I would only if they’re upper elementary like 4th and 5th grade. Otherwise, I’d have the teacher give it. But for lower elementary I had a different BIP that was more kid friendly. It was an “apple” color chart. 🍎☺️
I make sure the teacher and student communicate the expectations so it’s clear even after the ratings. The goal is for the student to understand and change behavior ☺️
Who exactly in the administration is responsible for creating the behavior plan? ... I know my son's hasn't been updated since first grade - but i only can recommend to them what works at home...they don't actually implement the current plan...and he's older so I feel it needs to change a bit
I hope you and your son are doing well, now. There was no plan in place, at the time of your comment. A plan needs to be reviewed every 6 weeks or so until it is no longer needed. At that point, there may be accommodations or supports that need to be continued for success.

What happens if teacher does not agree with student rating? Do you have teacher change the score?

It honestly depends. Counselors don’t always do them. You should schedule a conference and get more information.
