RMN releases video of helicopter collision at Lumut naval base

The Royal Malaysian Navy (RMN) today unveiled a seven-second video clip showing the moments leading up to the collision and crash of the Maritime Operation Helicopter (HOM-AW139) and Eurocopter Fennec at the RMN Base in Lumut on April 23.
The video, captured by an RMN camera, was shared with local media during the disclosure event for the final report of the crash investigation.
Navy chief Admiral Tan Sri Abdul Rahman Ayob said the video revealed the point of impact between the two aircraft.

root cause analisis … fishbone atau 5S … since you know the fennec was not at the planned altitude, ask yourself; why? laydown possible causes for it, investigate and then find the factual answers … if its ‘unknown’, why is it unknown and is there a way to mitigate it from being ‘unknown’ in the future for other aircraft of the same make …
