    I'm fed up with rollercoaster blood glucose histories. And I agree with Dr. Bernstein's practice of low carb replacing it with protein or else to keep the blood glucose in check.
    The issue is the wide spread bad eating habits of high carbs. I'm also guilty of it and struggle to prepare and look out for meals that are low carb.

    A low carbohydrate diet is excellent in my experience. However as a lifetime diet this is extremely challenging on a daily basis.
    When I try to reintroduce foods with some carbohydrates, it never ends up well. (After all I’ve been on it for a year hoping for results..) I also fear a keto diet, as amazing as it can be to reduce or get rid of insulin usage, is just a crutch, a symptom reliever. The underlying autoimmune response is ever present. I really hope more research goes into this.
    People with T1D have the worst quality of life compared to any other chronic diseases

    I have experienced the wonders of low carb on my blood glucose as well. However, I’m an awful and lazy cook, so I always end up eating something that isn’t low carb because I don’t know what to eat for dinner or whatever after a few days.
    I’ve just come to the conclusion that “falling off the wagon” is just part of the learning curve. I have to learn to eat low carb, after all, I’ve never done it in my 30 years of life. So “failing” is just another lesson for me to look at, as to why I “failed” that specific time.
    My blood sugar will be poor at that moment, but overall it better than it has ever been. I’ve chosen to look at the positive progress rather than focus on the failures.

    PS: reviewing your graph, it looks as though in this example the patient was receiving a high basal dose designed to partially "cover" meals, in the way that type 1 was treated 40 years ago: I was on 2 doses of semilente for many years. Still alive and kicking lol. Minor miracle I reckon!!

    Excellent talk. But I am astonished that you state a 2 unit bolus will "cover" 60g CHO. This is absurd. I have lived with type 1 for 54 years: mild background retinopathy is the only complication I suffer from, and that has been stable for the last 15 years. I need one unit of novorapid to "cover" 5g CHO, with basal levemir dosage 9u diurnal and 6u nocturnal, at 12 hour intervals, plus 2u novorapid on rising to treat dawn effect. I am greatly heartened at your praise of Dr Bernstein. He has, for so long, been a light in the wilderness! The diabetes UK and ADA guidelines are scandalous, as has been the demonisation of saturated fats and promotion of statins by the cardiologists. But will they admit that they got it wrong? One or two might, the rest stay quiet till people forget. Tragic for the hundreds of thousands whose lives have been devastated by bad advice.

      for some, 2 g is plenty to cover 60 g carbs. for example, those who are vegan whole foods anecdotally report a ratio of 1unit/30g, and those who have recently been diagnosed and are still in their honeymoon

      I agree, 2ubof novorapid wouldn't even touch the sides with 60g of carbs. I would need way more than that like yourself!

    What about prolonged fasting and its ability to reset the immune system? Could it be the key to stop its tendency to attack the pancreas cells and start anew?

      Fasting does not reset the immune system. You don't loose the protection from a vacine after fasting and you won't loose antibody's against your beta cells.

    Government funded study for LCHF would result in changing of dietary recommendations- health people don’t need drugs- drug companies won’t like that.

    This is a great talk but I would like to point out 32:00 I belive that this is very different from hypoglycemic unawareness which is dangerous because the body is not giving you the warning signals while it's starving & depleted of energy. When in ketosis you do not get the warning signals at the lower blood sugars because the body can be sufficient in energy with lower glucose and higher ketones & fatty lipid metabolism therefore no hypoglycemic symptoms . I do keto and occasional prolonged fasting up to 7 days. One day I was doing a prolonged fasting and deep into ketosis , my blood sugar was hovering between low - 2.5 mmol for a period of 24h ( stable) I was feeling great no symptoms and I didn't feel the need to raise the sugar levels. So Now you can point out I am hypoglycemic unaware? Absolutely not, when I'm in lower ketosis or out of ketosis I do feel the hypo symptoms from 3.6mmol which is very normal in a healthy non diabetic with the standard diet . It's interesting to know that Healthy New born babies are born with low glucose levels around 2.5mmol yet have elevated ketone levels this is a whole big topic by itself but the bottom line is if glucose levels are low yet the elevated keton levels are adequately sufficient to fulfill your energy needs your body is perfectly happy and shall not beg or show symptoms of low energy levels

