    Let’s be clear..we’ve all seen the video by now. These police officers killed George Floyd. The Hennepin County Medical Examiner, and the independent medical examiner hired by the family of George Floyd, Dr. Michael Baden, have concluded that his death was a homicide….but their opinion differs on the cause of death. But if both of them declared that his death was a homicide, does the cause of death really matter? (YES). I want justice for George Floyd, and that is why I’m making this video because the medical explanation for his cause of death is not simple. As a lung doctor, part of my job is to figure out why people can’t breathe.
    As an intensive care doctor, part of my job is to care for people on the brink of death. Like when someone can’t breathe. So when someone dies of asphyxia, as is the case of George Floyd, the determination of the cause of death is dependent on information elicited based on the investigation, which includes the deceased personal medical history, namely, autopsy, and crime scene investigation, which of course includes video evidence. Asphyxia is a Greek term that translates to “loss of pulse.”

    Mechanical asphyxia involves some physical force or physical abnormality that interferes with the uptake and/or delivery of oxygen. With asphyxia, the brain doesn’t get enough oxygen, and when the pons and the medulla aren’t getting enough oxygen, they can no longer function. This means they can no longer tell the diaphragm to contract, and breathing then stops. While this happens, the heart is also not getting enough oxygen, and typically the heart pumps slower and slower until it stops.
    Prolonged continuous application of extreme pressure on the thorax, such as with the bodyweight of several officers, is capable of causing death. This is important because this contributed to the death of George Floyd, in addition to the knee to the neck. The neck contains our airway, the trachea, and carotid and vertebral arteries, and jugular veins. The arteries deliver oxygenated blood to the brain, while the jugular veins allow the deoxygenated blood to flow back to the heart.
    So what happens when pressure is placed on the neck?
    It depends on many different factors (amount and duration of pressure, etc.). And looking at the George Floyd video, he was unconscious for more than 2 minutes with the knee still on his neck. There’s no doubt that he took his last breath during this time, and right around the same time, he lost his pulse. By the time the EMS guy checks his pulse, I highly doubt he actually felt a pulse because it was more than two minutes after George lost consciousness. Obviously, when they moved George onto the stretcher, he was completely limp because he was dead. And it wasn’t until much later, did they start CPR in the ambulance. Now let’s get to what the medical examiners had to say about this case.
    Dr. Michael Baden, who did the independent autopsy, says Floyd died of "asphyxiation from sustained pressure when his back and neck were compressed, with the neck pressure cutting off blood flow to his brain.” I agree with that assessment. I would also add that partial compression of the trachea, causing airway compromise, was also possible. The weight on George’s back made the work of breathing much harder for his diaphragm, and the neck pressure at the very least meant less blood (and thus oxygen) was being delivered to his brain, and less carbon dioxide could be removed from his brain.
    After a while, the diaphragm becomes fatigued and no longer has the strength to contract, which means the lungs can’t get oxygen into the blood and can’t get carbon dioxide out of the blood. And all of this caused him to lose consciousness. And probably within seconds, he lost a pulse. And despite losing consciousness and despite losing a pulse, they continued to apply pressure on the neck and put their weight on his back. The Hennepin County medical examiner's office said that the cause of death is "cardiopulmonary arrest complicating law enforcement subdual, restraint, and neck compression."
    This statement doesn’t really make sense to me. But the Hennepin County release also says heart disease was an issue; the independent examiner didn't find that. The county said that fentanyl and methamphetamine use were among "significant conditions." Still, its report didn't say how much of either drug was in Floyd's system or how that may have contributed. But Dr. Michael Baden got it right.

    As an honorably retired officer of 27 years, I was appalled beyond words at these officer's conduct. The lawful use of force upon another stops when resistance or the threat stops. Additionally, proper training includes recognizing when a fellow officer has exceeded scope of force during an arrest triggering a duty to act on the other officers to stop it. The other officers, unlike a bystander, had an affirmative duty to act to attempt to save George's life. I was trained to recognize excited delirium in the latter years of my career. I am afraid that the poor optics to which law enforcement has been subjected to in the media for the last 20 years, not supported by FBI statistics by the way, has made recruitment of candidates with the education and skill set necessary for police work very difficult. It has led to hiring of individuals that would have been disqualified in the past as departments become desperate to fill positions for which competent candidates will no longer apply. Gandi thesaid be the change you want to see. We need more minorities to be officers but they do not apply. Please be the next generation of empathetic problem solvers the forces so desperately need. Otherwise I fear the incidents of police misconduct will only rise.

      I don't understand your language at a crucial point! "poor optics" either hasn't made it across the pond, or into my lexicon - so are you saying that police in America have had sufficiently bad press to deter honest, upright, not to mention bright individuals? Hmm. Well, I'd think the status of all authoritarian figures in Western societies has reduced, for better or worse. It puts more onus on the individual to step up to honour their own morality, in spite of not getting much validation from society.
      The media focuses on sensational moments. Perhaps the onus should be on them to show what led up to these events.

