Loved listening to this conversation! I will say that I think this maybe really helpful for daily drinkers. Started TSM after a period of six months sobriety bc of the cravings and my binges. It always was that when I drank, I drank to excess. Going AF isn’t the problem. It was when I drank once a week that I’d blow up my life 😅 really exciting to watch more content while I’m working through Dr. Eskapa’s book.
Katie...your personal commentary around minute 30 + was so very engaging and important....Merci...mille fois...for this incredible testimony with Mr. Rearden....all very illuminating and precious.
Thanks again Katie! And Hank, very inspiring! So true that maybe the next day you aren't 'functioning' but you dó miss out on joy. Great interview!
Thank you Hank and Katie. Good stuff!
Hey HANK: Animal House was my mantre!!Blaaaaaahahaha.............NOW I cannot even watch the movie because I have grown- being sober is actually REALLY GREAT! Naltraxone is REALLY helping me!
Thank you
Just the right video at the right time for me. I’m at 6 months. Doing quite well but not at extinction. Knowing that I can start giving myself a little push to experience more af days and more nf days. Trying to get the endorphins from more fun activities. I’m happy with where I am, but would like to get further toward extinction.
I don't drink daily, but started drinking in my 40's. But when I drink, which is on the weekends, I binge. I want to take the medication, but I'm afraid of the side effects, since I suffer from anxiety. I take escitalopran and clonazepan for anxiety. I also have trouble sleeping.
You can try Pranayaam for ur anxiety. It will be a gud supplement vd the tsm
Any potential side effects from naltrexone usually dissipate over a few days or so, as you acclimate to the medication. Naltrexone is generally very well tolerated by the vast majority of people.
Good advice. I already have AFD (Alcohol Free Days), but I could not find much about extinction sessions. I think SD means Single Drink? I experimented with an extinction session because i think it is needed to rewire the brain. But I took 6 halve litres beer, which is better than the 9 halve liter beer on a "regular" drinking session. But now i understand I better aim for a 1 halve litre or 2 halve litre extintion session, followed by several AFD days (if i can handle that). Before i found the Sinclair method 2 weeks ago, i could manage 3 or 5 days, and once 8 days without drinking. But that always was followed by 4,5 - 5 litre of beer. And Now i see i only did 3 litre on my second Naltrexone drinking session.
Thank you ❤ I didn’t know what SD was lol
During the first few months on TSM, it's not about how much you drink so much as is having regular extinction sessions. The most important factor is 100% compliance to the protocol 100% of the time.
@@stevecooper6515 I did this whole protocol, and it worked. So I did not drink anything since June 2022. So the whole process took one year. But in that year I had long non-drinking periods. The longest period was 94 days. So now 100 percent alcohol free since June 2022.
Thanks for this. Super helpful!
Thank you both,
What an awesome video!!
Cindy Haynes
Thanks Cindy!! Hank was great.
@Embody Daily Will I get side effects if i take Nal only on drinking days ? usually I get fatigue and tiredness if I take Nal after a month.
everyone is different, but most do adjust to the medication over time. if you're taking it once a month, I could see how there might still be some side effects as your body isn't as used to it.... i hope that helps
what is low sd extinction session?
standard drink :)
An extinction session is merely drinking whilst on the medication. SD is the American way of measuring drinks.
I’m just wondering if you are drinking to stop anxiety and panic attacks self medication how tsm works if I remove the alcohol I feel I will have nothing to take the anxiety away does a drink or two on the medication still give you the relaxation benefits? Also no one ever talks about alcohol withdrawal symptoms and dts this drug doesn’t help with that so what happens if your drinking a litre of vodka a day and start the Sinclair method and can only drink a quarter of what your used to isn’t that quite dangerous should u then start the Sinclair method once u have reduced urself to maybe a bottle of wine a day until u start
Long term, alcohol only makes anxiety worse. It's a false way if medicating which only causes more issues itself over time.
The Sinclair Method is about changing your whole relationship with alcohol, so you no longer rely on it to real with life's issues & problems.
Withdrawal symptoms are generally avoided with TSM, as you're drinking reduces gradually over several months - thereby naturally detoxing and avoiding sudden withdrawals as caused bt abruptly & instantly stopping drinking.
I have taken naltrexone 50mg now for about 5-6 months when I drink. However I think I maybe need a higher dose since I dont feel that I can stop drinking as easily as in the beginning when first starting TSM.
Have you heard anyone who is taking more than 50mg for TSM?
My Doc upped me to 75 mg. It made a difference for me.
I have yes, some people need a higher dose. There are also a number of other things that can impact progress, including how much you're drinking out of habit/coping, what you're drinking, mindfulness, etc. Would have to know more about your unique situation. I hope that helps though!!
TSM dosage is 50mg Naltrexone taken between 60-90 mins prior to your first drink of the day.
50mg is sufficient to fully block the opioid receptors in the brain & is saturation point. Increasing dosage doesn't block anymore opioid receptors.
I watched a video with a guy that needed 150 mg to work. He was super fit and had a crazy metabolism.
For the Sinclair method to work, you have to drink?
Yes
LIFE CHANGING. Take a pill one hour before you drink. Never ever vary from that simple formula. Saved my life. These people helped me to save my life.
yes, people drink in combination with the medication on this method and over time it leads to extinction of the addiction. I hope that helps!
Yes, absolutely you have to drink on the medication, for the process of extinction to be triggered. The medication itself isn't curative, it has to be paired with actively drinking.

Naltrexone is not available in Alabama. This is the stupidest thing I’ve ever heard of. Why would they not want to help alcoholics?
Hi! I think I replied to another comment. Not sure who you talk to, but we definitely have physicians we work with in Alabama inside of our program. Here is the link to our website where you can get started. www.thrivealcoholrecovery.com/
@@thrivealcoholrecovery I’ve sought treatment but was told I had to go to Georgia to get help. I arrive Sunday to begin my detox and SCM recovery
I live in Alabama and my doctor in Birmingham prescribed Naltrexone to me about six weeks ago.
This camera angle is SO terrible I can’t even listen to this guy. #nosehair.
😆 awe too bad. It's a great interview!!
