Those who can get stronger unconsciously adapt "chuunibyou" personality as form as self-affirmation mechanisms to survive and gets stronger (mentally). And those who cant, obviously they re just reconcile with people and living among them (and maybe can be chuunibyou too someday if they suddenly gets potential to be feared)
Makima is written so perfectly that even if you know she has done something bad you can't help but think it's for something good because of how she is set up as someone Denji and the reader can look up to seeing as she has been helping defeat devils. She has us all wrapped around her finger. Even after I finished CSM i still couldn't help but think Makima did this for something good. That's how strategically written she is and Fuji did a damn good job. Even the first time we meet her you can tell something is off and when she does something like that (when she killed those prisoners and regenerated from the civilians on the train) you know she just did something evil, but you also start to think it's for the greater good bc thats how controlling she is. its actually so perfect im just astonished. its up there with eren jaeger.

Unlike psychopaths sociopaths are defined as being impulsive, hot headed, and prone to violence and have a limited conscious but irresponsibly give a poor excuse to supress the guilt and can develop close relationships with people. Makima didnt have any of those traits like a Psychopath which is a person who is intelligent, cold, and minipulative. She was also a low key sadist and mashochist when you pointed out how she smiled everytime she hurt or killed someone and she guninely looked happy when she was fightning pochita in his true form when she got chained up by him and said she was fine with getting eaten by him

This guy will psychologically fucked anyone and he don't need any supernaturally powers as well. I also recommend Monster, while slow paced the story is interesting but just like Makima,Johan is a psychopathic villain that made a judge impact on the story.
I think you actually missed one part of plot that made you believe that Makima tricked prime minister of Japan. Makima was always on a short leash of government, not the other way around. When she said "they are scary" she meant it. When she said that "Government knows how to keep evil on a short leash", she really meant it. All that time Makima was working for them, so they could grant immortality to her in human world. The reason why US president was aware of what's going on, and prime minister somehow didn't knew all of this, is because he did in fact knew all of it. Prime minister agreed on this plan with Makima and allowed to her to do what was needed to do in Japan best interests, not US or world interests. She protected Japan from Gun Devil during Gun Devil ark, she protected Japan from all sorts of threats by leading Public Safety. She was granted immortality for all of this, which allowed her to continue her plan without risks.
I don’t think Makima is narcissistic. Narcissists are insecure and constantly in need of validation. I don’t think Makima is either of those things. She doesn’t seem insecure at all, and she doesn’t constantly try to show/tell everyone how great she is, as narcissists tend to do. If anything, she seems to downplay what she’s capable of. Most people misunderstand what narcissism is. Narcissists aren’t really arrogant or in love with themselves. They put on a facade of arrogance to mask their feelings of inadequacy. A good example of a narcissist would be Zap Brannigan from Futurama.

Reason being that that sociopaths are made while psychopaths are born, we can see this later with the Nayuta version of her which still retains some of her Makima tendencies. Poochi even later confirms that by saying that Makima always wanted a family, much like Poochi being too strong when he was Chainsaw man and couldn't get a hug because of how menacing he was so is Makima unable to have a family with people she deems beneath her, and by default being the demon of control and a demon puts humans below her, kinda like long lived elves in fantasy are depicted as arrogant and all knowing snots because of their unatrual life span.

being a psychopath falls under ASPD and therefore cannot be "cured". However, just because someone is a psychopath does not mean they are criminals. Makima would just require a different upbringing to prevent her from murder/crime.
