Objective data for this would be an increased respiratory rate that you can count. Or accessory muscle use that you can see. Ayo Oluwasola, great question! This would be subjective, the patient is saying they are itchy, it is hard for the nurse to replicate that or observe that on their own.
How about if it is stated by the watcher, for example Jake's mother states that his son has a throbbing headache? How can we classify this one? Thank you.
Great question! I believe that is still classified as still being subjective. You would definitely want to document that it came from the patient's family, rather than the patient, as well.

Kathleen Villanueva, this can get tricky, great question! So objective things are observable and measurable, whereas subjective are more symptoms or what the patient is telling you. So you would need more specific information on whether you know the age for a fact based on data (DOB), and the same for gender based on genetic data. Or if it is the patient telling you these things. I hope that helps to further differentiate between the two!
blessed and favored, objective is facts, something that you can observe, think objective observe. Subjective is something that you cannot really prove. Something that the patient is feeling, something you cannot, as the nurse, experience or view. I hope that helps to clarify it for you, you are doing great!
