Partial or complete quadripedalism and may emit animal-like vocalizations such as howling, roaring, ect. 5 - Just an animal. Lives purely by instinct. These research findings are by no means concrete and may change with further peer review and field study.
I would say with 4 it's a bit like the human nature now also shares space with the more animalistic nature, and may act out of both natures depending on the individual's personality/mental maturity. But for the sake of grading it on a scale without knowledge of the personality, I'd say your method is sufficient!

But who is to say that the head and body are on equal proportions? The head is only like 8% of the body weight, but it is also where the emotions and soul comes from so should it be 8%, 50%, or more % due to that?
