    I'm at 13 weeks today. 7/30/2024 I'm soar and stiff all the time still. Still having trouble with sleep and pain. Dr gave me gabapentin for the nerve pain. I know I'll get through this but remember it's a 6 to 9 month process

    This really is bad recovery. Training until pain, weakness and stiffness returns and needing days to recover. These things will snap like twigs and then you are really screwed. Even the smallest stress will attract fluid around the repair and that will set you back. You need this thing working properly for the rest of your life, what is the point trying to advance the schedule set by the surgeon.

    I've watched a few of these vids and really have reservations about the advanced exercise schedule. I had complete tears in 3 of the 4 shoulder tendons from heavy muay thai training when i threw a right hook away from my body against a 120kg heavy bag. The momentum of the bag opposed the power of the punch and I felt at least one snap. Turned out my supraspinatus was irreparable due to the acromion rubbing. So I had major arthroscopic surgery with an acromioplasty and biceps tenodesis. The tendons were a mess and frayed from years of weights and impact. My recommendation... I had 1 shot at making this repair work because the surgeon improvised to reattach the supraspinatus. The entire first month was spent in the sling with progressively more frequent dangling outside of the sling in a passive range of motion to relieve stiffness. There are no prizes in trying to be Superman and thinking you can speed up recovery. Shoulder tendons have terrible blood supply and any stress will cause fluid surrounding the attachment. This will set you back and leave it vulnerable for reinjury. Most failures occur between months 1 to 3. Maybe because mine was more serious than most, I attempted little active stretching and strengthening through to week 10. This is the point at which repair accelerates. Between weeks 10-13 (3 months) the shoulder has shown remarkable progress daily, even with minimal work. At no point have I ever stressed the shoulder. In the videos I see these quite normal exercises done with load and the result is pain and weakness. But he does it again the next day and the next. The tendons are only about 25% repaired and you will invite reinjury and have to start again, if even possible. Take it easy, range of motion is your next goal not strength. Loading up the tendon is dangerous but gentle stress from stretching will be very beneficial. So even with my very pedestrian recovery program, by week 15 I had full range of motion, no pain and a shoulder ready to begin resistance work. Between 4-5mths size returned and there was no perceptible difference between shoulders. At 6mths I would consider the tendons were about 80% healed and I was ready to consider myself past the point of reinjury although I have not attempted any impact training since. I strike with my legs, left side and shadow box on the right. There is no such thing as 100% repaired; there will always be weakness at the point of reattachment and nothing beats the natural means that tendons attach to bones. My advice is to use pain as an instant warning but do nothing for at least 4wks. I was told it would heal in 6mths and without any physiotherapy or additional expense, I had a shoulder healed in 6mths with full range and 80% strength. Tendons are not like broken bones and you will probably only get 1 chance at it. Don't be a hero, you don't win a trophy if you are 3wks ahead of progress because everyone is different and have different levels of severity. Just remember the pain and inconvenience of the injury, operation and first month of recovery. Don't f*** it up! Good luck.

    Greetings from Germany. I am on day 7 post surgery (supraspinatus tear). I was so afraid doing too much or being too active, but once i see what you are doing I am really relaxed. :)) As you said, once i stopped to guard my arm on day3 it got way better! I only need pain medicine before night. Last but not least I started PT on day 3 and it helps so much, even if it is kind of painful.

      Yes, that’s exactly right. I used the ice machine and ice packs liberally for the first several days then continued with the ice packs for the first full week.

      I took 1000mg of Tylenol four times a day, every six hours with liberal use of the ice machine and ice packs.

    8 weeks post-op here. Full thickness tear with 5 anchors. Excellent advice. Doing many of the same movements and I can attest it is a rollercoaster ride and I'm an impatient person by nature and i have to remind myself, this takes TIME. An interesting thing to me was 'packing' the shoulders and I will bring this up at my next PT appointment. Appreciate the videos!

      Glad to read the videos have been helpful. Be patient, take your time, and you will give your repair the optimal chance for an amazing outcome. Packing the shoulders is a game changer. By having my shoulders in a neutral position, it allows more efficient movements, better posture, and less pain. I still pack my shoulders to this day, in fact just today I was packing my shoulders while doing Crossover Symmetry activation. It has been super helpful to me. Hope it helps you too.

