  • People with knee osteoarthritis are usually guided by healthcare professionals to focus on doing thigh exercises to help improve knee arthritis.
If exercise is supposed to help knee arthritis, why is there more pain during and after exercise?
Are these really the right muscles to focus on?
Could focusing on the wrong muscles possibly be the reason exercising is making your knee hurt more?
In this video, Dr. David explains to you 7 reasons why stronger thigh muscles make knee arthritis worse.

You are the reason I cancelled my knee surgery. I saw your videos on how to stand, walk and do glute exercises 4 weeks before my scheduled surgery. ( pre-rehab had me doing these exercises you are talking about now, my pain was worsening!) On the very first day of doing what you suggested. I got up easily with no pain from a standing position!!! I continued your exercises, its now 2 months since I was scheduled for surgery and I am walking, no pain, no walker and no cane!! You are a God send ❤❤❤ and I thank Jesus everyday for pointing me to your videos!!! God bless you Dr David!!!!
My surgeon asked, what made you cancel surgery? I said," The Lord God intervened and showed me these videos."
Never under estimate the power of God!,

Dearest Dr. David, You have turned me into a glute-maniac! Glutes while brushing teeth, baby glue squats while on the phone, etc. etc. And in 4 months of following your advice I went from severe knee pain to zero knee pain. And just in time because I tripped and fractured my hip on the same leg. I was ever so so grateful that the knee pain had gone or what would I have done? So after surgery, and bed bound I searched again to find my glutes and little by little got back in touch with them and started tightening while in bed, This glute work did hurt my hip flexors a bit but they have gradually recovered. I am a 77 year old female who is ever so grateful to you for sharing your wisdom.

Could you please do a video on the proper way to walk with knee pain to keep it from rubbing in the first place? I had a botched knee replacement which later caused both my hips to wear out and had 2 hip replacements. I was walking like a duck. I lost all my leg muscles. Now that I'm better, I had to learn to walk all over again. I'm an aggressive walker. I push up with my toes... I think that might be giving my last real knee quick pain. I go out and sit on my truck bed tailgate and swing my knees which helps, but I need to know how to properly walk so I don't get myself in that situation. I'm also working on my glutes, but at age 70, everything is such a struggle. I walk barefooted a lot, too... seems shoes do help some but I get tripped easily with shoes on. I'm in and outside a lot as I still manage a small suburban garden and farm. Help?

Today, I'm 8 weeks out from torn meniscus surgery. Love my doc, but he pushed quad exercises and physical therapy, too. I ignored them and have been going gluteous to the maximus. I can't imagine having recovered this well if I hadn't gone in search of someone, like you, providing alternatives. You make sense and I'm enjoying the results. Doc did smooth out some arthritis at the time, and it's reassuring to be prepared for possible issues with my other knee. I'm trying hard to not overdo it, but won't settle for less than full recovery. I'm in San Antonio, and the yardwork has been calling, but it'll have to be patient, as well. 😜 Much thanks for being a knowledgeable, charitable, and valuable resource.

I agree. I find I can't do leg extension, squat and deadlift without causing knee pain. But i can do romanian deadlift which focus more on glute and hamstring and less on quad without knee pain. And doing so allow me to walk and cycle long distance painless. Many thanks for your advice.

I was going to start private yoga again after a break with the start of covid. I did yoga for over 20 years and now thinking maybe those built up my quads because I suffer from at times infammation of my knees but not stage 4 maybe 2 with my knees. Thinking I better do your glute exercises before I start up again thinking the yogi may not understand about what you are talking about here with glutes!!!

Hello Dr. David. I had an X-ray and a mri scan last week and the doc said that it is a meniscus tear in left knee. He did prescribe me exercises which was tightening of the thigh muscles which also included floding the knees up until the chest while lying on the back. The next full week the pain only increased! Didn't know why though!
Then when i found your channel with videos for knee meniscus to strengthen the glutes like the clamshell, isometric standing, hanging the knees from a heightened table and swinging it, including firing the glutes while walking, the pain has reduced. I will continue doing these exercises.
Is there a way to contact you online? I am from India.
Thank you so much for what you do! You are precious!

All are great recommendations. But I think stretches for quads and hamstrings are essential for relieving the “pull” across the joints from those “stronger” and tighter muscles. Muscle strength and stretch are both essential for joint balance. You can’t adequately address one without including the other.

Dr. David: I see your diagnosing and treatment as a “new” specialty spinning off of what we currently know as physical therapy. To me, it “shouldn’t challenge what they’re doing” as much as it should have its OWN place in the medical field. They keep touting the tremendous advances in medicine, but it’s definitely a closed community that resists change!
