Lumut copter tragedy: No human error concluded in full report, says Navy

Announcing its findings on the April 23 fatal Lumut helicopter crash, Royal Malaysian Navy said on Thursday (May 30) that its investigation board had not found any data that connected the incident to any human error on the part of the Fennec helicopter crew members as there was no black box on the aircraft.

It could be faulty instrument if you say that the pilot was following the instruments.. but because no black box, you have to assume the pilot was thinking he was flying the correct path and height

"human error" here means, an action that has been done but that was "not intended by the actor". The action was intended by the actor, hence no "human error".

Mungkin ini bukan penerbangan awam namun hasil siasatan amat penting..
1. Sama ada latihan kepada juruterbang adalah mengikut standard piawaian atau perlu penambahbaikkan.
2. Sama ada pegawai teknikal yang menyediakan pesawat sudah menjalani segala prosedur,
3. sama ada helikopter cukup bersedia untuk terbang dalam jangka masa tertentu
4. Apakah pesawat mempunyai masalah teknikal atau tidak.

The Navy will point the finger to the pilot error but to the outsider it is the helicopter error. Normal for the goverment to find a scape goat.