    I'm a bit frustrated with my doctor, I'm on week 2 and still not allowed to remove my arm from the sling (I have, though, to shower using a shower sling and towel under my arm). From what I have read there are 2 camps, one PT in the first week and the second no PT until after 2 weeks. Week 2 for you seemed to be good progress with movement and PT. I also have no real pain, like you, more discomfort at times. Every so often I'll move a certain way and get a sharp shooting pain which goes away almost immediately. I am sleeping well, but my arm falls a sleep, so I do wake up a few times a night. Thanks again for these videos, they truly help and encourage.

      PT was absolutely crucial for me but that PT was very limited to only passive range of motion. In other words my PT and my wife took turns moving my arm for me so it didn't get locked up. My surgeon also prescribed passive range of motion starting on day 2. I did keep my sling on, with exception of doing the aforementioned PT and taking a shower, where I used a mesh sling instead of draping my arm to the side. I would highly encourage you to talk to a good PT and ask them what they recommend. I believe if I wouldn't have used passive range of motion early on, I wouldn't have had such a good recovery.

      I did not do massage. While I am not a healthcare professional, I would advise against it unless your PT is doing myofascial release or similar treatment. To me, massage by a non-PT could potentially be hazardous to my repair. Once fully recovered, yes, go for that massage.

    He wanted you to do all that day 1 and only tylenol. I call Cap. Especially when you said you took off dressing after day 2 😂. Thats a five day minimum. Your doctor never graduated

      My surgeon prescribed opioids which I fortunately did not need. I've been on them before and don't like the way they make me feel or how they slow my digestion. I was lucky that ice packs, an ice machine, and Tylenol did the trick for me. There was one day I was super close to taking opioids but my pain diminished enough I didn't take any. Later in recovery I used NSAIDS when I had discomfort. The movements my surgeon prescribed were simple passive range of motion, which my wife assisted me with. We did that 2-3 times a day until I got into physical therapy on day 6. It is normal for a dressing to come off after 48 hours, at which time they put stern-strips on instead of the big gauze pad which was no longer needed. My surgeon was awesome and I had an amazing result. I'm now pain free and am lifting heavier weights than before surgery. Best of luck to you with your surgery and or recovery.

    Hi. When would you say you were able to do all CrossFit movements without restrictions? Can you do all the Olympics lifts again? How about the gymnastics? kipping, butterfly, etc..

      It was 1 year for me, but my surgeon and I played it very conservatively. We were both thinking long-term. That strategy has paid off well. Yes, I can do 100% of all CrossFit movements. BMU, RMU, Kipping PU, HSPU, Cleans, Jerks, Snatches. I have consciously not done heavy snatches, but it's never really been my lift. It's not a limitation of my shoulder in any way. I've pushed new PRs with everything else.

      @@Albartdeguzman I Rx'd some things and scaled others. I'm 56 and before surgery could deadlift 450 lbs 1rm, but from 6 months to 12 months post surgery, I worked my way back to that 450 lbs. I used this strategy with other lifts like bench press, overhead press, push presses, thrusters, cleans, jerks, etc. I did move my back and front squats up more quickly because I didn't need to wait to move those back up. Another example, I could do 30 kipping pull-ups and about 8 bar muscle-ups unbroken pre-surgery, but worked my way up from banded strict to strict around the 6 month mark, then kept strict until about month 12 post-surgery, then started banded bar and ring muscle ups at that one-year mark, working into full bar and ring muscle ups the next few months. I was unrestricted at month 6 on most things like burpees, pushups, any leg movements, box jumps, kettlebell swings, sled pushes/pulls, any concept 2 machine, echo bike, assault runner, etc. The basic concept was to not stress my shoulder repair right away. To me, my surgeon, and Doctor of Physical Therapy, we agreed that I had a super successful surgery and would be able to return to sport 100%, but there was no need to rush it. The process of tendon healing takes 12-24 months to be 100%. I could still workout and be in great shape with lighter weights and non-dynamic movements. I didn't want to risk going all out on a repair that was 80-90% strength. There was just no need for me to do that, so I took my time and my result has been outstanding. No pain at all, none. I have been hitting PRs again on heavy weights and many dynamic movements.

