I disagree. The biggest "what if" in NBA history, in my opinion, is what if the Rochester Royals selected Bill Russel first overall in the 1956 draft.
Let me explain. First and foremost, obviously, the Celtics don't win 11 chips, although they still might get a handful (more than most teams even to this day). Second, the Royals wouldn't be bad enough to land the first pick in 1960 - which means Oscar never goes to the Royals. Now, there are conflicting stories about why Oscar left the Royals, but the one I have personal anecdotes to believe, is that Bob Cousy was jealous, and the two didn't see eye to eye. But if Oscar was never on the Royals, he may never leave whatever team he ended up on, which means he'd have no reason to sue the NBA, which in turn, means that there would be no free agency.
Obviously, free agency itself is reason enough, but there are more ripples
One of Free agencies biggest impacts, was the league's spitefulness about it near it's beginning. In 1976, Gail Goodrich became a free agent, and the New Orleans Jazz signed him, however, for some brain-dead reason, the league decided that even though a player was a free agent - not signed to any team - the team they came from had to be compensated. Even worse, the Jazz weren't the ones who got to decide what they gave up for Gail - the league arbitrated and decided to take three first round picks. And in order to get those three first round picks, the Jazz had to give up the pre-merger draft rights to Moses Malone in exchange for one, which they used to get Gail. Eventually, in 1979, one of those picks that was essentially stolen from the Jazz, ended up becoming Magic Johnson
As great as Kareem was - he wasn't good enough to win a chip without the help of two guys who during their career and for some, to this day, held the title of goat point guard. If The picks aren't stolen from the Jazz, they have a core of Magic Johnson and Moses Malone - and very likely never go to Utah.
If the Jazz has that team, Malone never joins the 6ers (side note, Moses to the 6ers is the most BS lopsided trade I have ever seen, reigning MVP for one mediocre-at-best starting center and one first round pick who turned out to be an okay role player). Which means Dr.J doesn't win a chip in the NBA (most likely, although they were in the finals the year previous, so who knows).
There are plenty more threads to follow here, but these are some of the bigger ones.

what happened to rose is tragic, all star name, all star face, all star highlight but he rely too much on athletism. so when he lost it due to injury he declined even tho he remained healthy.
westbrook, lebron are also very athletic but they do know how to break a fall to avoid major injury to their legs/knees. drose throughout his career, stubbornly insist on letting his knee absorb all the impact.
do the torn acl part is a matter of sooner or later. ja morant have the same problem with drose with bad landing technique.
i think brandon roy and grant hill would have been a bigger what if because they can't even stay healthy or play a game without pain hindering them.

Kawhi leonard is the biggest "what if" in NBA history.
His prime is riddled with injuries.
DRose didn't accomplish anything but MVP. He played 15 years!!!

by your own description Rose is the bigger what if. Kawhi still won 2 rings and he carried the Raptors to the first Canadian team winning the NBA championship. In a league where it's all about getting the bag and getting a ring, Leonard did both (and has been in the league for 13 seasons so far).

@@maxmeier532 DRose, Grant Hill, Brandon Roy and so few more are what ifs but not the biggest what ifs cos they never got to their prime and we never knew how much they could possibly do.
Kawhi, Bill Walton are the biggest what if cos we see how great they could have been cos they showed they can win but are limited to due their injuries. Drose and Co never proved anything so they could probably be a Melo guys who are so good but never won

I understand why he hasn't a long career, he was too much talented and has to much his body, unfortunately, that's shame, I would see that even on the knicks he was holding himself
