They can split it by percentage of income rather than equal amounts of money because if she has to contribute the same exact amount she will always end up with less based on her income. The more fair thing imo is to make a joint account and each contribute like 20-30% of their income instead of the same dollar amount and that could go to their shared bills. So that's 12000-18000/year for the teacher and 120000-180000/year for the doctor. I think that is more fair considering the difference in their income and they can still keep 70-80% of their personal income for their personal account and use those for their personal bills. Just based on their careers and schedules, it is more than likely that the teacher will be doing a lot of the household duties because her schedule would match the school schedule and depending on what kind of doctor the guy is then he will probably have a busier schedule and will less likely be home. So the home maintenance and upkeep will be on the wife which would make up for her smaller income compared to his.

I pay the smaller bills like groceries, water, electricity, household stuff, and my husband pays the mortgage. We manage our spending money individually. As long as the bills are getting paid, we don't have the need to discuss big purchases.