    1 week in, sleeping in my bed after 2nd night. Pain is minimal, I have been taking OXY only 1 a day before bed and get up around 2:30 or 3:00am and take Benadryl to relieve itching (also helps with sleep). I've had a couple of mishaps that were a bit painful. One, my wife accidentally hit my shoulder, the other my pup came up ang licked my arm which made me jerk my shoulder up. Not really taking anything for pain throughout the day, I have only taken Motrin or Tylenol a couple of times over the last week. I keep a journal of my medication and pain levels, the highest pain was an 8 after the nerve block wore off. That was short-lived as I took Motrin and an OXY once I felt it wearing off.

      You have a lot of similarities to my experience. One thing I learned that may be helpful is that if you don’t have any pain five minutes after a small incident like you experienced,, you are good. I had a few things happen, like you, but no issues after five minutes. My recovery went well.

    4 weeks in and still in pain and its worse at night. I'm surprised they started pt so soon with you. Im just scheduled to start at 6 weeks

    Summary was impressive, thank you. You are a blessing. I’m currently in recovery from a horrible motorcycle accident which tore both of my shoulders. Left-full tear, right-partial tear. We went to see a surgeon and he told me that I had a “frozen shoulder”, he didn’t recommend surgery. I ended up seeing a second surgeon which did not recommend surgery either. Currently I’m curious after speaking to some folks what to push for, they said surgery is the last choice. After watching your video it seemed like is not as bad as I was told.

While shoulder surgery recovery is long, I have a very good outcome. It wasn’t that bad for me. I am completely normal, pain free life.

    How are you now? Did you get any scapular winging/scapulothoracic dyskinesis? My surgeon told me this is whats causing weak scapula and the shrug movement when doing bicep curls. 17 weeks post op from bicep tenodesis and distal clavicle resection. Still lots of pain from impingement. Apparently I need to strengthen my scapula muscles.

      I started out unintentionally guarding by shrugging my shoulder toward my ear. I also use a computer and type a lot behind a desk, so my shoulders were rounded a bit. I re-learned how to pack my shoulders by pushing my shoulders down and squeezing my scapulae together. It feels like putting my shoulder blades in my back pocket. This posture was enormous for recovery and staying healthy after. You can find that in a section of my week 8 video.

    I really appreciate your dedication into doing these videos! Thank you! I am in week 1 after my supraspinatus + biceps surgery. Your videos encourage me and I hope that I don't stress myself too much with the progress and i now know i have to be patient. I am 33 years old and did gymnastics and do crossfit now. I would be interested to know how you are doing today as some time has passed since your last videos. Are you doing snatches, HSPU, MUs etc... again? I can't imagine an active life without this sort of sports 😢 Regards from Switzerland 🇨🇭

The good news is I am now completely pain free and doing all movements CrossFit can throw at me including Snatches, Jerks, RMU, BMU, Thrusters, HSPUs, etc. Now my left shoulder, which didn’t bother me before isn’t doing as well as my recovered shoulder. Not enough to require surgery though. So yes, I had a great recovery. The only thing I would do different not to have distal clavicular resection done. I don’t believe it was necessary and it has caused a few minor issues. Best wishes with your recovery!

    interesting.... i had basically the same ..bicep tenodesis rotator cuff repair. i had to wear sling for 4 weeks. except in shower. by day 23-24 id take it off for few hours. started PT after the 4 weeks and never wore the sling again. its 10 PT visits. once a week. only in visit 3 now. it does feel little better everyday. still long way to go. 3 months. back to work. then 3 more months for complete recovery/heal where i can lift heavy weight/gym/ contact sports. true, sleeping is hell first few weeks. i slept in crappy recliner and moved to couch. stayed up til 3-4 am some nights, and napped during days. all in all pain was very little after surgery and after nerve block wears off 24-34 hours later. (i was made to believe was going to be in hell afterwards). i get some dull aches in area where they drilled in bone sometimes and mainly in morning waking up it always quite sore. took only 2 oxys. didn't do much for me. the worst was the blood thinner medicine that plays hell with stomach. i went 15-18 hours without anything to eat for days. at times. and i got some weird nerve pricks down my forearm. and felt some in my neck and jaw/teeth.

      Recovery is definitely different for everyone. My surgeon was more conservative with me than other patients because I am so active. I could have been back to normal in six months, but decided to prolong that for a year to be 100% sure I could lift as heavy as I wanted to with no limitations. Process of. Tendon healing is 12-24 months to be 100%.I’m glad your recovery went well.
