The only reason the fish is acting as the stem is because that fish used to be the actual stem. The fish kept the stem's sprite ID, thus why it's acting as a stem.

1:40 according to the cutting room floor, this is supposed to be the original piranha plant, but was ditched in favor of the floating one, my best guess about the fish is that it was supposed to be the vine for the piranha, but was overwritten by an upside down fish texture
The upside down fish is actually in the hex where an upward stem would be for pirhanna plants if they acted that way in game. The only place they have vines is one level where they come down, so rather than waste space they put a new sprite where the unused upward vine would be
The black&white rom hack wasn't that bad, alongside with "Puzzle Piece", Windows Mario World, and "collect the chicken" rom hacks. (5:20) Know how SMRPG Seven Stars and some other Mario games have different names for different Yoshis? That's why.

***** You need to set hotkeys to do that,and there's a new corruptor that runs on Bizhawk,it's called Real Time Corruptor,you can corrupt the game while it's running,also you can race against the corruptions if you enable the auto-corrupt,Vinny used this corruptor in 5:33.
