  • This video presents a patient who experienced life threatening hemorrhage from a varicose vein that spontaneously ruptured. We discuss the correct use and application of tourniquets for extremity bleeding.

    I feel for this guy. Almost bleeding to death is scary. I have polycythemia and live with that possibility every day. Had 2 near fatal nasal hemorrhages. Awesome video's doctor. You always get the best stuff.

    Here are my recommendations as published in a blog I write for Emergency Medicine News.
    1. Applying a pressure dressing directly over the bleeding vein and follow-up with a circumferential dressing.
    2. If a tourniquet is used for a bleeding varicose vein, apply tourniquets below the vein for venous control and directly above for control of arterial flow.
    3. Elevating the limb.
    These bleeds can be impressive and direct pressure (with or without QuikClot) is the first step in stopping bleeding.

    I’m a NP in Texas, I specialize in cardiology and venous ultrasound. This is a great video! We see people referred from our local ER to get their veins ablated after they have a rupture. Most people don’t realize how serious this can be. Thanks for all your videos! They are so entertaining and informative!

    To begin with, I was so shocked, I couldn't think at all- only look at it shoot out like a water hose. Then I screamed & shot into action & grabbed a towel and tied it around my leg, but couldn't get it tight, so within 10 seconds, it was saturated & pouring down my leg again- so I called 911. Luckily EMS got here in about 7 minutes & applied direct pressure, but everytime they'd pull the towel back, it would shoot out at them again- I live close to ER- so they got me there fast- Thank God!!!

      our bodies aren't *_trying_* to kill us though lol, it just deteriorates after a while, your body is trying to keep you alive but our cells aren't perfect and they aren't immortal so that's just life

    This happened to me this morning! I was drying off after taking a shower when I noticed blood squirting across the bathroom onto the wall. Luckily my husband was there and kelp applying pressure with papers towels until it finally stopped bleeding. I didn't realized it was so serious. Now my husband is afraid it may do it at night while we're asleep and we won't know about it.

      Please find a vein specialist in your area if you haven’t done so already! This is so treatable! So sorry that happened to you.

    Absolutely correct, The war experience in Iraq and Afghanistan have clearly demonstrated the life-saving benefit of tourniquets, but those injuries and amputations from IEDs,etc. are different than a bleed from a varicose vein.

    This patient would benefit greatly from seeing a vein disease specialist and getting a venous reflux ultrasound and possibly need endovenous ablation of his great saphenous vein and possible phlebectomy and/or foam sclerotherapy of his varicose veins. Treat the problem, not just the symptom. A vein specialist (Phlebologist) should be able to help this guy and anyone suffering from similar issues.

    OMG! Now I know why my doctor yelled at me for wanting to go to work! I was getting ready for work, hopped out of the shower. Toweling off, turned around and I slipped. And there was blood spraying everywhere. Lots of blood! It didn't hurt at all. Just felt very cold. Took me awhile to get it under control. Luckily Quick Care was close by. Yeah... I was feeling drained and my leg had a buzzing sensation. Had no idea it was very serious.

    I had this happen. Believe me it IS scary! I applied direct pressure to the wound and elevated my leg to stop it. My grandfather had lesions and bleeding ulcers on his leg.

      Excellent Video! Excuse me for the intrusion, I would love your opinion. Have you thought about - Trentvorty Silken Legs Theorem (just google it)? It is a great one of a kind product for getting rid of varicose veins minus the normal expense. Ive heard some super things about it and my cooworker got excellent results with it.

    I had this happen yesterday. I had a blood blister ulcer on my left shin that was on an old scar. A blood blister formed on the scar about 6 months ago. I was getting ready to take a shower yesterdayand I felt something hot and wet going down my leg. I looked down and black blood was shooting out of my shin like a water hose. I reached down & a whole cup shot into my hand. I lost alot of blood (between 2-3 pints) before EMS got me to hospital. Doctor sutured it- A vericose vein hemorrhage. :(

    And luckily even though I had lost alot of blood (I figure between 2 to 3 pints) and was dizzy and heart felt fluttery, they didn't have to do a transfusion. Even today I still feel dizzy and a little fluttery- could be because I'm tired though because I am afraid to sleep- afraid the sutures will break free and bleed out again! This was the most terrifying thing I have ever experienced and I thought I was a goner for sure- all that blood shooting out so quickly!

      less pressure on the legs while lying down - extremely rare to have a rupture while in bed.

