Funnily enough, there is actually a "happy ending" to this. It's simple, you don't give her the brooch. Not having any reason to leave the house, she stays put. Scared, but eventually her sister arrives and turns out the lights as she calms her sister so she can sleep through this horrible night. They still have each other, and both are safe as the bars to their window and their doors remain unbroken even at the end of the game.
God, that has to be one of the most depressing story lines in Bloodborne. You try to do the right thing, yet it only yields death and destruction. The designers did one of the most gruesome thing they could do in a game, making a storyline that only gets its happy ending if you don't follow it
Probably not, since with or without you the younger one would probably have gone searching for her parents, and the sister would probably follow as well, leading to the same result DEATH. In truth there are no timelines in game where they survive

For a moment I forgot I was in a very dark game, I thought I could save her but no, that motherfucking pig (and probably whatever else was behind it) killed her like any animal would, it didn’t feel good even when I avenged her because the monster was just one in a thousand and there was no other meaning to her death so avenging her held no meaning, she died for nothing, because we told her the truth she died

Technically there is one. All you have to do is not give her the brooch. You don't send her anywhere, she stays put. Alone and scared, until her big sister arrives. Since the little sister never left, the big sister lulls her to sleep and turns out the lights of the house. The house remains untouched even at the end of the game. It's the happiest ending you'll get.
Yeah, I just checked it out. Her sister doesn't send her out. For some reason, she leaves and without even knowing where to go, still somehow ends up encountering the Pig and dying to it. To which there's no real logical reason for that... the most that can be reasoned is that she ran away from home after somehow hearing that her parents died. All that happens if you don't tell her is that you find the pig with the red ribbon and... no interaction with her sister... nothing. Pretty bullshit way of making "No Good Ending".

When I found out what happens to the little girl on my first playthrough, every new game I start or NG+ I never finish her quest line. I like to believe that she survived the nightmare and went on to live with some other relatives.
Mad Zoroark you can, just don’t ever think of starting any conversation with her or she goes to search for her mom by default. So that means don’t start the conversation you have with her when you meet her.

Eventually she will leave her home. Defeating the pig in the sewers will yield a Red (blood stained) Messenger Ribbon. Which is hers And if you don't give her the Brooch, and have visited the Oedon Chapel about it being a safe haven, you can tell her to go there for safety. If you choose the Chapel the blood soaked ribbon will still appear on the pig. She dies either way but hey there are much worst things other there than a pig for her to meet. The only way to keep her safe is to not talk to her that first time period AKA don't take the quest

The next time you visit the clinic you will receive a Lead Elixir, but the red ribbon will not appear on the pig in the sewers, which also prevents you from getting the white ribbon. But if you now enter the clinic's back door, from the Forbidden Woods, the young girl can be found inside, transformed into a celestial being. She's the one walking back and forth in the main hallway, near the room with the Cainhurst Summons. If you kill her she will drop the Formless Oedon rune. 1b. You may also opt to never interact with her again any time after receiving the music box (or just never talking to her at all) and she will eventually disappear from the window on her own, causing a similar premature end as sending her to the clinic, but with no rewards. There is no pretend sister here
KB3594 It's not fact that it's a fake sister the little girl only brought up her mom and dad because they were missing that night. The little girl and her sister are not gone even after the blood moon because you can hear the Tiny Music Box playing when near their house.

This is unnecessarily faithful to Bloodborne's storytelling. It's as grim, as sad, as unforgiving as the game itself, this animation. Great animation by the way, if a bit f***ed up (which is applauded, given the source material for this). Okay... Just got back into the game to see this for myself in-game. Recently I got a PS4. (Had to do other priorities first). My first PS4 game? I was looking games by prices so I found Bloodborne in discount, cheap af. Cool cover, I got it thinking "it might entertain me while I don't get my fighting games" (usually play with my buds, we like old fighting games) Never had played a "Souls-like" game, sure to say, I was in shock by difficulty, but soon i got myself into the mechanics. Started loving the pieces of lore, the setting, the beasts. The world itself. Until this sidequest... It broke my heart. After finish the game (hard fight, I know) I wikied any place to see if I did something wrong, tried to do every sidequest, check any place, cuz after I joined all the pieces I thought "oh crap, i killed that little girl!!!". Then there's this animation I just found. Amazing piece of art which illustrates exactly the way i interpreted the events. it's so beautiful yet so sad. Funny how games can inspire people. So, well done bro! Bloodborne is awesome. Since a while I bought a lot of Souls game to play, yet i'm still playing BB cuz I'm loving it. Thank you so much! (bad english, so y'all forgive me)

Like you are getting people to safety, you tell them to go to a church through the sewer. later, if you get through the sewers, and you kill the gigantic pig, inside will have a red hair bow. and finaly, when you come back to the house of the little girl, there is a bigger girl inside the house, asking if you know where her sister went, she have a red hairbow. so is imply that you send the little girl to get eaten by a giant pig.

well that was disappointing... I thought she was going to ride the pig back into the city and murder the hunter's ass and take back that necklace. Then they lead a happy life of murdering and devouring souls then settle down and have piggy/human baby hybrids.

Like you are getting people to safety, you tell them to go to a church through the sewer.
later, if you get through the sewers, and you kill the gigantic pig, inside will have a red hair bow.
and finaly, when you come back to the house of the little girl, there is a bigger girl inside the house, asking if you know where her sister went, she have a red hairbow.
so is imply that you send the little girl to get eaten by a giant pig.