      Another famous case is that of Angus barbieri. The longest prolonged fast world record. His sugar levels were at 1.7mmol yet was functioning perfectly. I would be interested to see a study that can up with a suggested healthy ketone vs glucose balanced ratio and whether it can apply to all populations

      @@mapleleaves2289 You have to understand how to relativize this phenomenon. Let's compare it to a professional athlete, endurance most likely e.g. cycling. Pro-athletes benefit from extra carbs, because of their extreme need for fast acting energy caused through the stress hormone cortisol created during such activity when performing/doing professional sports. You have to understand that the case of Angus Barbieri is another EXTREME, it is not the standard. Thus 1.7 mmol can work in theory, it might only be beneficial or occurring for the extreme amount of the population as a whole.

    Doctors have been telling me for years I can eat whatever i want and adjust my insulin dose accordingly abd that low carb diet is dangerous because it causes hypoglycemia, which is a clear evidence these doctors know nothing about T1D, they just studied about it. Anyone with type one diabetes knows that hypoglycemia is a frequent occurrence when you take carbs and insulin on daily basis. I used to have hypoglycemia every single day until I started low carb diet. These doctors are clowns and I advise every single diabetic out there to research and try things on their own until you know what suits you best because trust me most of these sophisticated doctors know nothing about the real deal, they just memorize theories in their books then pass the tests.

      Hi, Dima. I agree with you. I live in Paris, in France. And my doctor told me that low carbs is dangerous because it provokes Ketosis! ...My wife told that my doctor knows nothing about diabetes except what she learnt at university which is theories! I really want to start low carb diet but I don't know how to start!

    I diagnosed w/ Diabetes Jan. 2018 at the age of 66. Not until then did I learn that Carbs raise Glucose. I immediately put myself on a Low Car Diet. My Diabetic Dr. kept telling me I need carbs for energy. I finally said: But I don't lack energy. My energy level is high eating low carbs. Eating low carbs and low sugar has kept my Diabetes in check. I am opposite of most. Too thin, can't gain weight. I'm trying to figure out how to gain weight with Diabetes. 1 Dr. says I'm Type 2. Other Dr. says I'm Type 1.. Grrrrr

      You can do lchf and gain weight just add more calories. Oils and nuts shall do the trick. I am lchf 22years old type 1 60kg stable weight

      Sounds like your suffering from the immune response in T1D. What I realised for myself is that a keto diet relieves symptoms. The underlying immune response is still there lingering. I’ve done keto for a year (0 carbs) and I have a relatively good blood glucose profile with C peptides at normal levels. Doctors are confused. It’s very frustrating.

    Does your
    brain need sugar, when I was on keto I was at or near zero sugar, I am now
    eating a few blue berries or strawberries with nuts. Any advice?

      Your system will perform gluconeogenesis to provide the glucose your brain needs.

      The liver produces all the glucose you need and even more. Did you ever wonder why type one diabetics take a basal Insulin dose? Even if you eat 0 carbs your liver is providing you with all the glucose you need and even more.

      Healthy Keto is not 0 carbs. By the way there is a difference between net carbs which most keto people look at and total carbs. Fiber and fat subtracts & delays the total carbs but doesn't eliminate them all the way. In ketosis your brain can indeed rely mainly on ketones for brain fuel but there is almost always a percentage fueled by gluconeogenesis even if you take 0 carbs in diet . Note that The body can't function with 0 glucose since RBC don't have mitochondria to utilize ketones. To summarize keto is not 0 carbs, if you eat 0 carbs your brain will rely more in percentage on ketones, we can't live with 0 glucose in blood, your liver will produce all the glucose you need no need to have it from diet
