butt off!😂. Thank you for taking the time to inform us the true!
Thank you.
Thankyou Thankyou very much. God bless
Watching from Spain. This really makes a lot of sense. Gracias Dr. David!! 👏🤓

Fabulous thank you soooo much. You are wonderful 🎉
Brilliant Dr David I use your teaching techniques in my rehab pilates classes i advise my clients of the importance of glute strenth. Has become so helpful. To them you have so much knowledge so happy I found you on line god bless and keep up your great work you are helping so many people and its obvious you are so passionate about your work
Hi thanks again for sharing your videos I watch them all the they really do help!
Hey Carol! Thank you so much for your support, we are so happy to hear that we have been able to help! 🧡✨
Your exercises have benefitted me so much. These low impact exercises can be done sitting or standing. There is no pain. I can't believe how much more I can do verses last year when I was being told I needed a knee replacement.
Hi! Thank you for helping me recover from my menisectomy 😊 Do you have a video for sacroiliac joint pain that causes feet numbness when standing?? Thank you
Your suggestion of glute exercises has worked very well for me. Wish I was there in El Paso to meet with you and your staff you have trained. Maybe another day, Fall maybe:) Inflammation sneaks in every once in awhile which I need to get better control over that. Most days amazing however 😊
You are my go-to guy for everything about knees🎉❤I have condromalasia patella. I’m doing clamshells etc for glutes and also working adductors and abductors but want to advance to using weights. A trainer suggested kettle bell swings with 5-10 pound weights but keep all my weight in my heels. Would like to hear your thoughts.
If you dont strengthen your Quads how do you get the muscular strength to climbs stairs?
What kind of exercise do I do? What can use my thighs and my leg?
I was going to start private yoga again after a break with the start of covid. I did yoga for over 20 years and now thinking maybe those built up my quads because I suffer from at times infammation of my knees but not stage 4 maybe 2 with my knees. Thinking I better do your glute exercises before I start up again thinking the yogi may not understand about what you are talking about here with glutes!!!
Hello Dr. David. I had an X-ray and a mri scan last week and the doc said that it is a meniscus tear in left knee. He did prescribe me exercises which was tightening of the thigh muscles which also included floding the knees up until the chest while lying on the back. The next full week the pain only increased! Didn't know why though!
Then when i found your channel with videos for knee meniscus to strengthen the glutes like the clamshell, isometric standing, hanging the knees from a heightened table and swinging it, including firing the glutes while walking, the pain has reduced. I will continue doing these exercises.
Is there a way to contact you online? I am from India.
Thank you so much for what you do! You are precious!
How can we understand which one is our dominant muscle? Thank you.
Question having lumbar stenous and pinched nerve does wearing compression shorts Help?
Dear doc kindly assist me what I have to do
I am diagnosed knee arthritis

Kindly help me
Thanks a lot
Could you do a video on bone bruising in knee Will wearimg a knee brace help this? What can you do to help with healing that.
All are great recommendations. But I think stretches for quads and hamstrings are essential for relieving the “pull” across the joints from those “stronger” and tighter muscles. Muscle strength and stretch are both essential for joint balance. You can’t adequately address one without including the other.
At 22:45 you are mentioning about working on the quads when one has knee surgery sprain etc. Do this also apply for a meniscus tear!
Hello Dr. David, are there any glute strengthening exercises that can be done on a bicycle? Thanks!
I just looked up your location in El Paso and found 3 sites. Which one do you see and work with patients in?

Thank you for your suggestion that strengthening the glute to reduce the knee pain, after doing the suggested exercises for only one week, my knee pain reduce significantly! And after one month, I do not feel any more pain when I am stepping down the stairs! Thank you very much for your detailed instructions and explanation!
So…..we have to live with weak thighs? Just to spare the kneecap?
i read to sit on bed and kick out your leg to keep fluid flow so that not good
my knee started clicking when i walk
Dr. David: I see your diagnosing and treatment as a “new” specialty spinning off of what we currently know as physical therapy. To me, it “shouldn’t challenge what they’re doing” as much as it should have its OWN place in the medical field. They keep touting the tremendous advances in medicine, but it’s definitely a closed community that resists change!