      That's interesting, do you also think that an independent commission should be set up to investigate police matters as nothing will change when police investigate themselves, this scope should also include district attorneys and perhaps judges, also I head it mention if you are going to attract good police they should increase salaries, the justice system also needs complete reform as it is tilted heavily against defendants by having to take plea deals instead being presumed innocent etc, it is a complete mess with no political will as it is a money maker for those in charge and linked to police funding etc, Gov Cuomo got paid $174,000 or so from the police unions this year so is it any wonder he defends the police

      I wonder about the training involved here. That, and the situation will certainly be important in the trial, as will the levels of drugs and metabolites in his blood. The reported level of fentanyl might have taken his life that day, even if he hadn't encountered the officers.
      We'll see what comes out.
      I'm curious if there is an alternative method of restraining a suspect in such situations?
      If so, that would improve outcomes. Officer and community safety is an issue as well.
      It's easy to criticize when seeing a video from 1 perspective in hindsight.

      I think many officers are confused on the point of affirmative duty: courts have ruled that law enforcement has no duty to protect life or property, only to enforce the law. And most officers do not consider the actions of their fellow officers as breach of law and therefore not triggering a duty of enforcement. The notion of 'to protect and serve' has been completely dismantled as a result. And this leads to a lack of trust between civilians and law enforcement.
      On the civilian side, the logic is: if law enforcement is not to protect and defend the lives and property of the governed, then what exactly are they there for? Only to enforce the will of law makers?
      On the law enforcement side, the logic is: there is no duty to protect life or property, only to enforce the law. But officers are bound to department policy and protocols which are not direct application of the law itself (meaning not a cause effect relationship like a civilian has to law).
      This difference and thus lack of trust breeds violence. When police view civilians as perpetual risks to themselves and their fellow officers, they tend to act accordingly. And civilians, knowing that officers do not trust civilians and view them as potential threats then have to view the officer as a potential threat. This is the paradox of mistrust...both parties are poised to act through the lens of danger and risk which creates actions and outcomes wholly different than if there was a common level of trust and respect. And this creates a self perpetuating phenomena...each event seems to justify the predisposition of mistrust and sense of danger.
      Cops view 'war on cops' and civilians view cops as authoritarian and willing to assert aggressive force in a risk prevention manner.
      Thus we've lost the Peace Officer / part of the community aspect. Department policy seems to be a different standard for conduct and accountability...and wide discretion and allowances not afforded to civilians erodes further and further. Where civilians are told that ignorance is no excuse under the law (or its enforcement) law enforcement itself is allowed to be ignorant and even in violation of the law, yet considered within bounds of 'policy' or exempt of liability because they 'thought' or 'acted' in accordance to their ignorance in good faith; or they acted out of fear for their life. Civilians may not use, in general, any defensive measure out of fear toward an officer, even if the officer is in error or acting contrary to the law; and law enforcement is allowed discretion, to be in error or ignorant and still not accountable to the law as would be a civilian.

      The one guy had multiple complaints against him and there was a suspicious case where he shot a guy in the abdomen. I think you're right. He at least killed unjustly before. Vicious sicko.

      I believe it's apparent that this is not the first time he tried this method. He didn't learn this at the academy but definitely as a rookie on the street. The other three cops knew this wasn't their first rodeo either. If he wasn't trying to kill him, then he was definitely trying to break his will. Either way, it was a heinous act. Torture in my book. No excuses.

      @Sean 85 if you read the probable cause statement for Derek Chauvin's arrest, it states that Floyd simply used a counterfeit $20 bill at a gas station. It was when he was being pushed into the patrol car that Floyd resisted the officer's attempts to put him in, and then former officer Chauvin ordered the other officers to help restrain him on the ground. It was when Floyd was finally on the ground that Chauvin put his knee on Floyd's neck. Hope that clears it up for you.

    Thank you for taking us through a process and not just an oversimplified conclusion with an associated rant! I'm curious though, have you ever heard of the carotid sinus being implicated in c/v-related death? I know it's contraindicated for certain pts, but our classes haven't gotten into that much depth outside of its role in BP regulation.

Two photos have surfaced this week of cops doing the exact thing, one from Paris, one from Madrid. So it seems like a bonafide tactic. I shudder to think of all the mean spirited people out there who just learned a new way to harm or kill another human.

      I agree.Or they probably would have been arrested themselves. All of these people who are saying that they would have done something likely wouldn't have done anything. I'm sure that they would have TRIED to do something but I don't think that they would have been able to get the cop off of him.

      Friends of mine told me they would have pulled the cop away, but i told them exactly what you wrote. They would have shoot the person trying to help, or kick him into oblivion. And more important there we’re 4 cops. If you would have touched one of them the 3 others would have 100% pulled there weapons on you. I’m surprised they did not arrest the guy filming and destroyed his camera. But guess he told them he is live streaming and on facebook and this made them cautious not to touch him filming.

