It depends on the airline, Greece allows you in fully vaccinated with no pcr test but some (probably most) airlines required a pcr test taken within 72 hours before your flight.

I filled out the PLF for myself and my fiancè in the same form. I was able to add her, although it did not ask for any vaccine details for her. Should I just do a completely separate form for her?

US to Athens, only your vaccination card needed. Athens to US, negative PCR from within 3 days and vaccination card needed.
