I removed a shower head from a house that used lime scaly well water, no water softening system. It was BAD. I let it soak in vinegar about a day and a half. It came out almost completely clean. Just a little soapy water and a sponge wiped off the remainder without even scrubbing. A soft toothbrush to get in the crevices of the spouts.

works sometimes. also breaks down the rubber nozzles. sometimes it makes it worse. so instead of poor pressure it shoots every way besides down.