      I think Chauvin and Thao KNEW what they were doing...I don't feel like the ones on his back and legs had the intention of causing harm. They should all get charged, but Thao and Chain should be charged with 2nd or 1st degree murder. They both ignored both george and the bystanders. they knew what they were doing

    I can not accept the fact that the Emergency Medical people did not try to give first aid to George before loading him on a stretcher when seconds count. It was like they could not get permission from the officers to try to bring him back. So many people are alive today that had their hearts started again. There needs to be an investigation of other past inter agency events. The medical responders need to be questioned not only on the death of George but other cases.

      We do not have all the information. He had Fentanyl in his system , which will affect his breathing. Amphetamines, which affects the heart. They did not show the Medics assessment and treatment. You cannot make conclusions until all evidence is presented in a court of law.

      I watched a slow version of that video and when the ambulance arrived and one checked his neck for a pulse he was not wearing a medical uniform. It looked like they were in a khaki colour uniform and appeared to have a bullet proof vest on as well. I live in Australia so am not sure as what type of uniform is worn by ambulance men in the US but I would like to know.

      @ Helen I’m a Paramedic here in the US. All agency uniforms differ. Some are more flashy some are not. There are ems crews who work in more dangerous areas who wear bullet proof vests here. Hope that helps.

      I'd argue that the pig on his neck should get first degree. There's evidence they've interacted before. Plus he had more than enough complaints about excessive force prior to this that shows a pattern of aggressive disregard for life.

If he was alive and was standing trial for this event regardless of police misconduct or not, You would all care given his sheet then lets be honest. We all know the long term effects of the use of narcotics in the body and what it can do to the mind, it's been well documented enough throughout medical history and well explained. I find it odd that he can't put the cause of death listed from the medical examiner's on a record cause it's not a cause of death but can lead up to a cause of death if I heard that correctly although given his medical knowledge, and that he said " The heart pumps oxygen to the brain " instead of " The heart pumps oxygenated blood to the brain " and how he goes off to great lengths to explain thing's but uses examples in the ways he explains them suggestivly. He says the case is complicated cause there are many factors that either are or could be yet it's sounding like a clear cut case especially with his examples, and throws in a few of those " How-ever "'s except both exams say the same thing with maybe one or two added extra's like one finding heart dieses where one did not. But doesn't confirm if he did have those drugs in his system but says there is no telling how much he had, which a toxicology report actually could tell you and isn't it preformed on all people to figure out what it was and the levels if any at all no ? And how did it go from 4 officers to several as he says, as he uses that in one of his explanations ? And that saying he couldn't breathe but technically exhaling is technically breathing and he was also talking while exhaling. I'm wondering if he was worn down health wise from prolonged drug use and if he was indeed high at the time, if the situation scared him soo badly that he literally was " Scared To Death with paralyzing fear " that he couldn't breathe kinda like a panic attack, or is that just not medically and scientifically possible that paranoia which does tend to happen to users over time no ?.

    Dr Mike Hansen; I was "transfixed" by every explanatory metaphor and descriptive articulation. I am 68 yrs old and garnered a myriad of knowledge in your concise dialogue about the body's anatomical compilation. Thank you for a thorough understanding of asphyxia and hypoxic linkage. I watched my 16 yr old sister die from lung asphyxia in 1965; now I can comprehend why her asthmatic attacks caused lung scarring, leading to death. I've always been 'haunted' by my lack of knowledge as I tried encouraging her to breathe: I was 13 then; it stunted my intellectual growth. I barely made it out of 8th grade grammar school, the extent of my scholastic achievement.

      Burrdmizzle now that you are in Med school, timer to find Right Wing doctors (there are tons out there), the buck the traditional teachings of allopathic doctors. North American medicine “thinks” it’s the best, when globally, they aren’t. An MD is a drug pusher for big Pharma with their hands tied so they can’t think outside the box to alternative treatments that work. Please be open minded.       I doubt he is correct though hes making a lot of assumptions. The autopsy report specifically noted no injuries of the neck or laryngeal structures. There was no drama to the posterior or lateral neck. If you look at the video, his knee was not on the trachea or a crowded already. It was on the posterolateral spine. He was able to move his neck and had freely and was able to rotate his shoulders so I don't think there was undo pressure on the back or neck, just enough to hold them in place. It would not take much pressure to hold them in place since he was handcuffed. The autopsy showed had severe coronary atherosclerotic disease. He also had hypertensive disease with a biventricular cardiomyopathy, . He was on fentanyl which reduces his respiratory drive, and amphetamines which increases his heart rate and blood pressure. If you take somebody with hypertension baseline, put him on amphetamines, and then the stress of a struggle with the police, his blood pressure and heart rate were likely sky high If he has significant cornary artery disease, then he likely developed myocardial ischemia and subsequent ventricular tachycardia leading to ventricular fibrillation and death. Common things happen commonly.

