    My liver enzymes ALT and AST went very high...Doctor took me off Methotrexate because of this, but a month later my numbers are still high. Doctor told me to start taking three folic acid every day instead of one a day.

    I started with Biologics. They worked well until they stopped working. The first one had some weird side effects, though, and I was glad to be off it. I’m guessing that I built up antibodies to them. Rather wish I hadn’t-it be nice to go back to the 2nd one that didn’t have side effects. I’m on MTX now and either it doesn’t really work or I need a higher dose-I’ll have to sort that out in a few weeks with my rheumatologist. I’m really glad biologics are available to people.

    Thank you for going into detail about how biologics work. The drs never go into this detail. I always wondered how my Adalimumab has helped reduce the inflammation with my nr ankylosing spondylitis.

    Thank you for addressing this disease. I was diagnosed 6 years ago and it’s horrible. Utterly painful. My rheumatologist offers me plaquenil only. I can’t take it. Broke out in terrible all over body hives, chest pain and vertigo. Discontinued the medication and no hives, no chest pain and vertigo vanished. He offers me nothing else. Must have to do with the insurance. Very grateful for doctors like you doing videos on this disease. Because it’s difficult doing research on it!

    I’m so glad you discussed this issue. After years of various symptoms including plantar fasciitis I was eventually diagnosed with non radiographic ankylosing spondylitis. Initially I was on meloxicam but this wasn’t enough. I have been on Adalimumab for two years and this has helped a lot with the axial inflammation, especially in my neck. However I am still struggling with peripheral involvement in the joints you mentioned. I have been on 15mg of methotrexate for a while given by my rheumatologist to help with this. My archilles heel though is still inflamed. This seems like the most difficult area to heal. It just locks up when it feels like it. Any suggestions

      No medical recommendations here other than: talk with your rheumatologist as they should be able to help :) You could also get an ultrasound of the area to see what type of process is happening

      @@barefootbythecreek4859 I’ll do a live on drug induced lupus and we can talk about that! In general in auto-immune disorders we think of antibodies as an association/sign of autoimmunity issues. But the truth is that we aren’t always certain and in some cases as the disease improves some antibodies go away as well and as such we think they may be “harmful” when we aren’t actually sure if they are or just a sign of the condition. Fortunately, in practice, the goal is remission and as such we aim for no clinical symptoms. The antibodies may or may not follow, if there is no more clinical symptoms that means remission. I hope this helps

    Why do you always go to plaquenil it makes you have skin rashes but you doctors don't tell your patients??? I was put on it 2 yrs ago and it made me breakout in to rashes on my arms and legs. There are no good news when you are talking about a disease that will just attack your body and destroy your organs. I'm a 63yr old woman that just got diagnosed with this awful disease and you're talking about long term life??? Why don't you just tell the truth and tell your patients they are going to die from this disease. I have Burning Mouth Syndrome also which is a fight everyday from the time I get up until I go to bed at night. You need to talk more to GP's and educate them in paying attention to their patients and the symptoms instead they sweep you under the rug and send you on down the line of other doctors and specialists. I take 3mgs of prednisone everyday just to get through the pain. STOP giving patients Plaquenil and think that is the answer because it's not. Please make sure you tell your patients all the side effects that comes with the medication. I know you only have your 15min time with each one so don't waste their time because they are the ones who leave your office and have to go home and deal with all of the pain. Pain you doctors don't even want to give anything for the pain. You are so scared that if you do you will be slapped on the hand for prescribing it. So please don't bullshit people with all of your lies. By the way what do you think you would feel like if you lost all of that hair you keep flipping around. I had hair like that and I know it is gone add that to the disease which you should also go over with your patients. Do yourself a favor and tell the truth about Sjogren's Syndrome and not the bullshit of how you care.

      Yes, it is. I have GPA vasculitis and have had the PR3 ANCA that she mentioned in passing early on.

    Hi. I was recently diagnosed with psioriatic arthritis. Rheumatologist suggested a Methotrexate, but said maybe think about it first. When is the right time to start it. At the moment I don't have any swelling except right hand and my right ankle. I get pain every day in different joints and at night my back and foot. I am just not sure if this is the time to start taking the meds.

    This is very interesting. Doctors need to be excellent at reverse engineering it seems. You are basically given a highly complex system - the human body - and you experiment with providing certain inputs, observing the output behavior, based on which you attempt to define what's happening in the body. As an RA patient, I really hope the medical research community makes advances in the coming decade, especially with the help of AI.

    I’ve had 3 positive ANA tests and have seen 2 rheumatologists. The first one told me that he thought my ANA test was a false positive. The second one has diagnosed me with psoriatic arthritis ( I have psoriasis). He put me on methotrexate and I will say the first dose (injection) seemed more effective than the pill form. My fatigue seems to be much better, but one symptom that I didn’t think was related is that before the methotrexate is that if you lightly ran your fingers over my arm, it didn’t feel good. I wouldn’t exactly call it pain, but more uncomfortable. Now, it is comfortable and feels normal. I do have arthritis and am certainly confused as to whether it’s osteoarthritis or psoriatic arthritis. Some articles say psoriatic arthritis is not bilateral and osteoarthritis is bilateral. My rheumatologist says osteoarthritis is not bilateral. So there’s confusion in that. I guess my question is, could my skin sensitivity be part of some other kind of autoimmune condition? I wish you were closer, I would love to make an appointment with you. I live in North Carolina.

    This was really useful thank you. Can you have a positive RNA Polymerase III and not have Systemic Sclerosis. Or can you have a weak positive. As there is so much crossover between autoimmune issues it is likely to be overlooked

    Do you have any autoimmune conditions? Also, how effective have you found diet changes to be? My PsA flared badly a couple weeks ago - to the point I couldn’t bend over to pick things up - so I fasted for two days, then cut out all wheat and all sugar, and so far the flare is gone. I hope it stays that way. My Dermatologist wants to start me on Taltz, but I almost want to hold off and wait to see if diet works first, cuz I don’t know how I feel about suppressing my immune system when I’ve already been getting so many sinus infections, etc.

    Wish I had the money to see you, especially as I’m near Denver for a few months. I would love to do direct care for all my healthcare needs. I think that’s the best option for anyone who can afford it. And I would love to create a health plan for my RA with a rheumatologist who believes in treatment plans that are more comprehensive than just prescribing drugs. I am a huge believer in lifestyle changes. Seeing your channel gives me hope that there might be more rheumatologists out there like you.
