I’ve worked every shift and have even got surprise admissions on night shift before!!! Started agency and it’s soooo hard doing admissions and not being familiar with the dang on charting systems... but it gets done! Thanks for all you do.

Just to piggyback off what you said about pain, I would also ask if they are on narcotics and if so are they sending a script. It saves time when getting the meds from the pharmacy opposed to the doctor/np calling it in after med are reconciled. Also if the patient is on dialysis, when they were last dialysis Ed, where they go, schedule and how they go (w/c or stretcher), if they had an IV was the line removed or not, also concerning catheters when were they last changed. Ask about wound treatments if they have wounds. Most facilities have a report sheet but if they don’t I would recommend making one for yourself and make copies, I would also do this for when the patient arrives and you interview them and do your head to toe assessment.

YEAH!!! I love this video. Girl I had to quit a job recently. I was working with an irrational nurse leader who was sending residents to the building without notifying anyone! They were just showing up and the staff had no time to prepare! I realized she didn't understand continuity of care and preparation. I totally saw it wasn't important to her to be professional. But I had to quit because she was flexing and acting like she was the shit.... knowing full well, she was not doing anything that looked like nursing. I was bugging out at the arrogance of this woman in all her lack of professionalism.
I am a new LTC nurse. Had no training on admissions. Came to work my first 2-10 shift. The 6-2 nurse hands me a stack of paperwork and says you just an admission 15 min ago. Let’s do the med cart counts so I can leave. I tried my best to look in a binder but got overwhelmed. They paperwork Just sat there. I didn’t have a clue what to do. 😭😭😭
Ask how well does the patient swallow and how does the patient take their meds. Do they take them one at a time, all at once, or crushed in apple sauce.
I hate when supervisors/managers don't help out with things like admissions. Where is the teamwork? They don't care about anyone but themselves, and that's really sad.
Question: do you also have to do MRSA swab for patient coming from different facility/ hospital? And once the patient arrived at your facility do you have to insert an IV cannula or not necessarily? just came accross to your channel and i found it very informative and helpful. u got me subscribed coz i loved how you impart your knowledge and your enthusiast in helping other nurses most especially the newbies.

Its sad but alot of times the hospital nurses are in a hurry and may not give a full report
