You’re right, this is a rare condition, however when this does happen it’s usually due to a high velocity impact upon a person in the wrong spot at the wrong time. When this does happen, it usually involves someone conducting an activity where high velocity impacts happen often, including but not limited to athletes. There are multiple instances where Football and Baseball players take a blow to the chest and have a sudden cardiac arrest, albeit this usually doesn’t happen in pro or semipro leagues. This more commonly happens in high school sports where there’s a large discrepancy between skill and size. This happened to a player from a neighboring high school of mine in a basketball game. He was around 6’0” 165 taking a charge from a player who was 6’7” 220 on a fast break and got landed on after the impact. No one is sure whether the charge or the landing caused it, but the 6 footer got up and collapsed. He luckily lived and revealed it was a cardiac arrest through blunt force trauma and a collapsed lung. The Wikipedia article for Commotio Cordis is great for understanding this on a more thorough level, especially when you consider that this mostly happens in youth sports. This means we’re less inclined to hear about this on a national level. Also, believe what you want but turning this into a political debate is not the move. Take the vaccine or don’t, no one cares. Don’t turn this young man’s life and turn it into a pawn to prove a trivial point, it’s poor, poor taste.
@@Noncollectable The key point here is that Commotio Cordis has known predisposing conditions. These should have been picked up by what are likely to be extremely well trained sports med physicians. We can safely assume that those known preconditions were not present.
Hopefully he had CPR immediately and a quick return of spontaneous circulation after CPR and will have a good outcome. I am a neurologist and have done many consults to assess neurological function after a cardiac arrest, but I have never seen commotio cordis. I would think an aortic dissection would take longer to develop than a few seconds. I have had patients with a good neurological outcome who had CPR for much longer than 9 minutes. There is a big difference between medical professionals doing CPR immediately versus laypersons in a gym. I had a patient with a severe cardiac contusion from a car accident who did not develop a cardiac arrhythmia. The cardiac contusion was apparent on EKG (The Q waves had a distinctive unusual morphology different from what is seen with myocardial infarction). It is concerning that he did not reanimate after CPR, but I have had comatose survivors of cardiac arrest with a good long-term outcome. Thoughts and prayers for Damar and his family.

Had this been say a high school or small college game with lesser trained first responders and no AED, BVM or O2 (a situation that happens several times a year) the outcome would not be optimistic in my amateur opinion. Given Hamlin had well trained first responders, an AED and a BVM with O2, I am optimistic about his recovery despite the relatively long time he was given CPR in my amateur opinion.(again) I have read some being critical about it taking too long for him to get loaded up and then driven away in the ambulance. This criticism is unwarranted in my opinion.

My elder son died of an aortic dissection at 31- no previous symptoms that were noticeable. Our family has never been the same, 15 years the 11th of this month, his father died 4 years later, not the diagnosis that was official but I feel a broken- heart contributed to the death. The 2 family members still go on; but forever things are changed.
I am fairly certain that his right arm moving is simply decorticate posturing. I don’t think he’s controlling that whatsoever. The couple of times I’ve performed CPR when I was present for the onset of cardiac arrest, they started to posture. And for anyone wondering, this is a reflex when the brain doesn’t get enough oxygen or severe trauma to the brain has occurred
@@stevebetker829 posturing doesn’t mean much more than ‘lack of oxygen to the brain’. People have started to posture and end up being ok in the end. You see posturing a lot when people injure their heads….but I’m fairly confident he is posturing here. Once again, that doesn’t necessarily mean he won’t recover, he totally could and he’s young and CPR was administered fairly quickly. So he’s got a lot of good attributes going into this.

This was so scary to experience just watching at home. I can't imagine what the team is going through, what his family is going through. His mother was at the game, and rode with him in the ambulance. I have kids close to Damar's age, and what she is dealing with tonight is my worst nightmare. I will keep praying for Damar and his family and teammates.