    In light of the new video, George died of the extreme amount of Fentynol in his system. It was enough to kill someone by creating respiratory distress and or coma. If George had said yes when asked if he was on something, the arrest would have went different. Sorry, no one is above the law.

      I agree. Breaking the law is breaking the law, however, I am only commenting because we need to remember that we can be equally outraged at different things at the same time. Politicians have turned us into all or nothing supporters, and people are weak-minded and fall for it. You didn't mention justice for Mr. Floyd so I am assuming you are not someone who has been out marching over his murder, which is your right, but if you are only watching media, then you probably assume that the people out on the streets are alright with the looting and violence and that's not true. You might think that everyone out there are a bunch of hooligans because the fires, looting, and violence, is what the media chooses to show, but that's not true either. Most people out there marching are rightfully angry and passionate, but have no interest in stealing and destroying property. I have been out marching. As a black woman, raising a black son, seeing unarmed black men murdered is highly personal to me. But I also love where I live, and it broke my heart to see businesses in my area destroyed. I guess what I am trying to get at is that wanting justice for Mr. Floyd, and wanting those who were looting brought to justice don't need be in opposition. Although you didn't write about being upset with what started this whole mess in the first place, I hope you are a person who understands that this doesn't involve "sides". The sooner we we can stop with all the "this side or that side" way of looking at issues, and recognize that as humans, we all want the same fundamental things, the better off we will be.

    We see in the videos that Chauvin's knee was not always in the same location. When the other cop asks if they should move Floyd over on the side, Chauvin's knee is actually between Floyd's shoulder blades - not on his neck. We may also conclude that total tracheal occlusion did not occur at _any_ point in time, because the placement of the knee and the angle of the neck is all wrong. Obstruction of blood flow, while the most dangerous kind of choke, is not asphyxiation. Asphyxia is specifically restriction of oxygen to the brain from obstruction to breathing. Floyd probably had difficulty breathing due to pressure on his thorax - being Covid positive, his lung capacity was probably already reduced, and the knee on the _back_ probably did a lot more to kill him than the knee on his neck.

    Two things were immediately clear to me. 1/ he was saying he can’t breathe(many times) before he was on the ground. This could in some way explain why they weren’t taking him seriously. 2/ For someone who’s claustrophobic (regarding the police car) he seemed to be fine in his own car. He was also on drugs and had been convicted of armed robbery including holding a fire arm to a pregnant woman’s belly. This doesn’t excuse the police officer’s handling of the situation as it was clearly over the top. But neither should we be beatifying Mr Floyd.

    Dr Hansen thank you so much for this clear and informative video. You made it so easy for me a layman to understand. I was watching the video of his murder and was yelling at the screen saying stop stop stop you are killing him even though I knew it was too late. It wasnt a movie it wasnt something that one could pause the screen or rewind to stop it. I guess it was just a human reaction. I was watching the bystanders begging and if I had been there would have been doing the same. I could not believe the officers just ignored. They had plenty of warning from both Mr Floyd and the onlookers. It was so distressing to watch. The 8 minutes of the video seemed much longer to me watching it. It seemed to go on forever.
    However listening to you it was not about emotions it was about getting it clear in my head as to exactly how it happened. I do hope justice will be done and those putting continuous pressure on his back and neck are charged with murder as a direct cause and any underlying medical conditions should not come into it at all. If they had not been doing what they physically were he would be alive and I hope the others are charged with at the very least aiding and abetting a murder. Not sure of the legal term. Also not sure legally whether they should be themselves charged with murder or a lesser charge so I will leave that to the legal system. Standing by and not stopping it while witnessing it and preventing others from coming to his aid in the simplest terms is how I witnessed it. This is so clear and so well explained. Thanks again. It broke my heart (not literally as I know you are a doctor) watching him begging for his life and watching the other bystanders begging too.
    PS Even not relating to Mr Floyd's murder I found this very interesting and informative. I now know a lot more than I did a few minutes ago.

    Mike, I just looked at the video again.
    The "EMT" guy arrives, stands at the head of Floyd, then walks back to the ambulance and is later seen with a stretcher, and what appears to be a pistol pouch on his belt, ALL THE WHILE Chauvin CONTINUES to keep his knee on Floyd's neck, and which is not removed until the "EMT" guy kneels and prepares to shift Floyd over onto the stretcher.
    Does this not strike you as being very odd...in almost every respect?
    Why would Chauvin keep his knee on Floyd's neck when for some 2 minutes after some of the officers were expressing concern, not to mention bystanders screaming that Floyd had stopped breathing?
    Why would Chauvin keep his knee on the neck of Floyd after the EMT's had arrived, and supposedly "checked his pulse" of which there was supposedly "none"?
    Where did an ambulance come from so quickly?
    Who called it, and at what time during the 8 minutes or so of 'knee on neck' restraint?
    Was it called and arrived from where during the elapsed 2 minutes from Floyd's cessation of breathing to the arrival of the EMT's--otherwise, why would you call an ambulance-- because it can be established from the video that Chauvin's knee remained on the neck of Floyd until after they arrived and viewed the body of Floyd?
    Alternatively, was a 'special" ambulance with "armed EMTs" called because, and prior to the 8 minutes of "knee on neck restraint", Floyd had expressed concern for his own health during the arrest, which might explain why he was dragged back out of the patrol car to await the ambulance?
    Reports suggest that Floyd was acquiescent right up until they tried to put him in the patrol car, whereupon he resisted, and cops relish nothing more than a challenge of physical resistance by someone already in handcuffs.
    There is either a lot more info associated with this event, or there is something dodgy that happened, as tragic as it appears to be.

