  • Ivermectin for Prevention and Treatment of COVID-19 Infection
A Systematic Review, Meta-analysis, and Trial Sequential Analysis to Inform Clinical Guidelines
    journals.lww.com/americanther...
    Background
The antiparasitic ivermectin, with antiviral and anti-inflammatory properties, has now been tested in numerous clinical trials.
A 2018 application for ivermectin use for scabies gives a direct cost of $2.90 for 100 12-mg tablets.
Most trials were registered, self-funded, and undertaken by clinicians
    We assessed
Efficacy of ivermectin treatment in reducing mortality
    Chemoprophylaxis
    Data sources
Databases up to April 25, 2021
Sifted for studies, extracted data, and assessed risk of bias
Meta-analyses were conducted and certainty of the evidence was assessed
    GRADE approach
    training.cochrane.org/grade-a...
    bestpractice.bmj.com/info/too...
GRADE (Grading of Recommendations, Assessment, Development and Evaluations)
Reproducible and transparent framework for grading certainty in evidence
100 organisations worldwide officially endorsing GRADE
GRADE has four levels of evidence, certainty in evidence
    very low
    low
    moderate
    high
    Decreasing confidence
    Risk of bias
    Imprecision
    Inconsistency
    Indirectness
    Publication bias
    Increases confidence
    Very large magnitude of effect
    Clear dose-response gradient
Residual confounding is likely to decrease rather than increase the magnitude of effect
    Data sources
    24 randomized controlled trials
    N = 3,406 participants
Ivermectin reduced risk of death compared with no ivermectin
    Meta-analysis of 15 trials (n = 2,438)
    Average risk ratio 0.38
    Moderate-certainty evidence
(Confirmed using DerSimonian-Laird method and Biggerstaff-Tweedie method)
Ivermectin verses no ivermectin in hospital patients
    Ivermectin, 2.3%
    No ivermectin, 7.8%
    Ivermectin prophylaxis reduced COVID-19 infection
    3 trials, n = 738
    Average reduction 86%
    Low-certainty evidence
(due to study design limitations and few included trials)
    Clearly favored ivermectin use
    improvement
    deterioration
    Severe adverse events
    Rare among treatment trials
Evidence of no difference was assessed as low certainty
    Conclusions
Moderate-certainty evidence finds that large reductions in COVID-19 deaths are possible using ivermectin.
Using ivermectin early in the clinical course may reduce numbers progressing
    to severe disease.
The apparent safety and low cost suggest that ivermectin is likely to have a significant impact on the SARS-CoV-2 pandemic globally.
    Current NIH recommendations
    www.covid19treatmentguideline...
there are insufficient data to recommend either for or against the use of ivermectin for the treatment of COVID-19
The sample size of most of the trials was small
Various doses and schedules of ivermectin were used
Some of the randomized controlled trials were open-label
Patients received various concomitant (confounding) medications (e.g., doxycycline, hydroxychloroquine, azithromycin, zinc, corticosteroids)
The severity of COVID-19 in the study participants was not always well described
The study outcome measures were not always clearly defined

I was asked by the doctor at the clinic if i would take a pescription, and i said yes, as long as its not something polititions are pushing. I was given one for an antibiotic and one for a two dose Ivermectin. After the second dose of Ivermectin my symtoms vanished completly!!!

Anecdotal is not empirical. My doctors use empirical, not anecdotal.
the problem is Ivermetcin in small doses isn't going to do anything. Your antibiotics probably did more than Ivermectin.
It's not going to work that fast. You are either lying, OR, you might not have had Covid or not a very severe case of it. Being tired, loss of appetite, head pain and congestion is not the only symptoms of Covid. Are you sure you didn't get a false positive test result? That can happen. Which test did they do?

Even if there's insufficient data, there's virtually no downside to using it at all. It's dirt cheap, already used all over the world and seems to have some positive interaction of a variety of levels from the docking component to protease inhibition. The saying of never ascribe to malice what can be explained due to stupidity doesn't apply when there's hundreds of billions of dollars on the line for companies that have knowingly bought up most of the worlds governments influence.
The same companies that shut down HIV treatment medication production in Africa in the 80s because they were using their intellectual property to produce a no name version of the meds for 1/20th the cost (at no profit). They were willing to let tens of million of Africans die over a patent they barely even used in the first world. What makes anyone think these companies who have the largest class action lawsuits of all time over death/complications from medications against them, should be considered trust worthy in regards to their incentives is beyond me.

I've been taking vitamin D and k2 for years, since the pandemic I added vitamin C and zinc.
A few days ago I developed a cough, I did wonder about it but didn't give it too much thought. By night time I also started to feel short of breath.
The next morning I woke up with quite severe chest pain and difficulty breathing, I panicked a little as it felt very serious. I had ivermectin on hand that I bought months ago, so I took 18mg.
Throughout the day the chest pain persisted, breathing was difficult, I kept having to lift my arms up over my head and breathe deeply to feel like I was getting enough oxygen.
I went to bed early, when I woke up the chest pain was all but completely gone, breathing was normal, and the cough was gone. This was 17 hours after 1 ivermectin dose. About 4 or 5 hours after that, chest pain was now totally gone. 1 dose of ivermectin, all symptoms gone within 24 hours.
To be clear, I have no idea if what I had was covid, but I had a very serious respiratory issue, I had never had chest pain and trouble breathing like that before. And 24 hours after 18mg of ivermectin it was gone.