Hi Mike, fellow physician here (surgeon). My Alma mater Cornell University had a situation that led to the death of a Cornell Lacrosse senior in 2004, the late George Boiardi. If I remember correctly, he was defending and took a goal shot in the chest, went into arrest and died at the medical center. Every time I watch Cornell Lacrosse if I’m reminded of this it hurts. Cornell lost to Maryland in the NCAA Division I championship game this past season with Maryland coached by no less a Cornell Class of ‘91 grad. Well, we had Richie Moran coach us to three national championships in the 70’s and two championship loss while I was there in the late 80’s and he played for Maryland. Thanks for the video. I know this is unlikely, but could this be a complication from a recent Covid shot? I’ve heard of increased occurrences of cardiac issues in otherwise healthy males after getting a Covid shot. I ask this because the hit he took was not near as severe as many other hits players receive during the course of a football game.
@@arerki09not sure, I know the lacrosse community took major steps to improve chest protection after 2004. Don’t know what George was wearing that day. But at the highest level Division I lacrosse teams like Cornell, Hopkins, Maryland etc. these guys can shoot the ball at 100 MPH. Not sure if any body armor can stop this one in a million tragedy. I’m pretty sure goalies are mandated to wear pads. But I would assume attack, mid fielders and long stick defenders need to strike a balance between speed and protection. Good thing is body armor can be pretty light yet strong. I think best design is one which disperses the energy of a hit across a large surface area thus decreasing the energy of the shot at point of impact.
This kind of stuff is so scary and serious. I saw one just like this a couple years back at a high school football game. The kid had drank 4 monsters to get energized for the game, and he went into cardiac arrest on the field. They almost suspended the game, but continued to play about an hour later. He was immediately taken to a local hospital in our hometown and was okay. Prayers up🙏🙏 to damar and his family.
The first thing that comes to your mind is something that has NEVER happened in the NFL before? Hmmmm. Well, here we are 5 days later and the first-hand doctors at the hospital don't buy what you are saying. They also said that more tests would need to be done to KNOW what actually caused the cardiac arrest. I would hold off on coming to any conclusions based on watching this from your couch, because there are plenty of highly qualified doctors who think this might be related to an adverse event from an injection. Especially given the fact that CDC scientists have done a study which found 229 young folks, aged 12-24, develop myocarditis, out of one million doses of injection. Blood clots were also found among others. So there are plenty of OTHER causes that are more probable than something that has NEVER happened in the NFL before.
I'm an RN (retired) and was at a Jiu Jitsu class, when a guy I was practicing with dropped into me after the class. I started CPR immediately with another guy (two man CPR) until the fire departement arrrived. They defibrillated him on the mat and he was ambulanced to the hospital, where he was given emergency bypass surgery. He made a full recovery with no loss of function. The only permanent damage was memory loss of the event.
That cause is different than what we are talking about. Cardiovascular disease vs commotio cordis are very different. You don't have a bypass from the later
The class should have a defib.. especially nowadays. Look into how many recalls there have been on Philips pads in the last 2 years vs last 10. Its almost like they are being used and inherently tested more recently.

I don't think it's any worse than the countless times I've seen players get brutally knocked out in football. Sure, this young man may pass away, but all these players are dying a slow and horrible death with brain damage.
There are so many ways to die. Our bodies are constantly performing a delicate balance to keep us alive. It's almost as if life is a constant struggle with a universe of chaos.
I know the incident is a freak timing thing between the collision and when his heart is re-polarizing...but I can't believe this hasn't happened before in the NFL given the physical nature of the sport.
I wonder what underlying issues were going on. Because that initially would cause an arrest, but a healthy heart may spontaneously start again in NSR. Hopefully he makes it and recovers. May require further workup. EPS etc.
Impacts are not as violent now as they were. Helmet to chest are now illegal but were once common. His hit (chest to chest) seemed tame compared to many others I've seen. Usually, its kids who have weaker less developed chest who have cardiac arrests from hard chest hit. I'm thinking something else was involved. Heart damage to electrical system from (myocarditis?)
As an update he hasn't thanked people for the donations or the charity, but he did show up wearing a ski mask and a hoodie indoors with not one picture of his face.
It wasn’t commotio cordis. Their shoulder pads provide way too much chest protection these days for that to happen. Plus, the hit wasn’t near hard enough. The only way in football for commotio cordis to occur is basically if the very tip of an elbow somehow could get past those hardcore chest pads and hit the left chest wall area in the way a softball does. Softball or something similar where a smaller object at an insane speed directly to the heart or temple is where you’d typically see something this severe. It doesn’t happen in football. If you can tell me the last time you saw something just like this in the NFL then I’d recommend you to become a sports analyst or historian. It wasn’t commotio cordis.

Soccer players have dropped dead for many, many years now. Look up a few soccer player collapsing vids and you'll find a lot from years ago. Lots of soccer players get heart issues because it's the world's most popular sport and billions play it

being an avid NFL fan from Australia, I have seen something similar to this almost a decade ago but in a different sporting code, as a sporting fan, it is one of the most horrifying events one could witness, you don't want to see it happen, and you want those players to make it to the next week safe and ready to play for their team. praying that Damar can pull through.

