PC ISP v4 0 Overview

This video is designed to provide an overview of the coming updates to Virginia's Person-Centered Individual Support Plan (PC ISP). The primary updates this year will move the Risk Awareness Tool (RAT) into the ISP to place the tool into the SC workflow, create the opportunity to reduce redundancy and streamline ISP elements, and automate identified risk factors within the ISP.
