    Hi doctor. I’m diagnosed with Frontal fibrosing alopecia unfortunately. I lost most my hair diffusely and also the hairline receded. Do you think hair transplant can be done if the disease is not active anymore? I cant stress enough how this disease affects mentally especially at young age. I’m wearing a clipped hair system but I think hair transplant eventually is the way to go.

    So what you’re saying is that if you experience hair loss in your 20’s you are going to lose more hair than if your hair loss started in your 30’s. Or are you saying that 20’s is too early for a transplant because there’s no telling how far the hair loss will go?

      Actually both of what you say are good reasons not to have a hair transplant in your 20s. Also, people in their 20s expect more density than is possible with a transplant, as 80% of their peers in their 20s aren't experiencing pattern hair loss. It's more important to treat high DHT-sensitivity in your 20s to slow hair thinning, and try to thicken existing thinning hair. I hope this clarifies things for you.

    Hi Dr Prasad,
    Good video as always! With more than 7 years of clinical datas, do you have patients that had a good initial response with the treatment, but after some time, continued to lose hair despite further treatments? Is there any trend that indicate that the treatment is beneficial for a limited period of time and after that, further treatments don't help?

      Hi Mike Reynold,
      Of the patients who responded well to the Hair Regeneration treatment years later, they have continued to sustain hair coverage after a second treatment. However, pattern hair loss can't be cured as it is hardwired into genetics. What we are doing is managing hair loss by keeping as much hair as possible, for as long as possible. We are only at time a time period where people are getting their second treatments after 5 years or more, so we can't say for certain about treatments after 10 years or more.
      You also have to consider that male pattern hair loss has different levels of severity. A man who loses hair in his 20s has a much more severe case than a man who starts to lose hair in his 40s or older. Younger male patients who are extremely DHT-sensitive is harder to treat, and will need a DHT-blocker along with the Hair Regeneration treatment. A man who starts losing hair in his 40s will have the Hair Regeneration treatment last longer, even without a DHT-blocker. I hope this clarifies things for you.

      Thank you for the answer. It's very enlightening! Do you prescribe dutasteride to people with very aggressive pattern hair loss? I have read alot on the internet that dutasteride is better than finasteride for hair loss. It's hard in Canada to have a doctor willing to prescribe dutasteride off label for hair loss.

      Hi Mike Reynold,
      I don't believe that dutasteride blocks more DHT than finasteride. I primarily prescribe finasteride to patients, then if they have any problems with tolerance, I switch them to finasteride. I suggest you consider taking finasteride first and see how it works out for you.

    Even after the hair transplant that achieved good results in the beginning
    And finasteride daily for life term
    Will the patient still have good results after long term
    As in 20 years later
    Sure
    The transplanted hair thins out but they don’t fall off (as I’ve learned so far)
    And the other hair which were not transplanted will live off by Finasteride
    But finasteride efficacy reduces by time
    It’s like 60% approx. by fifth year
    So along with thinning of transplanted hair
    And
    Non transplanted hair having lower efficacy with finasteride
    Will the patient still achieve good results after long term
    Or
    All is lost in vain in long term results

      As stated before, level of DHT-sensitivity of the user is the more important factor of finasteride's effectiveness, not a reduction of the drug's effectiveness in the 5th year. The follicles of thinning hair miniaturize until they disappear, and stop growing altogether.     Hi Dr, I suffered from some progressive frontal hair loss. My scalp tends to get oily after shampooing the next day. I am not sure if this is a sign of androgenetic alopecia. Can I go for the non surgical hair growth treatment, as I am 29 this year?

      Non-surgical treatment is possible at your age, but androgenetic alopecia does need to be properly diagnosed before the treatment can be considered.       Hi Lance Irvin,
      The risk of side effects from finasteride is only about 2% of users, so they’re very rare, and the vast majority of men taking the drug do not have side effects. I’ve had patients on finasteride for over 20 years who have not had side effects, and none of my patients have had long-term side effects. I hope this clarifies things for you.

