I think they should add distraction mechanics along with the stealth as well. (For Example, The last of Us 1&2 has bricks and bottles that can be thrown to distract and lure enemies.) Also, rockstar could use the whistle sfx from RDR2 (in GTA:VI) to make enemies come closer to you for a clean/quiet kill.
In a game where there is a system to chase you relentlessly based on your level of unhinged at any given moment, it's not surprising the stealth system is abysmal. Cayo has never felt thematically connected to 5 to me, it feels like something that should be in the Caribbean amongst the two dozen or more larger islands within 600 miles of south Florida (6 minutes or less in game travel time, give or take) . Beside that they don't want us grinding it group or solo, all they've done is add to an inflated economy with time and money drains that may keep the above average player somewhat interested every so often while burying newer players with 200-500 hours of grind and distractions depending on their level of commitment, or lack there of because live service bait, shark cards and gta+. 6 may be better build for longevity but I don't think they'll lean into the stealth aspect other than bait that ends in full blown shoot outs, I think we can expect ubisoft or worse levels of stealth.
A better mechanic than difficulty levels would be AI that adjusts to the level of the player with a max point. That way the better you are the better the NPCs would be.

The whole gunplay just needs to be like red dead 2