Hey, the family has now said he was resuscitated at least twice, once on the field and once again at the hospital. Does that still fit a diagnosis of commodio cardis? From what I've read of reporting pre-incident, it's not the type of issue that the heart continues to fail, but I dont know. I haven't read tons of medical papers on it. So I'm curious if multiple heart failures after reussucitation is normal for this issue, or not?

What testing needs to be done to confirm commotio cordis? Im at a loss as to why the hospital did not give an update on the type of injury sustained today. Does it take multiple days to confirm commotio cordis?
Retired medic here. He was a witnessed loss of consciousness. On assessment, he was found to have no pulse, so they started CPR, to achieve indirect, supportive output, which worked, he survived. Then they put on the AED, as per typical no pulse protocol. The heart can have various rhythms with no cardiac output. If the machine finds ventricular fibrillation, which is caused by chaotic, disorganized electrical control in the heart, it calls for a shock to be applied. The shock shuts off ALL electrical activity, in the hope that the heart's electrical activity spontaneously restarts in a normal rhythm. That is very likely with such a patient and such a well organized, rapid response. However, if a shock were delivered, it would be VERY obvious to everyone, and I haven't heard any reports of that. If no shock is delivered, the machine found something other than VF/VT (including asystole, or flatline, no electrical activity at all) and it calls for more CPR, because a shock won't do anything for those, you need drugs or a pacer. He is reported to be alive and in an ICU, no report of being paced, which suggests a return to spontaneous circulation, due to restored cardiac output. So the possibilities fall into these general categories: 1) electrical control system malfunction. i.e. Blunt force trauma electrical, (tripped the breaker)... 2) heart muscle or blood vessel damage. i.e. Blunt force trauma physical, aortic or ventricular rupture. It's rare to survive such catastrophic trauma, I haven't heard any reports of emergency surgery, unlikely. 3) No oxygen to the heart muscle. Mechanical: i.e. blocked blood flow in the coronary arteries. atherosclerosis, dislodged clot, (unlikely in a young, very fit man), mRNA induced calamari clot. (Lots of those these days) 4)Ambient: Not enough O2 in the inspired air, choking. 5) Comorbidities: i.e. pneumonia, poisoning. My compliments to the responding medical team, very well done. This worthy man has a good chance to recover, my very best wishes to him and his lovely family.

He did play HS Football and his EP recommended I buy an AED and keep it in the trunk of my car, because the H.S. did not have an AED. Crazy thinking on the part of both the EP AND the School.

I'm not sure how you can make a video so fast after it happened. This is very suspicious and would take a doctor who was present quite awhile before they could make a diagnosis.
It is greatly appreciated that you can take this horrible tragedy of an injury that millions in the US saw on live TV and be able to come on here and make a video about it very quickly to inform people as a doctor what you viewed as happening as somebody who also watched it on TV with us. We know you aren’t treating them and of course I’ve only heard that Statements released by the NFL like everybody else. But being able to hear what we were seeing from a medical professional is extremely helpful but also hopeful because we now have an idea of what’s going on and hopefully he can come through this and recover to be healthy again, regardless of his football career.
As a Paramedic 32 years service I have responded to countless sports injuries, rugby,football, combat sports/ sudden collapses,and 1000s of cardiac related collapses I have never seen a person who went into cardiac arrest ( Trauma, arthymias ( electrical or other cause) Who's had no palpable pulse get up unaided to a standing position and then collapse?( once the heart goes into cardiac arrest the heart isn't beating? ) The Brain might still be functioning and sending signals to the heart , but the heart has no out put? I am sure they will be an explanation to this, and the 14,000 extra cardiac arrests we have seen in the U.K over the past year. ( The truth will prevail )

I've watched football since I was fresh out of diapers and I am now 61 and I have never seen or felt like I did last night. Sick is an understatement of how I and millions of others felt witnessing what happened on TV. It has to do with the timing of the impact. It has to be during the upswing of the T wave portion of his heart beat. Literally a split second of timing.
This is similar to "dim mak" or "touch of death" in Chinese martial arts fiction. "Touch of death" in rare cases, death can occur in response to trauma such as Commotio Cordis, an often lethal disruption of heart rhythm that occurs as a result of a blow to the area directly over the heart.

Once again, that doesn’t necessarily mean he won’t recover, he totally could and he’s young and CPR was administered fairly quickly. So he’s got a lot of good attributes going into this.

