@FSXISCOOLThey monitor their position in relation to the runway and monitor how the aircraft is flying during the entire approach and landing.. at any point something goes wrong they cut the autopilot or press toga and abort the landing... the airplane doesn't know it's in cloud... so really, every landing is the same.. the only difference is what the pilots can see out the window.
I (somewhat) illegally went to 100 ft and nearly shtitted myself. Hand flying naturally I wasn't as well lined up. Just the wife and i in a Turbo Arrow, so I still shouldn't have done it. Had airport environment in sight so I thought. Still, I knew I could power out of any danger. That's what I told myself. I never pushed it again. To this day I have never gone below maybe 400ft.

I love landing during fog, it's a bit scary and suddenly the approach lights come in sight :)
