The doctor's notes line was referring to how doctors always seem to have really messy writing, which is also how someone would probably write after being tortured.
I'm pretty sure the doctors note bar was in reference to Guy Fawkes who was tortured to the point he couldn't even write his own name in the same way Doctors just scribble while not being readable, with Rustage being English the link makes sense to me.
