I love this episode, as it shows how well Scrubs can surprise you and switch the mood from high comedy to heart-tugging drama. I love how they make it seem like the Janitor is bothering Mr. McNair, given that he doesn't respond. And then comes the end where you see that Mr. McNair COULDN'T respond, and would have basically been abandoned ALL DAY if it weren't for the Janitor taking the time to talk to him like he was a real person and not just information on a chart.

This episode is in my top 3 favourites. It just captures everything that's amazing about Scrubs: takes a purely comedic figure (which is true btw - the Janitor was only meant to be in episode one, but the creators liked what Neil Flynn did so much that they wrote him into to the rest of the series) and gives even him an episode to himself and gives him an emotional level that makes him more than just a 2D character.

I once met a young woman who had something similar to this. I'm no doctor so i don't know the proper term for it. I talked to her for almost 3 hours just venting until i noticed that the computer for her translator was muted or something. I called the nurse to check on it and once she turned it back on the woman started communicating back to me. It was a very touching conversation. She told me that a lot of people don't feel comfortable talking to her because of the machine and that she was enjoying my nearly 2 hour long rambling before the machine was turned back on. It was a life changing experience for me. It's something i never could've imagined myself doing until that moment. I'm a very shy and introverted person, but talking to her just felt like something we both needed and I'm glad i did.

Thanks. Most of my shift is spent pretending to work, so I have a lot of free time to just talk to people, teachers, students, security guards/police, dogs, crickets, passerby, etc. and I've learned a lot about life and love through them. Everyone's got problems, and those that work at night are often left alone to dwell, so at least for the night there's no one better to talk to than a janitor with 5 hours left on the clock. A lot of us are a bit weird, but we're good people, at least we try to be.
We can't replace your therapist, but we're good for venting to and some of us have good advice. Before I quit I used to let this one teacher talk my ear off for 3 hours almost every night and I never had to say a word, the day I quit she gave me some inspirational autobiographies because she knew I'm an aspiring musician. We're supposed to get coffee at some point. Sometimes the best thing you can do for someone is say nothing, they already know what to do, they just want to express their frustrations.

People always lists their saddest/emotional moments but they never have this one just because it has no special effects or big building up scene's or even because it's not very long. I think this is on par to be one of the best simply because of how well Neil play's his character, just in the same way as how Ken plays Kelso in his most emotional ending. You really feel and appreciate his emotion during this ending scene if you follow the series as much as the true Scrubs fans do

“I work in a place that helps people and I guess I felt like I was a part of that. Who cares”. As a hospital janitor man do I relate to that line so badly. Whenever you hear the people talk behind your back and give you that look like your gutter trash for what you do. It really makes you wonder what’s the point. It’s easy to become jaded in my field. Even worse still is when you have a moment of doing something good it’s just brushed off by most of the staff. But it does feel so good when you hear thanks from the staff. Just one compliment from residents or patients goes a long way to helping us get through the shift. And I’m glad to see that Scrubs could show this perspective to people. We work a shitty job. It doesn’t make us shitty people. In fact many of us had big dreams. They just didn’t work out is all.

If the hospital is not clean enough,t here is no amount of good nurses and doctors that could mantain a patient healthy. The cornerstone of the hospital is the cleaning staff. If you read this and work as such in a hospital, you are the most important piece even if you dont see it.

After this show, I wanted to work in a hospital in a non-medical role. I have no interest in the medical field but I liked the thought of being adjacent to it.
I got that dream for 7 months through late 2013 and part of 2014. I got to work as a computer support technician. Basically an IT guy supporting computer equipment in the hospital. A lot of my work was dealing with these buggy cart mounted computers that the nurses used to.... do stuff. Some also had drawers where they could store little things that the nurses needed, like tape, straws, gauze pads etc., Doctors probably did stuff too. My favorite part was always when I got to interact with the patients when their TV sets broke. They weren't normal TVs. They were basically computers hooked up to a monitor, connected to a system called "Get Well Network", which had TV channels and hospital services.
I loved that job and I wish it weren't just temporary.
But support jobs like that and janitorial services are critical to a hospital. Or any business. The place cannot function without the non-medical staff.

People wonder if Janitor ever knew Mr. McNair thanked him, but I think he somewhat knew. Dr. Cox was acting all... cocky since he "saved the day" with his new laptop, and that all went away when Mr. McNair looked at Janitor with Dr. Cox looking back in confusion and a deflated ego.

You know, this is fiction, but people like these do exist. I've been in an hospital for 3 weeks last year, my hip joint fucking exploded, and it was unbearable, but there was this one janitor, Ciccio (short for Francesco over here), he'd came in every day to clean the room, and he'd go as slow as possible in my room, so we could chat. He was very hopeful for me, chatting with him was nice, he wasn't a nurse or a doctor but he genuinely cared about patients down with their luck. He was happy when he saw me walk on my two legs when I had to get there again to get my leg checked. Sometimes you don't make the cut for medicine, but you can still make people feel better.

Janitor is basically every guy with his job. I work at a school and it is the same thing, people don't give a crap unless they need something from you or you did something wrong. I work with my father on the custodial crew and for the first 5 months of me working there no matter how many times I told the teachers and principals my name they still called me, "Domingo's son". However, once they figured out my name it was every morning I say, "Good morning, how are you today, Mr. or Ms.?" and they either ignore completely or they would give a quick, "Hey" without so much as looking at me. It is when they want something done that they start acting like they are my best friend, same with my father they treat him like an idiot but we just go to work everyday trying our best to make the school the best it can be. Not for the teachers or principal but for the students to learn in a clean and safe environment. A lot of times we are the ones students come to with their problems, I think it is because we just listen and not always teach. All we want is to be seen as genuinely important.