    Hi doctor,
    I am in my mid 20's. I regrew about 60% of my hair with finasteride for 2 years but it is not fully recovered yet. It went from a diffuse Norwood 4 to a norwood 1.5. It is stabilized however. Am I eligible for a hair transplant?

      I still don't recommend transplants at your age as transplants won't give you the scalp coverage to match your peers of the same age who don't have pattern hair loss. You are still better off with non-surgical treatment at this age to thicken thinning hair, and stimulate hair growth beyond finasteride alone, or wait until you are in your 30s for a transplant, when more men your age have some degree of hair thinning.

    Thankyou for explaining my earlier question I kinda have one more question....I'm 25 yrs old and loosing a lot of hair on the right side of the crown...it's genetics since my father has the same type but he kinda lost his hair at his 40's...I'm having this sebum bumps on scalp which then causes hair loss and my scalp is very oily that I have to shapoo daily...do u think dht may really be the factor for sebum bumps on scalp and can finn help me maintain my hair atleast for a few years?

      You can't build up a tolerance to finasteride like people can to minoxidil. The effectiveness of finasteride is more related to your individual level of DHT-sensitivity.       Like the title says, you should not get a hair transplant in your 20s. You can watch the video to see why it's a bad idea for you. Finasteride is a prescription drug, so you need to see a doctor before taking it. It is more effective as an oral drug since DHT is in the bloodstream. I hope this clarifies things for you.

      I still wouldn't recommend a hair transplant at your age since the hair density expected in your 20s to keep up with your contemporaries is very different from hair density in your 30s and 40s. I would advise the Hair Regeneration treatment featured in the video at your age to thicken your thinning hair, and stimulate hair growth from inactive hair follicles. Hair Regeneration patients your age would still continue with finasteride, but can either stop or be weened off of minoxidil. I hope this clarifies things for you.

      It is possible after a physical examination to determine there is no pattern hair loss, and is usually done on people with no pattern hair loss, but who want to lower their natural hairline. However, due to the limitations of hair transplant density being less than the natural hair density, the transplanted area will look different from your native hair, and may require at least 2 surgeries for more natural looking hair density.     Sir I am taking hvit forte tablet daily. If I stop that tablet my hair fall is very high. Is there any side effects by continuous use of biotin for a long time?

      Biotin is part of the B complex of vitamins, so it's not harmful, but it also does not treat the genetic causes of hair loss. If you have pattern hair loss, then you are DHT-sensitive, and will need a DHT-blocker like finasteride. I hope this clarifies things for you.

      I absolutely do not recommend a transplant for someone your age, as shown in the video. Shedding 50-100 hairs a day is normal as those hairs do grow back. Pattern hair loss happens when shed hair grows back thinner with every growth cycle, until it grows back very thin, then stops growing altogether. I suggest you watch this video more closely about treatment options for someone your age. I hope this clarifies things for you.

      Your doctor should have discussed DHT-sensitivity with you, and the need for a DHT-blocker like finasteride as hair transplants do not treat progressive hair loss. If you only use minoxidil for 6 months, the hair it delays from shedding will shed after you stop using it. Finasteride will need to be used continuously to slow thinning of native hair. My native hair has no further progression to hair loss because when I opted for hair transplant I was not shedding from last 2 years . Now I’m only concerned about hair transplant.       I do think finasteride is more beneficial than minoxidil as it can slow down and stabilize hair thinning. However, if you stop using minoxidil, you can experience a sudden hair shed as the main benefit of minoxidil is delaying the hair shedding cycle.
      Also, you need to clarify the type of hair loss you have. Male pattern hair loss is gradual hair thinning and delayed growth after normal hair shedding. Pattern hair loss is not hair shedding, and it is not normal to not shed any hair for 2 years. I hope this clarifies things for you.

