What does Pars Defect in the lumbosacral region signify? - Dr. Hanume Gowda

  • Lumbosacral region is part of the spine which is your lower back where there are 6 joints, 5 in the lumbar region and 1 in the junction of the lumbar and the sacral region. So between the joints, when we look at it anatomically, there are 3 joint areas. Between 2 joints in the front is one joint called the disc space area. In the back there are 2 more joints and this allows for the small little movements which can happen. The spine as such has 29 motion segments. So that leads to a complete bending, straightening and the rotation of your body. So what happens is the pars defect is the articular area of the lower bone and the articular areas of the upper bone need to form a joint. Similarly the same upper part continues to form one more articular are for the upper bone.so the junction of the 2 articular surfaces in a single bone is called the pars and this term is restricted only for the spine and it is not for any other parts of the body. So this gives the stability so that the bones should no move forwards and backwards. So when there is a break in the bone in that area, it is called as defect in the pars or when it is technically called as spondylolysis. Spondlye is spine and lysis is something that is broken or something that is eroded.so that is called as lysis. In that case, the patient experiences a sort of backache which is very very limiting and which is limited to a very small area of the back and it is excruciating and more so when thy bent backwards, they tend to get more pain. When they have extension of the back, thy have more such pains and the second thing is the spine become unstable. This pain is because of instability. What is this instability? The 2 bones have to oppose on each other and stay there. When one of the bone start moving forwards or backwards, it is called as listesis, that is called as slipping, the bone starts slipping. So that is when there is a defect in the pars area and it is on both sides, that is on the left side and on the right side. So if both the pars are broken, then there is a chance that the upper bone will move forwards, compared to the lower bone. So it is a bigger mechanical problem. The commonest problem what can occur is the pain and the second thing is what we call as stenosis pain that is a pain which starts in the lower back and it starts paining in the thigh and the lower leg, more so when they walk for a longer distance. We also call this as neurological claudication. So these are the common complaints. Rarely patient may develop neurological deficits, in which there is weakness of the lower limbs or loss of sensation in the lower limbs or some sort of disturbance in passing urine or stools. So these can develop but it is rare, but most of the times the patient can develop excruciating pain on movements.

      doctor should give you either a brace and physical therapy, steroid injections, or either a pars repair surgery or a spinal fusion

    Hard enough to understand Habeeb, let alone poor demonstration holding the vertebrae model far away from camera. Good video and sound quality notwithstanding. No offense just being short to the point for all native English speaking peoples. I would edit or record new video and speak slower 25% with more clarity and do closeups with more thorough demonstrations on vertebrae with simple analogies for the avg Joe.
