  • Understanding the interpersonal context within which conflicts and symptoms arise is essential work and often neglected. Being able to help the patient make emotionally meaningful connections between their current relationships and those in the past, as well as seeing how these dynamics are at play in the therapeutic relationship (T-C-P links) is highly associated with outcome. The concepts of "repetition compulsion" (Freud) and cyclical psychodynamics" (Wachtel, Strupp and Binder) are discussed.

    I enjoy all your videos. They're extremely informative and I particularly liked this one. it reminded me of how uncomfortable I feel when I'm being played. I often respond angrily or passively. Either way, I lose my power to some degree. This was a very useful reminder to take back the focus to my own needs.

      Thank you so much! I will definitely read it. I have studied many models within psychoanalysis and it seems to me that the ISTDP has a super clear, systematic approach and guides the process more than other models...

    Your client examples makes concepts so much more clear, thankyou.Though at times a client doesn't go beyond the defences, even after realising what's happening in the transference, What do I do then to facilitate behaviour change in the client?

      Help them examine both the benefits and the costs of the defenses. If we simply focus on negative consequences and don't acknowledge the benefits of defenses, the patient can cling to them. Conversely, if the patient is highly identified with the defense, he may not see the cost it exacts - particularly the way in which excessive reliance on defenses cause or exacerbate their presenting problems!

      We typically refer to the triangle of conflict ( feelings - anxiety and defense) and triangle of person (past-current-transference).

    Thank you Patricia for these videos! Great description of how you work intrapsychically and interpersonally in the same time. I would love to hear more about this topic with from a practical perspective. Like, how do you help your patients see this connection between T and P for example?