    This was a lovely and thorough report! I was an ACLS Director and in my career have taken care of at least 50 cases of ARDS, & early AIDS/ HIV and all the horrible flus of the past before COVID19. And so many cardiac and lung problems- 1000 deaths at least. REPEAT:

      Any experience with fentanyl overdoses in the field? Police training?
      We don't, nor should we EVER, judge an entire situation on a video. There are edits and factors that cannot be recorded by a camera.
      This is exactly why we have trials. Things aren't always what they seem in videos, and are all too often not. Video gives us a myopic view of the world, taking a 3D, interactive world with emotions, thought processes, etc. and turns it into a 2-D visual with limited audio, no peripheral vision, no thoughts, emotions, touch, smells, tastes, etc. that occur in the situation recorded.
      That's why people are so well manipulated by video, unless they think outside the confines of that medium.
      Let's see what else was going on there, then decide.
      This is why we have critical thinking classes. It would seem they need to be taught in high school, as regardless of the outcome of the trial, so many young people are willing to scream "We want justice", yet without having all the facts that are available, and assuming that there are no other facts.
      THAT is an incredibly scary situation!
      That's simply being prejudiced!
      Mob thinking is dangerous.
      Do I think the officer's actions at least contributed to or perhaps even caused his death? Yes! I believe the officer should have been arrested immediately.
      I also believe there needs to be further investigation.
      There are quite a few things that need to be looked at.
      Let's use our heads, NOT emotionally have a knee-jerk reaction to an incident we watched in a few minute video.
      I have many questions about what happened.
      This is exactly why we have trials. We absolutely cannot convict anyone on a video alone. If we do this, justice is not served.

      @@CarolReidCA Social Media, by definition, is knee-jerk reaction to a visual stimulus. That aside, there is no question that the police murdered George Floyd, and that they did so in concert with each other and under color of law.
      I do agree that there are many unanswered questions regarding what the (ex) officers did, and what they failed to do. Also questions about what the paramedics did not do. And one that bothered me from the get-go..... Who called for the ambulance! Did one of the 'Power-3" finally realize what they were doing and have a twinge of compassion or responsibility? If it was "just an arrest" an ambulance would not have been called until their victim actually ceased breathing. None of these pigs even checked for a pulse or breathing even when the bystanders were screaming at them to do so. We will never be able to un-see that.
      My concern at the trial is that this is a very complicated medical picture and I don't think that our Average John and Mary Doe are capable of fully understanding the processes as were clearly explained in this video. It only takes one uneducated juror to acquit all 4 cops..... I say this after recently experiencing a jury not at all understanding a true-or-false concept that was vastly more simple than the concepts that will be presented in this case. Even though I expressed my concerns to the judge in pre-trial motions, he denied additional instructions to be given to the jurors. Unfortunately my gut feeling was right when the jury returned an "impossible" verdict and the judge accepted it.

      Carol Reid - Yes, I have had experiences with Fentanyl in the field. I rode ambulance (and flight ambulance) for 16 yrs. Your points in your letter were fabulously on-point. And I get you. Thank you for your clarity.
      I mean that. Your are so right about these matters.
      My opinion is that America voted on this death and how it happened. If I were a jury member I’d have the “cahones” to hear the whole story. But that’s me.
      In the first segment of film on MrFloyd’s arrest --it was done by a camera mounted on a building. What I saw was WNL. ( within normal limits). In fact Mr Floyd was pleasant. When he was walked over to the building from out his vehicle after being handcuffed there, he was sat down on the sidewalk up against that same building. He had trouble sitting upright. I call this THE FIRST CLUE. He may have been ill with the COVID19 -that he may have had very covert symptoms right then having to do with his back muscles. Maybe he took pain Rx for said back pain that could be related to the screwy symptomotology this coronavirus is famous for!!
      Again. Thank you. You would be the kind of person I’d like to have worked with.

Which is what the population is responding to, and why the officers in question were immediately fired. The time it takes to watch someone die (over eight minutes) due to four police officers, brutalising a restrained man, is what caused a mob to rise up, and say, this is not a standard, law enforcement should be using. So you are right, and you are wrong about mob thinking. To get this case to trial, to get the authorities to take it seriously, required an independent autopsy and a lot of mob rule. Otherwise it's just business as usual, where people like Floyd, never get represented by the law, with men who killed him, paid to patrol the streets again.

