Introduction to Antihistamines

  • Everybody knows what allergies are, but what causes them? What drugs do we have available to address them? As we learned in the immunology series, allergies are symptoms of an immune response to a perceived threat. Histamine is a molecule in the body that orchestrates many aspects of this response, and antihistamines are a class of drugs whose purpose it is to neutralize the effects of histamine in the body. Many of these are over-the-counter drugs which should be familiar to us, like Benadryl, Claritin, Zyrtec, and Allegra. How do these drugs work on the molecular level? Let's find out!

    I'm an epileptic and I noticed a pattern that most of my seizures occurred during the spring and I eventually learned that antihistamines can lower the threshold for seizures, so I stopped taking them. I haven't had a seizure since.
    It was interesting to learn about the lipophobic antihistamines so I now know that they have a tougher time crossing the blood brain barrier they are a safer alternative for me.

      Benadryl can be nasty, even have habit forming properties that I've seen happen to close people. Didn't know it can lower the threshold for seizures too.

      Alas, you're talking about an average cross section of society being interested in science and knowledge.. not going to happen.
      I feel like the internet has been taken over by morons. Back in the 90s when it was difficult to use a computer and the internet was far more niche - you were called a nerd or geek for being into computers,. Now that we all use them they've been dumbed down to allow it for everyone. Not sure if that's a good thing or not.
      There's still good stuff out there, like this channel and many others - I have no idea why the top youtubers have any following at all. It baffles me that people want to watch swag, reaction, crying people, teen girls doing makeup, influencers - the general popular junk. And everyone has lost their attention span so badly that they now need it delivered in a few second chunks on Tik Tok. Never used it - never will.

    It's interesting how the same brand names are used for different drugs around the world. Here in the UK, Benadryl is mainly acrivastine, while diphenhydramine is often found in brands of chesty cough mixtures like Benylin

    Hey man I really value your opinion. I work in the sports supplement industry rn part time and I have been researching a lot about some stuff. Seems like there are some things that have very clear benefit like protein Creatine and citrulline and then some snake oily type things like test boosters and fat burning stuff.
    With your history in biochemistry and your love for debunking things I would love to see some of your ideas on the difference between these and I think it would make for a great video

      Professor Dave has already done a number of videos on these things, including some good debunks (just do a search on his channel for them). Also, consider reading his book, "Is This Wi-FI Organic?" which covers these topics in a brief, informative way.

      @@ProfessorDaveExplains I tend to jump into topics you talk about in my own time and I’ve always looked at more practical applications and not the actual chemistry of it. Like for example I read a couple pieces about benefits of electrolytes and a lot of these products I sell have alkaline ph balance and your water videos enlightened me and led me to my own research and it’s like, unequivocally fake. So that’s why I would love your take on some of these supplements. Thanks for responding.

    I helped write materials for a course on medication management for seniors. If I could only take one thing away from that work, it would be that people over 65 should generally stay away from first generation antihistamines because of those anticholinergic effects.

    As someone who has chronically used Zyrtec for more than 20 years, this drug should never be use prophylactically for extended periods of time.
    The side effects / withdrawal symptoms of getting off this drug for some people are worse than anything you can imagine. I would envision it rivals the withdrawals from opioids. It can create a histamine cascade response that causes itching all over your body on a level that is psychologically traumatizing. However bad you are imagining, it is worse than that. Yeah, seriously, that bad. It can persist for weeks or even months.
    I have never been able to stop using this drug because of this, and I wish someone had informed me this was possible before I started. I would not wish the withdrawal symptoms of this drug on my worst enemy.
    I posted another comment about this here, but for some reason it was scrubbed. I am hoping I can save someone else from these serious side effects. Antihistamines are a great technology, but they should absolutely never be suggested for prophylactic use on a regular basis.

