    If anyone want to use aerotor while splitting, leave 1mm tooth structer before furcation. Then use elevator to mesial or distal side of tooth to seperate middle

    I'm a dental student, and I'm still new to extractions. But I used this technique for the first time. Pretty awesome stuff. Was literally the easiest lower molar I've done.

      Glad to hear it was helpful. Back when I was in school if you had to section a tooth it was almost viewed as a failure. Sectioning and removing roots individually is less traumatic to the patient, helps preserve bone and lowers the chances of a buccal wall fracture

    Very nice review. Thank you. I had not heard of the slight mesial and distal reduction via diamond bur to allow for a slight amount more space when using the elevators.....

    That's a really great method, I usually just section lower molars, but opening the mesial and distal contact points really gives you a lot more space to work with. Will definitely try this out.

    Yep that sounds great- try luxating and forceps- minimal movement, then remove M-D areas IP, try again and then see if you get enough movement to extract tooth, if not apply sectioning of the tooth and remove in two parts.

    It's a good tip, but I'm wondering if it will make the crown weaker, and then any use of forceps will just fracture the crown from the root and make the extraction more difficult.

      @@H982FKL-TG Vitality shouldn't matter. Consider the physics involved in extracting the tooth with the least amount of energy transferred to bone.

    tips for extraction of upper canines and premolars without breaking buccal plate? i find these much more difficult than molars, particularly endo treated broken at gumline

      for single rooted use a lot of rotational force. Hold one direction for 10-20 seconds even if there is no movement, switch to the other. Rinse, repeat. do plenty of rotational force prior to buccal/lingual.

    for me the extraction of premolars is so diffcut roots are long and slim can you advice me think you

      For multi rooted premolars treat like a molar and section it. For single roots still remove the mesial and distal contact to get movement in that direction. Also, a figure 8 motion will help expand the socket. In some cases I've used a Physics Forcep but be careful. Too much time of pressure on the buccal mucosa will result in a painful (but temporary) ulcer for the patient during healing.

      Get as far apical as possible with a periosteal elevator, then use your elevators to luxate, then switch to a forcep. Get as far apical as possible, move buccal for 5 seconds, move lingual for 5, then repeat. Use controlled force and reposition the forceps further apically if possible. Once you have some movement, rotate in a figure 8 and get that tooth out

      @@PivotDentalCompany I think you should be able to clinically see the path. Under regulations where I work this probably would not be justified. I appreciate these differ from place to place. Plus if you keep your cut in tooth structure say two thirds can you not probagate root speration with a elevator much as you would sectioning a third molar crown?

      I feel sorry for the tight regulations under which you work. What happens when your assistant misses catching the apex of a tooth during a root canal? Does he/she retake the film? What if contacts are not open on a bitewing film rendering it non-diagnostic? I bet another attempt at a bitewing would be okay. Sectioning a molar is done with the intention of making the extraction less traumatic. If the angle is off and you catch it with a film then there's still an opportunity to correct the path. Imagine severing the root which leads to more bone removal in order to get a purchase point. If that could have been avoided by taking a film then I think it's worth it. Hopefully your superiors can see the benefit compared to other procedures.

      I don't know the case of course but sometimes pt can't afford RCT and restorative tx.

      Patient can't afford RCT and periodic visit, so they oftenly choose extraction instead

      I agree with you but think about it. There would be so much pressure (patient may feel pain or not) for the patient. When you use handpieces, you are giving so much better experience to your patient.

    I can't understand the reason for breaking of mesial and distal contact points. A small elevator would fit into interproximal spaces anyway. We can't elevate roots toward adjacent teeth because they would break. So no need to create extra space there. I could only justify this procedureIn if the tooth would be a single rooted or with fused roots. It would then be similar to extraction of upper wisdom tooth. In my opinion partial crown removal would be more practical as we can visualise root anatomy, location of furcation. Splitting roots is much easier after crown removal.

    Would be faster to trough deeper inter radicularly, at least 50% of the bone that the root is encased in, in my opinion

      It's a trade off. Is it worth it to trough deeper subtracting more bone in order to make the extraction faster? Each doc will have to weigh it out.

      You can, but the cracking noise will scare the patient, even if they are prepared. Plus, if you section the tooth with a bur you'll get a nice leverage point