      Carol Reid if it was the fentanyl that was the cause of death, he would have shown signs of fentanyl overdose. If overdose were the cause, he wouldn’t have been conscious or even breathing well BEFORE having his neck crushed. Even disregarding the lack of any clinical signs of overdose, the concentration above is no where near overdose levels. It is truly unfortunate when people without medical expertise make comments about medical things. It can cause confusion for lay people. I have an MD and just spent five weeks in NYC in the ICU treating ARDS patients and using fentanyl every day. People don’t magically die of fentanyl exposure, or even COVID exposure for that matter, they do die within in minutes from asphyxiation however. And if you don’t believe me, watch Mr. Floyd’s murder, it’s an excellent example of how asphyxiation kills humans.

    I agree George Floyd needs justice and from the looks of it he is getting it what happened to him should not have happened. But justice shouldn't only be from those 4 officers.
    All police are trained to put the knee to the back of the neck to subdue an uncooperative suspect. The police academy, the police chief, the county all that allows that training should be held accountable. There has been a lot of people that died from police using the knee to the back of the neck. That training needs to be Abolished. Handcuffed on the ground, maybe if very uncooperative legs held down or ankle cuffs. That should be good enough. This knee to anything from the waist and above is BS. Especially when their already handcuffed :(

      Obviously with Floyd it was not necessary he was handcuffed. and the officers knew that and done it anyways.
      It is Not legal to place a knee to the front or sides of the neck in any state but some do allow the back of the neck..
      My state Oregon don't allow it, according to reports Washington and Florida and obviously Minneapolis does.

      They didn't follow procedure so it's kind of irrelevant. Instead of turning him on his side so he could breathe they did what they are trained not to do despite knowing it could kill him to leave him like that.

      @Palmarino Rossi but he was cuffed already before he was on the ground. There was no need and he didn't resist but most of all he said he couldn't breathe. You don't have to keep someone in that position for that long, common sense says you'll kill them or make them panick. The whole thing is disgusting. The length of time it took for arrests, the charges and states medical officers ridiculous autopsy repost. It's systemic and rife.

      Palmarino Rossi add the one kneeling on his legs as well. The symptoms ( hemoptysis absent physical trauma ) fit the symptoms and his physical profile hits several of the minor criteria of the ACCP for increased risk for DVT/PE. If the autopsies were conducted hastily in the name of expediency bowing to public pressure, it could have been dismissed as post mortem embolus.

    Dr. Mike, have you also considered the effect of full cervical rotation on the ipsilateral vertebral artery? And extreme compressive force from posterior to anterior on a prone cervical spine would force the neck into extreme extension as well. Forced stretching of the contralateral vertebral artery (possibly narrowing the lumen) coupled with the mechanical kink in the ipsilateral v.a. could perhaps greatly reduce the O2 delivery to the M.O.'s respiratory control center, no? Forceful inhibition of rib cage movement from the weight of the officers, possible inhibition of diaphragmatic movement, elevated heart rate and oxygen demand......the perfect storm. The Floyd's private M.E. should consider more than just the anatomy and physiology, but also the biomechanical stressors, don't you think?

    You cannot tell by watching the video just how much pressure is being applied to Floyd's neck. The autopsy did not find any bruising. he was ill before he was hancuffed.

      There is so much reasonable doubt. Floyd also wanted to be put on the ground. Derek Chauvin is also only 156 pounds, so the force put on his neck may not be big enough.

      Hi Dr.Martin , I watched your u tube video regarding the cause of death in the case of George Floyds death, which has shocked everyone in the United kingdom. your expert medical facts gave me a clearer understanding on the cause of Georges death. However i have noticed the discrepancies between the first autopsy report and the second autopsy report. which could cause a conflict of medical opinion in court can you please explain what will happen will the judge believe the 1st police autopsy or the 2nd independant family autopsy or will there need to be a third autopsy. i like many people in the uk would like to see justice for the brutal murder of george floyd . i know you are a busy man however i would be grateful if you could spare the time to reply.

    Thank you for explaining in terms I can understand. It's upsetting that the county examiner was vague and gives room for these officers to fight in court. They accelerated George's dead by their actions. The fact that Paramedics did not try at the site to apply CPR shows just how everyone there were negligent.

    I want to point out that reportedly one of the officers checked for a pulse a little while after he went limp and said he couldn’t find one but the officers on his neck still continued to apply pressure and hold him down.

      Not really. Exhaling is not breathing. You can exhale with your last breath and speak. We just don't know when he died because the body does produce movement after death.

    Why weren’t the paramedics held liable for starting CPR so long after they arrived. Obviously they didn’t do an adequate job in doing a Primary Assessment and acting quickly on their findings of Cardiac Arrest!

      The paramedics arrived. They needed to wait for him to be unhandcuffed.the put him on the stretcher and started cpr in the ambulance. They did it exactly right.

      I doubt that it would have mattered. He was already dead. If he had a pulse, which I highly doubt, then brain death would have been a huge possibility. 😢 He was obviously dead when they placed him on the stretcher.

