    Thanks for your help. Few months ago I was diagnosed by a neurologist with "a mild case of parkinson" after going thru a nuclear test at the hospital. For two months i have been taking Pramipexole 0.25 mg (3 tablets a day). To date I have not seeing a great progress but i was told there is not a cure for this disease. My biggest frustration is i have poor balance and have to walk with a cane. Before this medication and after seeing a different doctor I took sinement, ropinirole and another one i do not remember now but they were giving very bad effects. I have gone to therapy but did not helped me much. Now I am doing exercises on youtube which are pretty good. I am an old lady and any help will be appreciated. Thanks for your time.

      I walk 30 minutes with a cane on no meds in the morning. I’ve been diagnosed over 12 years and take levodopa. Balance issues get worse end of day.

      I recommend an evaluation with a movement disorders specialist in your area to look into the details of your medications and symptoms to make a better diagnosis. Sometimes it is difficult to make a formal diagnosis.

    Hello Dr. Could you tell me if essential tremor can develop into Parkinson’s? And if so what are the percentages that do. Thanks

    • @TheParkinsonsDoctor
      @TheParkinsonsDoctor  11 месяцев назад

      Usually not. There are a few cases of a genetic mutations that associate ET and PD, but generally, the answer is no.

    I am 54 years old. I just completed DAT scan. I am also beginning a clinical trial for a possible new medication. I am hearing conflicting information regarding beginning medication this soon, at my age. Is there a downside to beginning medication at my age? Should I hold off taking medication for as long as possible. Only symptoms are tremors in arms and legs, stiffness, gate is not normal. What would you suggest? Sooner is better, or hold off as long as possible, due to medication side affects?

    • @TheParkinsonsDoctor
      @TheParkinsonsDoctor  Год назад

      This is how I discuss this issue with my patients. I aske them: How is your quality of life is being affected by your symptoms? How different lie would be if we can improve your "tremors, stiffness, slowness, walking, etc."? If the answer is that it owed be a major change, that could justify treatment. Every person is different. I am bringing a video soon about when to begin medicines for PD.

      Unfortunately, I am unable to provide a definitive answer to your question at this time, as I currently lack sufficient data to support an argument either for or against the matter. However, I assure you that I will continue to investigate the subject and will update you as soon as I have more information that could help us reach a conclusion.

    • @TheParkinsonsDoctor
      @TheParkinsonsDoctor  11 месяцев назад

      Extended formulation refers to a version of the tablet that has been made with the intention of lasting longer, providing longer benefit. However, this is not the case all the time as we learned over time.

    I have seen a discussion about “good Parkinson’s” that described “tremor-dominant Parkinson’s” as “good Parkinson’s” because it usually has a better long-term prognosis than does Parkinson’s with postural instability (PIGD - Postural Instability Gait Disorder). What are the differences between PIGD and rigidity and/or slowness?

    • @TheParkinsonsDoctor
      @TheParkinsonsDoctor  Год назад

      PIGD is the most challenging of all. Your comment above is accurate.

    • @TheParkinsonsDoctor
      @TheParkinsonsDoctor  Год назад

      Hmmm. Did you have a DatScan to assess dopaminergic dysfunction? Are you levodopa responsive? I’m not a fan of DBS in secondary Parkinsonism, except if tremors are a major issues, which can respond. Interesting case.

      I see many people coming to the clinic with similar situation. We adjust timing, dosage, use extended formulations trying to extend the benefits timeframe.

      It means its absorbed more slowly and stays in your system longer.