This was so scary to experience just watching at home. I can't imagine what the team is going through, what his family is going through. His mother was at the game, and rode with him in the ambulance. I have kids close to Damar's age, and what she is dealing with tonight is my worst nightmare. Hi Mike, fellow physician here (surgeon). My Alma mater Cornell University had a situation that led to the death of a Cornell Lacrosse senior in 2004, the late George Boiardi. If I remember correctly, he was defending and took a goal shot in the chest, went into arrest and died at the medical center. Every time I watch Cornell Lacrosse if I’m reminded of this it hurts. Cornell lost to Maryland in the NCAA Division I championship game this past season with Maryland coached by no less a Cornell Class of ‘91 grad. Well, we had Richie Moran coach us to three national championships in the 70’s and two championship loss while I was there in the late 80’s and he played for Maryland. Thanks for the video. I hope we win one for George one of these days and Coach Moran who just recently passed away. Incredibly sad reminder today for me of 2004.

I ref girls lacrosse, so I’m not as familiar with boys. Are boys now required to wear chest pads? Are they the same as the goalie’s pads?
I know this is unlikely, but could this be a complication from a recent Covid shot? I’ve heard of increased occurrences of cardiac issues in otherwise healthy males after getting a Covid shot.
I ask this because the hit he took was not near as severe as many other hits players receive during the course of a football game.

@@arerki09not sure, I know the lacrosse community took major steps to improve chest protection after 2004. Don’t know what George was wearing that day. But at the highest level Division I lacrosse teams like Cornell, Hopkins, Maryland etc. these guys can shoot the ball at 100 MPH. Not sure if any body armor can stop this one in a million tragedy. I’m pretty sure goalies are mandated to wear pads. But I would assume attack, mid fielders and long stick defenders need to strike a balance between speed and protection. Good thing is body armor can be pretty light yet strong. I think best design is one which disperses the energy of a hit across a large surface area thus decreasing the energy of the shot at point of impact.

This kind of stuff is so scary and serious. I saw one just like this a couple years back at a high school football game. The kid had drank 4 monsters to get energized for the game, and he went into cardiac arrest on the field. They almost suspended the game, but continued to play about an hour later. He was immediately taken to a local hospital in our hometown and was okay. Prayers up🙏🙏 to damar and his family.

Well, here we are 5 days later and the first-hand doctors at the hospital don't buy what you are saying. They also said that more tests would need to be done to KNOW what actually caused the cardiac arrest.
I would hold off on coming to any conclusions based on watching this from your couch, because there are plenty of highly qualified doctors who think this might be related to an adverse event from an injection. Especially given the fact that CDC scientists have done a study which found 229 young folks, aged 12-24, develop myocarditis, out of one million doses of injection. Blood clots were also found among others.
So there are plenty of OTHER causes that are more probable than something that has NEVER happened in the NFL before.
I'm an RN (retired) and was at a Jiu Jitsu class, when a guy I was practicing with dropped into me after the class. I started CPR immediately with another guy (two man CPR) until the fire departement arrrived. They defibrillated him on the mat and he was ambulanced to the hospital, where he was given emergency bypass surgery. He made a full recovery with no loss of function. The only permanent damage was memory loss of the event.

Look into how many recalls there have been on Philips pads in the last 2 years vs last 10. Its almost like they are being used and inherently tested more recently.

I know the incident is a freak timing thing between the collision and when his heart is re-polarizing...but I can't believe this hasn't happened before in the NFL given the physical nature of the sport.
I wonder what underlying issues were going on. Because that initially would cause an arrest, but a healthy heart may spontaneously start again in NSR. Hopefully he makes it and recovers. May require further workup. EPS etc.
Impacts are not as violent now as they were. Helmet to chest are now illegal but were once common. His hit (chest to chest) seemed tame compared to many others I've seen. Usually, its kids who have weaker less developed chest who have cardiac arrests from hard chest hit. I'm thinking something else was involved. Heart damage to electrical system from (myocarditis?)

It wasn’t commotio cordis. Their shoulder pads provide way too much chest protection these days for that to happen. Plus, the hit wasn’t near hard enough. The only way in football for commotio cordis to occur is basically if the very tip of an elbow somehow could get past those hardcore chest pads and hit the left chest wall area in the way a softball does. Softball or something similar where a smaller object at an insane speed directly to the heart or temple is where you’d typically see something this severe. It doesn’t happen in football. If you can tell me the last time you saw something just like this in the NFL then I’d recommend you to become a sports analyst or historian. It wasn’t commotio cordis.