      Minoxidil doesn't block DHT formation at all, drugs like finasteride and dutasteride are DHT-blockers. Minoxidil won't thicken your hair, or stop hair loss, but delays the hair shedding cycle, and extends the hair growth cycle so you appear to have more hair on your head. You will have to use it daily to sustain these effects, but these effects can also decrease with time even if you use it daily. I hope this clarifies things for you.

      No, I don't advocate or perform body hair to scalp transplantation. The texture of body hair is different from scalp hair, doesn't grow as long, and the graft extraction process will cause visible scars in the donor sites. I hope this clarifies things for you.

      Hormonal imbalances that are not DHT-related can cause hair shedding, but not hair thinning. Hair can be brittle due to nutritional deficiencies, but not thin. If you have hair thinning, you surely likely to have pattern hair loss, as pattern hair loss makes up 95% of hair loss cases. Pattern hair loss is embedded into the DNA, so there is no permanent cure, only management to keep as much hair as possible, for as long as possible. I hope this clarifies things for you.

    Sir , do you have any clinic of yours in India , I need to maximize the density with this tricho stem formulla. I have lost hair on my scalp about 2 years ago , I tried most of the medicines available in the market , but I can't see any new growth ,and I don't face any hair fall either, since I lost my hair for the very first time .I can't go for HT as I don't have any bald spot , but this thin volume needed to be covered , any possibility please ?

      Sorry to hear about your problem. We don't currently offer the treatment in India, but we do have many Indian patients coming to the US for treatment. Since it is a non-surgical treatment with little to no recovery time, and in many cases no additional treatment, you don't need an extended stay in the US. Many patients actually get back on the plane home the same day as treatment, but I don't recommend this for everyone as it is dependent on general health condition and age.

      Telogen effluvium is hair shedding caused by severe stress, where the hair does eventually grow back. The time it takes to grow back varies with the individual, but no treatment is required for it. Male pattern hair loss is hair growing back thinner after the normal shedding-growth cycle. If untreated, hair will miniaturize until it stops growing altogether. The main cause of male pattern hair loss is having scalp hair follicles sensitive to the hormone DHT.       Sir would like to ask one more question.What are the symptoms of Telogen because for the from Feb to May I was suffering from Obsessive Compulsive Disorder and depression.I was in a stress trauma and I had 8 inches of hair.In June I went to my uncle's home and he noticed thinning spot above my temple it was out of no where I never saw it even before three days.Can you help clarify this.Thank You

      It is hard to diagnose without a physical exam, and your explanation is a bit unclear. Thinning where hair is thinner happens gradually, but shedding where hair is gone happens suddenly.

      The phrase "stem cell therapy" is too broad to say it works. The Hair Regeneration treatment recruits your body's own stem cells to renew a healthy hair growth cycle, and allow thinning hair to grow back thicker. It uses PRP as an activator of the main ingredient extracellular matrix, so it works. However, other treatments are also stem cell therapies, and don't work. I hope this clarifies things for you.

      A hair transplant is more appropriate after age 30 for several reasons: 1) only 20% of men in the 20s experience hair loss, so men in their 20s keeping up with their peers with a lot of hair will expect more density from a hair transplant to match their peers with lots of hair 2) if you experience hair loss in your 20s, you have a very rapid progression of hair loss, so you are losing hair faster than it can be transplanted, also known as the treadmill effect 3) if you continue to have transplants in your 20s, you won't have any donor hair left in your 30s and beyond 4) the amount of hair density to match your peers lessens as you get older, as more men your age experience hair thinning and hair loss 5) if you continue to have hair transplants without stabilizing rapid hair loss in your 20s, you would have spent tens of thousands of dollars on multiple hair transplants, but won't have much hair in your 30s as your native hair does continue thin after a transplant alone, leaving only sparse transplant coverage.       @@AmiyaPrasadMD I’m 23 and I’ve been losing hair since 2020 and it’s still as bad as ever losing way more than before. I have not gotten a hair transplant even though I’m considering it only because the medications (minoxidil, dutasteride and finasteride) that I was prescribed by dermatologist aren’t working at all. I’m at the turning to go for hair transplant. Even if I decide to wait till my 30s. You do realize that possibly I will be bald at that time and possibly can’t do anything about it. That’s why I feel like Doing it right now is better than later. It’s like it gotta be done before it’s too late. Explain that to me, what If by the time I turn 30, I’m bald and only have on the sides and back but won’t be possibly to get a hair transplant because my top head is empty of hair? I live in Connecticut but considering a transplant treatment at a country cheaper like Mexico

