Hi! Those fields are generally where people write each check number they are depositing (the check number can be found in the upper right of the check).

I've used dep slips for years long ago I think I'd forgotten because they're so seldom used now. I only got interested because I have a tax refund and my credit unions a long drive.
you fill cheques out in the same way, yes and you generally would pay in a cheque at a bank branch, using either the machine or going to the counter, though many banks in the UK at least and likely some other countries too, let you use the bank's app to take a picture of the cheque and pay it in that way
