I have always loved this movie, especially with that ending. Ernest died, but in this last 37 years, he really lived. Madeline and Hellen were made immortal by the potion. But, they were confined to their own personal hell, or at least purgatory, forever dependent on the person they both hate the most, each other.

I can't believe I never noticed the pin on Helen's red dress and I have loved this movie for over 20 years. Good eye! Ooh, I just thought of a good scene. I would have liked to have seen an open casket at Ernest's funeral. We would have seen Ernest's young hand resting on his aged body. I assume the potion would have kept his hand from aging.

    Fun Fact:
Originally the ending happened 27 years later in Switzerland. Ernest is alive and old, with his wife.. sitting on the grass in a park having a picnic. They are still very much in love. Then we move to a balcony where Madeline and Helen are... still beautiful... but bored to death. They notice the old couple and Madeline ridicules them. Helen doesn't, she looks at them... then at herself and Madeline and realises what a mistake she made. Then Ernest touches the hand of his wife and we see he still has the one young hand. Madeline & Helen keep watching them as the movie fades to black... during this there is a subliminal glimpse of skulls over the eternal ladies' faces.
Because this ending was cut an entire subplot was removed and an immense amount of scenes were removed also because a big part of the third act was gone they had to even things out.

At the point where the lady who gives Meryl Streep's character the potion and says about a client saying, ".....I want to be alone....," she's quoting a famous line said by the famous actress Greta Garbo in one of her films.

The screenplay was intended as a segment in an anthology film called Tales from the Crypt. It got made into a full picture that removed a whole subplot involving Tracey Ullman as co-star; and different ending (both were filmed but never shown). There is a screenplay online with the full script and is rather good IMO.
Garbo said the classic line "I want to be alone" from the movie Grand Hotel, 1932.

15:27 I think she'd much prefer the title 'enchantress', sounds more elegant - and more fitting for someone who dresses like she does....and doesn't wear a bra......

In the shooting script there is a scene where she tells us she is 600 years old (admitting this to Ernest when she tries to have him take the potion). She shows her true colors when she admits to having killed Lincoln, Max Factor and Shakespeare cause they didn't want to take the potion.

The thing is that the potion gave them long life in their bodies but not an eternal body. That's why they were told to take care of their bodies. So if they died they were eternally stuck there even though like at the end, the body was dismembered. Kinda a curse in disguised. Not like the blessing that the Lord offers us, where He offers us an eternal body as well one day as well with an eternal life.
