What diameter & steel grade the anchor bolts? What psi strength the concrete? Was a slump test performed on the concrete? Do the rebar need welding to each other, or just tie wires? thx.
OH, & what is min. concrete coverage from wood form to rebars? Seems like 2" comes to mind , ,

Thanks for your comments,Anchor Bolt diameter & grade, concrete grade are depends on design loads. where as rebar weld are not necessary and yes, slump test is advised before every concreting...

Very good animation, but I suggest the following improvements to the subject matter, specifically related to the anchor bolts. First, there should not just be a deeply embedded hex nut. It should be a hex nut / fender washer / hex nut sandwich. Second, above the embedded “sandwich” there should be a concrete-bond-breaking tube or tape to allow axial movement of the bolt w.r.t. the concrete.
