This is a really great tool to understand hormone replacement. As both a NP and a man in my 50s with crazy low testosterone, (below 120ng/dl) it taught me tons. Holy crap am I having symptoms, but finding someone to do pellet testosterone replacement as part of all hormone optimization in th OKC metro area is a nightmare. Any recommendations? I wish I lived closer to TN. I'd come see you and be a patient. Is Farideh still a part of your practice? I really enjoy her contribution to your videos, and it's nice to see NPs like me involved in very high quality advanced care.
My only option is the franchise Low T clinics in OKC. I have all of my labs, including SHBG, estrogen, prolactin etc, as well as my TSH, t3/t4 etc. It's frustrating to find care. I'm a healthy man in my early 50s and not satisfied getting "fat and old". If you have a peer in OKC, Dallas etc, we in the Texas/Oklahoma area would appreciate any help you could offer.

This can cause dramatic impacts in many people's lives. With hormone pellets how would a patient go about having the pellet inserted if they are on vacation or traveling out of country. How would that issue be addressed when they are not at a "home" location with their clinic?
72 yo male here. I was taking Testosterone Cypianate for two years but had to quit because of prostate cancer treatment. I'm told by my docs that I can resume testosterone if my PSA continues to fall. I cannot wait! I never felt better in all areas

So im 52 been feeling like 💩 for the past 4 years or so and its been a progressive decline that i can tell...my total testosterone has been all over the board from the high 200s to up to 454 and free was anywhere from 9.2 to 16.1 and those numbers are definitely low considering my ongoing symptoms so i finally got to see a urologist this past week just find out i was his very first patient at his practice that had just opened that day just for him to quote from his medical books that oh you're within the normal range you just need to exercise which i do weight training 3 to 4 times a week just to then say oh well you need more cardio and then finally trying to convince me i might be depressed SMH....on to the next doctor because this one is joke and i could have explained how testosterone works and how we make it better than he could.
Question for the Doc. Went on a few rounds of Test. About 6 months worth at 200. Went from 390 to 1500 in a yr then back down to 390 or so with almost no free test. Went from lean 165lb to now 185-190 and soft, tired, not as motivated to be active like before. Something Backfired.
I would get your shbg checked. If all your T is bound up with shbg it can't be used. Boron helps, but does not end all. I would definitely say you need to see a doctor. Could be your AR receptors.
You're not telling the whole story here about drug routes- and your biases are showing. Topical and injections have a different set of pros and cons. Injections tend to be easier to dose accurately. They also tend to be cheaper. They are easier to remember 1-2 doses per week rather than 2x/day. Once you inject, you're done and can do ANYTHING. They tend to convert less to DHT. However, they DO convert more to estrogen and they DO tend to make your HDL/LDL cholesterol ratios worse. Topicals tend to increase DHT and often times that can be a benefit to building muscle and better sex drive. Topicals tend to aromatize less. Topicals also tend to produce a more consistent feeling to it (you will always have highs and lows in injections, unless you do so daily). And, topicals are very well absorbed- especially with formulations that are used on the testicles themselves. The main downsides to cream are transference, absorption time (you can't sweat or get wet immediately after dosing), and you have to do it twice/day. I get it that you prefer injections (and that's 100% fine), but be balanced and let patients decide what might work best for them. Both are perfectly acceptable options that many men get excellent results with. Testosterone cream works extremely well for lots of men! And, sometimes injections might not be best if someone can't really inject themselves or if their LDL is already a little too high. For them, cream might work better. Likewise, creams might not be the answer for men who do a lot of physical labor and sweat all day or those who might forget the 2x/day dosing. In that case, injections are CLEARLY best.
I was on testosterone for over 15 years probably longer than most men on here. I had alot of good years but the last 2 were horrid. Drs dont know the long term side effects of arrogenous testosterone. I tried every protocol. I got off it 2 years ago and went on animal base diet and my Testosterone flutates between 550 and 700. Im not agsist TRT but guys try lifestyle changes before you jump on it.

Don't do pellets. Do injections into your fat with 29g insulin needles. You can fine tune your dosage week to week this way. Pellets make no sense

Look up BAT therapy for prostare cancer they use high doses test

Also you said a few rounds...you have to keep going. It won't stay higher. You have to keep getting treated.