Not only that, but most recommended drugs used for infectious diseases did not meet a "high quality evidence" threshold. Here's what one doctor who is familiar with the grading systems said:
""Any physician walking around with the belief that infectious diseases is a specialty or clinical area guided predominantly by ‘high quality evidence’ is living in a fantasy world. Go through the Sanford Guide. Take note of the references that inform up-to-date recommendations. Examine the makeup of IDSA's guidelines and recommendations. It's predominantly guided by limited small RCTs at best, observational human studies and expert opinion most frequently.
"Reputable and authoritative medical consensus-making entities/professional medical societies have systems of scoring recommendations. G.R.A.D.E. for example. 1=informed by high quality randomized db trials. 2=RCTs with important limitations, or cohort studies. 3=case controls or limited cohort studies 4=case series, descriptive studies 5=expert opinion. A=strongly recommend B=recommend. You can have 2A, 2B, 3A, 3B, 4B, etc. So, strong recommendations based on low-on-trial-hierarchy studies.
"What else would yield a strong recommendation if study design isn't strong? Safety, magnitude and precision of effect, reproducibility, cost, effectiveness of alternatives, societal impact of effective treatment, severity of illness vs risks/costs/burdens of treatment. Most guidelines/recs are 2A or lower. 1A represents "best practices" and "standards of care." Sweeping practice changing recs that you had better have a good reason NOT to do. Stroke, heart failure care, cancer/screening, etc, have such recs. Infectious disease management, except for pneumonia, bacterial sepsis, UTI, and some others, has few.
"Somehow, the brain of medicine at large has been damaged, and we all think 1A level data is meant to be used as a threshold to adopt a practice at all. And the notion that a pandemic should not impact this? Pure unprecedented idiocy." -Eric Osgood MD, 01/15/2021

To begin, the principal authors of that study deny conflicts of interest (All medical authors are required to declare any conflicts), but are in fact affiliated with the organization BIRD (British Ivermectin Recommendation Development) that has loudly campaigned for the approval of ivermectin for Covid since last year. This fact does not invalidate any of their data, but makes it unlikely they would ever publish anything opposing ivermectin, and could bias their interpretation of data.
The Bryant study is a meta-analysis, a type of study that does not provide any new data but combines data from multiple other selected studies to develop a single conclusion with greater statistical weight. The trouble with meta-analyses is that the combined data from multiple poorly performed studies does not magically create a high quality conclusion, even if that might seem to be true on paper. “Garbage in, garbage out” is a common criticism of some meta-analyses.
Indeed, the authors admit that some of the studies they use in their meta-analysis are weak, because of lack of peer review, potential bias, small numbers, uncertain endpoints, variable treatment regimens, and other concerns. Three studies that they evaluated were thought superior in terms of “low risk of bias,” according to Bryant, but all three failed to find any benefit from ivermectin.
Bryant et. al. also used the large Egyptian study by Elgazzar in his analysis, that claimed the most impressive clinical benefit (90%) from ivermectin. Subsequent to the publication of their meta-analysis, though, that Egyptian study was withdrawn by its publisher for “ethical concerns” after significant evidence of plagiarism, fraud, and manufactured data. Without the imagined impressive results from that study, the conclusions of the meta-analysis would have been far less impressive.
Since the Bryant study, two other meta-analyses of ivermectin have been published with slightly different inclusion criteria. “Ivermectin for the Treatment of Coronavirus Disease 2019: A Systematic Review and Meta-analysis of Randomized Controlled Trials,” by Roman, et. al., concluded that “Compared with the standard of care or placebo, IVM did not reduce all-cause mortality, length of stay, or viral clearance,” and that “IVM is not a viable option to treat patients with COVID-19.” Another meta-analysis by the Cochrane Group, considered the gold standard for systematic evaluations of medical evidence, concluded there was “no evidence to support the use of ivermectin for treating or preventing COVID-19 infection.” Specifically, it reported, “The Cochrane review cannot confirm whether ivermectin (administered in hospital or as an outpatient) compared with placebo or usual care, leads to more or fewer deaths after one month, whether it improves or worsens patients’ condition, increases or decreases unwanted side effects, nor whether it increases or reduces negative COVID-19 tests 7 days after treatment. Likewise, the review cannot confirm whether or not ivermectin prevents SARS-CoV-2 infection or reduces number of deaths after high-risk exposure.” Both of these meta-analyses commented on the poor quality of evidence available.
More recently, there have been two more high quality, blinded, placebo controlled studies published. The TOGETHER trial investigating the “Effect of Early Treatment with Ivermectin 3-day on Risk of Emergency Care and Hospitalization Among Patients with COVID-19” found no effect in a study of over 1,350 patients. “Effect of Ivermectin on Time to Resolution of Symptoms Among Adults With Mild COVID-19,” by Medina, et. al., found no benefit either.
This extended post is to demonstrate that the recommendation of one particular meta-analysis has to be questioned given the potential bias among its authors plus its methodological concerns. The question of ivermectin’s benefit for Covid infections was hampered early on by a bunch of poorly done studies. More recent and better designed investigations have demonstrated ivermectin’s lack of therapeutic benefit.

The idiot in me is prone to ask:
Why would a drug company want to fund any study that would say an inexpensive drug might work better or in lieu of a more expensive one that the tax payers are funding?
Anyone want to provide a list of any elected officials receiving campaign donations from drug companies?
Any of the patients studied have gun shot wounds, signs of physical abuse, or any other odd quirks that just don't make sense but are claimed to be covid related? Asking for a friend.
Last one, let me see, no funding from drug companies, results show it might work, they don't like it because they can't make any money off the back of the general public, and it appears to be honest? Anyone want to wager a guess what would happen if you put a huge pile of money on a table with a sign that reads, "Take only what you need"?
    Thank you for posting!