    You can still speak out just very very little at the last breath when your lungs have no air. Someone somewhere said if he can talk, then he can breath. That is not true. If you put your knee on a water hose, no water would come out as you block it. Same thing on the neck, you blocking the air. Poor George, how he struggling trying to grasp for air and crying for his mom. . All mothers are very special and their children feel safe and secure with them. That is why George were calling for his mom. RIP George. .

      Exactly!!! As I watched that horrific Murder, all I can see and hear from Chauvin is it was "Deliberate" and it was "Intentional". He wanted George to die! Why else after George was Handcuffed, was he Not put in a Cop car and taken to jail?? There is simply No Other Reason for that Cop and the Other Cops to keep George on that pavement when he was already in "CUSTODY"!! And something else that was telling, was CHAUVIN telling George to "get in the Ca"..."get in the Ca",(Car to a normal person is said...Car!!!), when Chauvin KNEW George couldn't get in the "car"! So just That alone is called "TORTURE"!!! I hope the Protesters start shouting "GEORGE WAS "TORTURED" "BEFORE" HE WAS MURDERED!!

    Great tutorial about the lungs.however as an internist you completely overlooked the fact that he had two kinds of drugs that mixed together could be lethal in itself. While focusing on one aspect of the anatomy you completely ignored a primary result of having drugs like fentanyl in your system.

    Mike Hansen you are clearly biased here...from one doctor to another. The position of Chauvin's knee on Floyd's neck could not compress his trachea significantly at that angle. He was pressing from behind...his neck, slightly lateral if one might say. So...possibility of cervical spine injury is high...but the autopsy ruled that out. Compression of carotid artery...that's more like it considering the lateral pressure. But common carotid arteries still run slightly anterior and any impairment to the flow causes stroke or cerebrovascular injury. Not cardiopulmonary arrest...unless his circle of Willis was completely dysfunctional and no flow was delivered at all to the brain. Still...the major autopsy result would be stroke. Something just does not add up. I am not defending Chauvin for his action...i believe his action contributed to the death of George Floyd, and his behavior is inexcusable. He deserved to be punished. But asphyxiation due to tracheal compression? Considering the location of the crooked cop's knee? Have a second look and be a unbiased medical doctor. In addition, a significant amount of fentanyl and methamphetamine was found in Floyd's blood. Fentanyl use may easily cause respiratory depression...even in controlled hospital setting. Even misuse of fentanyl patch may result in unwanted respiratory difficulties. You're a pulmonologist...you should know that. I believe a combination of factors led to his death...which includes both the external forceful compression and also the fentanyl induced respiratory depression. PS. I am an Asian...not american...and i am just clearing the anatomical overview here. You are very welcome to have a science-based, educated comment or argument on my description here, Mike. Thanks.

      @Jack Inglis I am a radiologist so i am fully aware of anatomical structures of the neck. You cannot compress trachea only from behind without damage to the cervical spine first...unless there is a solid object in front of the trachea of course. Trachea is located anterior most in your neck...almost parallel with your thyroid gland. In George Floyd's case...his head trachea was not in a position that it was compressed significantly against the ground...his vascular structures were however, could be compromised just as i've mentioned. Significant pontomedullary damage would occur if both vertebral arteries were compromised...lets say in a severe cervical spine injury, or severe atherosclerosis. Because there are two bilateral vertebral arteries which contributed to a single basilar artery...where the pontine artery branches originate from. So, unilateral damage or vascular disruption of a single vertebral artery could cause hypoperfusion of the pons...but not enough to render it severely damaged. Because you still have flow coming from the contralateral artery...however, it should be noted also that vertebral artery is encased within the structures of cervical spine, so you cannot damage vertebral artery without damaging the cervical spine, before V4 level. There are 4 levels of vertebral artery (V1 to V4). Common carotid and internal carotid arteries give way to anterior circulation...which is separate from the posterior circulation until the circle of Willis. If one of your carotid artery is compromised...brain infarct would not occur immediately if the circle of Willis is open (notably the anterior and posterior communicating arteries). Severe condition of hypoxia (either reduction in CBF or CBV) will result in the form of stroke...or ishemic stroke.

