I use 1" fender washers for my minis. I buy them in boxes of 100, and the are perfect for either custom bases or as weights under production bases. They are especially used with Reaper Bones minis.
Once upon a time companies use to sell metal bases. I've got a few on my old warmachine. With as crazy and dynamic as poses have gotten, I'd think weighted bases would be more common
Not that I can recall. They're supposed to attach to a unit if I remember the main rules correctly. I believe in both the rank and file and first blood they act as followers and basically accompany leaders. Don't quote me on that, as I misplaced the rulebooks!

Yeah would be great as Goliaths for DnD. For Age of Sigmar I could see using them as Ogors; very similar to the one in Cursed City.