First thing comes to mind was maybe because of the bad weather? But then I remembered they always play on the rain & snow games. being an avid NFL fan from Australia, I have seen something similar to this almost a decade ago but in a different sporting code, as a sporting fan, it is one of the most horrifying events one could witness, you don't want to see it happen, and you want those players to make it to the next week safe and ready to play for their team.

Retired medic here. He was a witnessed loss of consciousness. On assessment, he was found to have no pulse, so they started CPR, to achieve indirect, supportive output, which worked, he survived.
Then they put on the AED, as per typical no pulse protocol. The heart can have various rhythms with no cardiac output. If the machine finds ventricular fibrillation, which is caused by chaotic, disorganized electrical control in the heart, it calls for a shock to be applied. The shock shuts off ALL electrical activity, in the hope that the heart's electrical activity spontaneously restarts in a normal rhythm. That is very likely with such a patient and such a well organized, rapid response.
However, if a shock were delivered, it would be VERY obvious to everyone, and I haven't heard any reports of that.
If no shock is delivered, the machine found something other than VF/VT (including asystole, or flatline, no electrical activity at all) and it calls for more CPR, because a shock won't do anything for those, you need drugs or a pacer. He is reported to be alive and in an ICU, no report of being paced, which suggests a return to spontaneous circulation, due to restored cardiac output.
So the possibilities fall into these general categories:
1) electrical control system malfunction. i.e. Blunt force trauma electrical, (tripped the breaker)...
2) heart muscle or blood vessel damage. i.e. Blunt force trauma physical, aortic or ventricular rupture. It's rare to survive such catastrophic trauma, I haven't heard any reports of emergency surgery, unlikely.
3) No oxygen to the heart muscle. Mechanical: i.e. blocked blood flow in the coronary arteries. atherosclerosis, dislodged clot, (unlikely in a young, very fit man), mRNA induced calamari clot. (Lots of those these days)
4)Ambient: Not enough O2 in the inspired air, choking.
5) Comorbidities: i.e. pneumonia, poisoning.
My compliments to the responding medical team, very well done.
This worthy man has a good chance to recover, my very best wishes to him and his lovely family.

He did play HS Football and his EP recommended I buy an AED and keep it in the trunk of my car, because the H.S. did not have an AED. Crazy thinking on the part of both the EP AND the School.

If you are questioning the school and the doctor regarding playing contact sports then the easiest decision as a parent is to not let your child play.

I'm not sure how you can make a video so fast after it happened. This is very suspicious and would take a doctor who was present quite awhile before they could make a diagnosis.
It is greatly appreciated that you can take this horrible tragedy of an injury that millions in the US saw on live TV and be able to come on here and make a video about it very quickly to inform people as a doctor what you viewed as happening as somebody who also watched it on TV with us. We know you aren’t treating them and of course I’ve only heard that Statements released by the NFL like everybody else. But being able to hear what we were seeing from a medical professional is extremely helpful but also hopeful because we now have an idea of what’s going on and hopefully he can come through this and recover to be healthy again, regardless of his football career.

As a Paramedic 32 years service I have responded to countless sports injuries, rugby,football, combat sports/ sudden collapses,and 1000s of cardiac related collapses
I have never seen a person who went into cardiac arrest ( Trauma, arthymias ( electrical or other cause)
Who's had no palpable pulse get up unaided to a standing position and then collapse?( once the heart goes into cardiac arrest the heart isn't beating? )
The Brain might still be functioning and sending signals to the heart , but the heart has no out put?
I am sure they will be an explanation to this, and the 14,000 extra cardiac arrests we have seen in the U.K over the past year. ( The truth will prevail )

Sick is an understatement of how I and millions of others felt witnessing what happened on TV. To the medical personnel and first responders that reacted so quickly to Damar's emergency...Thank You!!!! Everyone of you gave him a fighters chance to pull through this and hopefully make a full recovery. I'm not sure I can ever watch football the same again. I honestly believe this is one wound time will never heal for me.

Wouldn’t this happen often with lineman then? They get hit in the chest constantly for hours in a game and for months of the year. Wouldn’t it be more probable for it to happen to them but it never does. Something doesn’t add up doc
People go outside in thunderstorms everyday, doesn't mean that you are guaranteed to get struck by lighting every time.

It has to do with the timing of the impact. It has to be during the upswing of the T wave portion of his heart beat. Literally a split second of timing.

This is similar to "dim mak" or "touch of death" in Chinese martial arts fiction.
"Touch of death" in rare cases, death can occur in response to trauma such as Commotio Cordis, an often lethal disruption of heart rhythm that occurs as a result of a blow to the area directly over the heart.
