  • Am I seeing more of this in clinic than ever before? I’m not sure. Maybe. But it’s certainly a pest for those struck down and the tendency for it to be mislabelled is a big problem as conventional acne treatment based on an antibiotic regime will make matters worse.

Having been on and off the antibiotics bandwagon for years, and become increasingly frustrated by the lack of progress, I saw a locum GP who took one look at my face and said: "That's folliculitis. Your spots are all in the beard area and your beard is thick and coarse owing to your Middle-Eastern heritage". No doctor had ever said that to me before, they just assumed I had adult acne vulgaris! That said, she conceded that the treatments for acne vulgaris can also be effective for (acne) folliculitis. Indeed, short courses of lymecycline and benzoyl peroxide have normally worked for me in the past (though became less effective after a few months). However she thought that topical retinoids would work much better for me in the long term, so for the first time I was prescribed Differin instead of BP. It took some getting used to before I could use it without peeling, but my skin is now the best it's ever been! It's even helping with the scarring, as well as my fine lines. Just started supplementing with once/twice-weekly Tretinoin 0.1% which I bought on a private prescription and should hopefully flatten those scars even more.
Most GPs have no interest in or understanding of acne, so it was refreshing to finally find a GP who was able to give me a true diagnosis. She was Mediterranean, so perhaps that's why she understood how folliculitis affects men with coarser, darker facial hair. She said that shaving can aggravate the condition, but added that in my case it was more than likely my slightly oily skin. Her diagnosis made sense to me, even before I started with the retinoid treatment. As a teenager I never had acne, the spots only came in my mid-20s and got progressively worse. This was around the time my beard went from a "boy's" thickness to a "man's". Also, refraining from shaving for a week or two made no difference to the number of spots, only their severity (redness and soreness). If only I'd had this diagnosis years earlier, I probably wouldn't have the scars that I have now! Going back to this video... At one point I considered whether my spots were fungal, and tried rubbing Canesten cream on my face. Made no difference, haha! I hope Dr. Sam will do a video on folliculitis, another lesser-known type of 'acne'. Also pseudofolliculitis, which I believe *is* shaving-induced and usually affects black men.

Dr Sam, if you see this, please help a poor girl out 😭😭😭 I dun have access to skin specialists here and I've gone to many doctors who've made my skin worse.
First of all, thank you sooo much for doing this video, I loveeee how you touched on how people with BOTH acne and fungal acne can treat their skin problems - I really can't find this info anywhere else :')
Like your first patient i was wrongly diagnosed when my fungal acne broke out Jun' 19. I was told I got acne and given erythromycin and low dose accutane. Within the next month, I got lots of closed comedones (which later became inflamed cysts and pustules) and my fungal acne worsened. Feedbacked to the doctor, who only told me to continue taking the same meds for another 3 months. I've always had clear skin before and suspected what I had wasnt acne as they are itchy, really tiny bumps that spreaded like wildfire across my face. This began within 2 weeks after using a fermented skincare. After 4 months on erythromycin and isotretinoin, my skin progressively worsened. I now have BOTH acne and malassezia to deal with. I went to another 2 doctors and after bringing up the possibility of malassezia, I was given topical Ketoconazole and a sulfur mask to try. And they worked!! My skin very slowly got better, but the cysts just would not go away. I am now starting on oral Ketoconazole for 10 days. I have ditched my erythromycin as antibiotics can worsen fungal acne, but I do not know if I can continue my low dose accutane as I really want to get rid of those painful cysts😭😭😭
If you see this, kindly advise if I can take both accutane and antifungal together, or if I should do something else. Thank you so much!! 😭

It comes and goes in severity but I’ve been doing this treatment for over a month and using nothing but basic simple cleanser and moisturizer to no avail. Dr Sam. I am currently going through chemotherapy. Part of my treatment is an immunology drug called Cetuximab. It creates an acne like rash (that is not acne) with dry, flaky and sensitive skin on my face. Can you recommend a very gentle face cream that is emollient but won’t irritate this rash? I’m also using a topical antibiotic gel ( I believe clindomyocin) for the rash.

Olivia Bruce Hiya I suffer with fungal acne and have been researching it for some time (my GP is completely clueless about it). It’s near enough all oils that are common in skincare that can trigger it, EXCEPT squalane and MCT oil. I personally use squalane and it’s caused me no issues (it’s cheap too!).
Sadly it’s not just just like that can trigger it though. Fatty acids too, and polysorbates, both of which are common in skincare. I would recommend having a read of the Simple Skincare Science blog as the owner has down extensive research about fungal acne and it’s all backed up with peer reviewed articles.

Gaby B thank you so much that’s really helpful. Yes I’ve been to a doctor and they didn’t have a clue. It is difficult when you don’t have a medical background to figure these issues out. I appreciate different skin issues aren’t life threatening but when they impact your confidence you wish a GP could be more helpful. Thanks once again.!