    This happened to my friend yesterday, blood squirted out across the room, every time she removed the towel, what causes this, hers finally stopped bleeding, but this is the 3rd time this has happened to her, she didn’t go to the hospital

    I work for Grady EMS and this is an very educational video. Dr. Mellick, You said that the using a tourniquet could cause more damage. Would putting the tourniquet below the bleed do more good than harm? And what are your thoughts on using Quikclot on a bleed like this in the streets?

    Definitely. That is the most effective for venous bleeds.

    For a young guy, he has poor circulation. I wonder if he has greater vessel disease as well.

      hypertension/high blood pressure - I think restricting blood flow in the thigh area increase his blood pressure

    According to the Committee on Trauma of the American College of surgeons, we are able to use a tourniquet at least for 100 minutes.

    As someone who didnt get far in life, thank you for sharing this experience. I always aspired to be a doctor... Unfortunately, now Im no one with a bunch of useless medical knowledge. Thanks to the fact I will never be able to afford an education. What was this patients discharge orders?

    putting direct pressure on the bleeding is like trying to use your finger or thumb to stop the flow of a faucet that has been turned on. one sure realizes the word "pressure" in the term "blood pressure" is no exaggeration. always have seen blood flow downward but to see it going straight out for a distance of around 8 inches(?) is a bit disturbing. seen it happen a few times on gore sites but never first hand before last night.

    Wow, this sounds scary. I think varicose veins in the legs can bleed just like a jugular vein. I think Clint Malarchuk was going through the same thing when he had his jugular vein cut by n ice skate. I think the best thing to do in this case is to apply direct pressure n raise the leg.

    I accidentally cut one of mine shaving. It was a big one on the back of my leg I was taking a shower to so I was washing blood down the drain saying no no IAM fixing to catch a plane so I kept washing it down the drain and then I tied a wash cloth as tight as I could onto the vein , I was very lucky buy the time I was ready to go out the door it stopped!! Thank God 😊

    One thing the medical profession needs to do is shave the area. I realize pulling a bandage off is like waxing the area. Just shave them please.

      Bleeding takes a little more presidence than shaving the leg. But yes, if it is feasible, shave we must! :)

      Beca there's no such thing as a stupid question.
      Varicose Veins are Veins that are permenantly dilated and have thin, weak walls.

      just to add, they are usually very common in older adults, heck, my mother has them, it usually makes the veins appear a thick, bluish purple color. When they rupture it can be unexpected or it can be caused by exterior trauma, when it's unexpected it usually isn't too painful, which is what makes them especially dangerous as you don't even realize the real damage, especially with clothes on, it can even squirt everywhere lol

      Yes, but I have seen them in 19 year olds, so crazy! A lot of it is genetics, being obese, having kids, standing or sitting too much, trauma to the vein valves (including DVT). Millions of people walk around with vein disease and don’t know it. And you don’t necessarily have to have varicosities to have vein disease. The one thing people need to know is that it is so treatable and most insurance companies cover it (still have to meet deductible first, etc.). It is not considered cosmetic to fix large varicosities and underlying venous insufficiency.

    I've got similar sores but mine never bled... They still haven't diagnosed me they've come & gone for the past 3-1/2 yrs.... I'm due for venous ablation in a few weeks.... Hoping it helps...so much pain & swelling

    my mom has varicose veins extremely bad but the doctors won't do anything for her she's in excruciating pain every day all day .

    If one knew the bleeding was from a vein, would applying a tourniquet distal to the bleeding make sense? I'd expect that tightening it down to venous pressure would stop the bleeding.

      It appears that bleeding from the lower extremity continues until it the blood is emptied.

    Unfortunately, I don't have insurance so treatment is not possible.

    Have them check your inferior venacava vein. I'm sure that's not how you spell it. I was born without one and have thousands of little collateral veins. We have the same troubles. My issue is on the right leg as well . I went in to have my appendix taken out and told me the news that my body is weird as shit. It's a long long shot but what the heck go get an ultrasound on your legs. And carry quick clot. And get some GOOD pressure stockings and never leave home without them

    wouldn't a pressure bandage actually work better on controlling venous bleeding then a tourniquet?

      Direct pressure would work (more pressure than bandage). However, I've seen bulky "pressure" dressings completely soaked with blood.