      You need to know that hair transplants alone do not treat progressive hair loss, so hair loss continues after you have a transplant, or even several hair transplants. You can refer to the man who had hair transplants done starting in his 20s, who is featured in the thumbnail, and also at 3:17. It's your decision, but there is a reason why many hair transplant surgeons who are concerned about their patients' results don't recommend hair transplants before age 30.

      Yes, you will lose hair after a hair transplant, and quite rapidly if you have it done in your 20s. Hair transplants don't treat progressive hair loss at all, they just move hair from the back of the head to the front. I hope this clarifies things for you.

      They can actually mean the same thing, as men with pattern hair loss have a genetic scalp sensitivity to the hormone DHT, which is major factor in hair thinning. However, there are other causes of hair loss due to hormonal imbalances that are not genetic, such as severe stress causing temporary hair shedding, or childbirth or postpartum hormonal changes causing temporary hair shedding in women.       There's no treatment required for telogen effluvium.
      I don’t administer PRP alone for hair loss because it has a limited effect on hair, usually just short-term hair growth. Also, the number of sessions needed does vary, and the longevity of results is unpredictable. Hair Regeneration is a PRP combination treatment where PRP is the activator of the main ingredient extracellular matrix by ACell. Hair Regeneration is a one-time treatment for most patients, and can last up to 5 years and beyond, depending on the aggressiveness of the hair loss pattern. You don't need repeat injections periodically, and many older Hair Regeneration patients don’t need maintenance drugs like finasteride, but finasteride can concurrently help younger male patients with very early and aggressive hair loss. PRP injections for hair loss are needed indefinitely.

      It is possible for male pattern hair loss to follow diffuse thinning pattern, which is more associated with female pattern hair loss.       You are about 10 years too early for a transplant, and your hair will still progressively thin despite your transplant. You will need to take finasteride to slow hair loss progression.       DHT is present in the bloodstream, so it's better to take finasteride in pill form rather than a topical. Biotin is a B vitamin that helps with hair health, but it does not block DHT. Finasteride is more effective than minoxidil as minoxidil does not treat hair thinning, nor does it treat DHT. I hope this clarifies things for you.

      Sorry to hear about that. There is a difference between hair shedding that's not associated with pattern hair loss, and hair thinning which is a sign of pattern hair loss. It is normal to shed 50-100 hairs a day, and they do grow back. Pattern hair loss occurs when the hair grows back thinner, and there is a delay in regrowth.       We don't currently offer the treatment in India, but we do have many Indian patients coming to the US for treatment. Since it is a non-surgical treatment with little to no recovery time, and in many cases no additional treatment, you don't need an extended stay in the US. Many patients actually get back on the plane home the same day as treatment, but I don't recommend this for everyone as it is dependent on general health condition and age.

      The chances of side effects from finasteride are pretty rare at about 2% of patients having them. I've had patients who were on finasteride for 20 years who didn't suffer from side effects. Of the patients I've had with side effects, they were temporary, and restored normal function after stopping the drug, often just temporarily. There are many common causes of sexual dysfunction, such as: lack of sleep, lack of exercise, stress, smoking, alcohol, and diet, but it is really easy to blame the drug when there could be other factors.