      Damn dude.. I used Zyrtec for colds as a child a lot and it’s taste was pretty addicting.. but I never knew extended use fucks you up that bad…

    Do you know the mechanism behind diphenhydramine that can cause excitation in certain people? I think it was common in people who had a very high level of CYP2D6 activity. Would that mean one of its metabolites cause stimulation?

    My life is heavily effected by Histamine I have several conditions that effect my life with it, 1 Asthma 2, Dermatographism aka as its simple name Skinwriting and 3 Mastocytosis a Mast cell activation syndrome, My day can be really bad with itching and pain to hives swellings and random Anaphylaxis blood pressure drops and a whole range of other symptoms mentally and physically

    Cetirizine makes me drowsy & sleepy everytime I take it while Diphenhydramine HCl does not. Weird!

    I'm an allergy sufferer, and I work in a place where there is lots of dust in the air. Wearing a mask is impractical due to the physical nature of the work. I find that OTC antihistamines don't work for me anymore and I don't want to take drugs every day in case I *might* have an attack on any given day. I don't have attacks every day. I wonder what else there is out there for people like me.

    Unfortunately our medicine industry focused on inhibiting the symptoms rather than treating the cause, my opinion it’s increasing the immunity by taking quercetin will stop immediately the whole allergy story.

    Great stuff here but maybe try to make the information a little more digestible. It seems your purposefully using lesser used words, which makes me rewind as I’m listening to this whole doing other tasks. Example: efficacious, just say effective lol.

    I'm allergic to all metals & only certirizine works for me, I take it every day. It's not the worst allergy to have but it's impossible to avoid metal...I've tried! 🤷‍♀️🤦‍♀️

    I'm lactose intolerant. Does that count as an allergy?

      My understanding is that people who are lactose intolerant do not have the enzyme lactase, so they are not able to digest lactose. So being lactose intolerant does not involve the immune system at all and therefore is definitely not an allergy. However, being allergic to milk is also a condition that exists. So there are at least two different possible reasons why a person might not be able to drink milk (I guess there is a third type which is galactosemia but that's kind of rare).

      Lactose intolerance is when you do not have the enzyme lactase. This condition does not involve the immune system, therefore it is not an allergy. However, being allergic to the protein in milk is also a condition that does exist. So there are multiple possible medical reasons for not being able to drink milk. Another type is galactosemia which is much more rare and serious.

      Antihistamines won't work for lactose intolerance. That's due to the lactose not being broken down and thus gut bacteria using it as a good source. It creates gas and causes osmotic diarrhea. Basically it happens in the gut where histamine isn't a player. Use lactose free products or take lactase (enzyme) tablets before eating any dairy that causes you problems.

    Before watching this video: I know a bit how the body works 🙂.
    After watching the video: I have learned something but I still know nothing about the body 😦.
    How the hell can anyone even learn anything about the body if that's the level of precision shown on RUclips 😦? (I'm assuming you're skipping tons of details that would make the videos last hours instead of 10 minutes.)

      @@ProfessorDaveExplains I've started to watch some of them and plan on finishing them but following "Dunning-Kruger logic", I have a feeling that even though your videos are extremely helpful and concise, it'll take a long time before anyone watching only your channel as a source of knowledge will have a sufficient level of expertise to say "I know how the body works" 🤷‍♂️.
      Maybe it's wrong to think this way, but I'd rather think that I know nothing (very little) about a subject instead of thinking I know a lot about it.

      This is bizarre logic. Your solution to not knowing enough is to avoid learning things? The more knowledge you have the better. That you won't become a leading expert in a field by watching my tutorials doesn't negate the value of the information. Just know as you go that your knowledge is not complete, as all humans should do at all times.

      And so what if he doesn't believe in god and only science. That is his right as a human being. He doesn't have to believe in god if he doesn't want too. Science is backed up by facts, God isn't. Religions indoctrinate people into believing what they want you to believe in without facts. They tell you to believe in god with no questions asked. That's called willful ignorance. Dave is an atheist btw, so he won't side with you on this one lol