      @Jack Inglis you can have flow disruption...or hypoperfusion without infarct. Correct. But it has to cause significant ischemia to render an organ dysfunctional. And as i have mentioned previously...respiratory center of the brain is supplied by posterior circulation, not anterior (carotid) circulation. To sever the vertebral arteries and basilar artery, from behind...one most literally break the posterior elements of someone's neck (cervical spine). Venous flow disruption? No way. If you block one jugular vein it will go the other way...because that's how transverse sinuses work...unless both veins were completely blocked, and the external jugular veins also blocked...which was just not possible with the knee compression at that angle. Have a look. The cause of death based on my analysis...was a multifactorial condition...neck compression causing hypoperfusion, with possible atherosclerosis of his carotid - cerebral arterial circulations which made matters worse, and of course his fentanyl intoxication. Fentanyl is a strong opioid used mainly in palliative care of cancer patients. A high dose of fentanyl will cause respiratory depression. That is it's more fearsome side effect. Even fentanyl pathces. When poor george said i cannot breathe...i believed he literally could not breathe, mainly not because his neck or thorax was compressed very heavily...but he literally could not breathe. So...it was a very tragic combination of factors that led to his death. The officer would be punished of course and he will be found guilty, because his actions DID contribute to Floyd's death. Chauvin should have helped him when he said he could not breathe...instead of kneeling on his neck until he died. But as doctors, we should know and assess more on that. I liked Mike Hansen and i watched his videos...but his explanations here was poor as a medical doctor. I do not care about his political bias or affiliation.

    Excellent video, and a great service to the communities. The fact that this technique of neck compression is (apparently) an approved technique in that particular department is shocking. Clearly the Minneapolis Police Department's training curriculum needs to undergo a serious external review given that this dangerous technique is being taught to officers.
    My concern of course, is that, assuming this is the case, it will provide the officers defense attorney with an avenue to plead the innocence or at least lesser culpability in the case.
    Also, I've seen the video from the bank exterior surveillance camera which depicts Mr. Floyd being cuffed, then walked to a wall, and sitting/kneeling at the wall. There is not much sign of struggle or conflict between Mr. Floyd and the officer who cuffed him, although it has apparently been claimed that Mr. Floyd's first instance of "resisting" was in initially refusing to exit his vehicle. Apparently the store clerk who made the initial 911 call reported that Mr. Floyd was 'so drunk he could not control himself,' so perhaps Floyd was not really resisting so much as disoriented.
    I noticed in that bank video that Mr. Floyd exhibited some very unusual facial expressions and bodily/postural motions during the few seconds that he walked around the driver's side of his truck past the back of his truck and toward the bank surveillance camera and then turned to his right and kneeled against the wall. I would describe those facial expression as "grimacing" or "wincing" as if he was experiencing discomfort of some sort, and the bodily motions as "flinching" or "writhing" also as if he was experiencing discomfort. I suppose this could be many things, including being intoxicated, finding the cuffs to be uncomfortable, or experiencing the first symptoms of his impending medical emergency?
    So this is my question to you doctor, and I ask it because I suspect it is a question which a savvy defense attorney will be posing to try to undermine the prosecutions case: based on Floyd's pre-existing health conditions, the level of intoxication indicated by his toxicology report, and the understandable stress which a man in his context would experience from realizing the prospect of being incarcerated (he was, apparently, in breach of his parole, driving while intoxicated and apparently fleeing from having used a counterfeit bill, so at some level Floyd probably knew that 'he was in trouble') and perhaps even being sent back to prison; is it possible that the medical emergency which led to Floyd's death had started prior to him being restrained by officers? Could this be ruled out in any meaningful way? Assuming that it could not be ruled out, then is it possible for a medical expert to state that 'Had Floyd not been restrained with compression on his thorax and neck, it is beyond a reasonable doubt that he would not have died'?
    Me personally: the senior officer appears to have completely ignored Floyd's distress and whether he caused Floyd's death partially or in full, that was an egregious breach of his duty. The statements made in the videos suggest that the officers claim will be that ". . . he was concerned that Floyd presented a violence risk as a result of 'Excited Delirium'" and thus believed that use of this pinning technique was 'necessary.' That seems unbelievable incompetent to me, given that Floyd was cuffed, obviously in a state of distress, not showing any combativeness and there were four trained officers present to keep him under control. It seems unthinkable that this officer actually would have felt that, in these circumstances, the officer actually felt it was necessary for Floyd to be pinned down by all four of them in order for them to mitigate any violence risk he could present.
    In sum, from my perspective, the senior officer if not all of them are guilty of something negligent, though I cannot say if it amounts to 3rd Degree Murder, much less 2nd. My real concern at this point is that, the level of outrage over the homicide, and the escalation of the charges--in view of the complications of establishing the exact level of culpability and the exact quotient of causality affected by the various factors (officers behavior, health conditions, general psychosomatic stress from being arrested, intoxication, etc.) is going to make it MORE difficult for a just determination to be reached. When this is combined with the fact that, finding jurors who are truly naive about the case is likely to be nearly impossible, I'm really worried that this is not going to end well for justice, the African American community or any communities in our country.

    I appreciate your independent and educated evaluation of the cause of George Floyd's death. It explains it in a way that most laymen can understand though it remains complicated.
    I was sure that Dr. Baden would have a much different summation of the autopsy than the county coroner as that person is usually trying to protect the city/county from culpability.

    That was the first thing I said when the official M.E. report came out implying that drugs and ill health made it not so much the cops fault. If he had something in his system, if he had a slightly weakened heart... SO WHAT???
    What killed that poor man was having the air slowly crushed out of him... and the rest of us get to watch another police based SNUFF FILM. Justice for George.
