it looks like the offshoot is coming from the rootstock of the tree and not the top part. I'd check down if it's coming from below the graft point before making a decision.
When it comes to the pear tree 🌳, keep both, root the branch you want to cut, then cut it once it has roots, it's not that hard to do, watch a couple of RUclips videos and learn a new skill, I'm about to do this with a star fruit tree.
That offshoot is likely coming from the root stock. I would hack it off.
