You're right, it is a pretty desperate play all things considered. Basically, Ellsbury stole home because he could. He took a big lead off of third base, took advantage of the fact that left-handed pitchers have their back to third base, and waited for the pitcher to start a long wind-up pitching motion so he would have enough time to make the run.

Besides the fact that there's virtually no downside with the wind up (the above video exemplifying such risk), it often calms down the pitcher's nerves by putting them in a more comfortable mindset, which helps when you've fucked up enough to load the bases in the first place. A lot of pitchers will even pitch from the wind up with runners on first and second, just to help them out mentally.
It'd be allowed sure, but to see it actually happen (a full swing) would be very rare. When a runner is heading home and the batter is swinging away, this poses a huge chance of injury for said runner. Most the time if a runner is going home and the batter is in the box actually looking to make contact, it'll be nothing more than a bunt, which is called a squeeze bunt. If the batter is swinging away, I'm sure there would be big problems between the runner and batter after the fact.

Bases Loaded, 2 outs and with a 1 run lead. Of course, this was in the fifth, while Vandy pulled off their heist in the 9th. Still, this is a impressive and gutsy call for Boston to try this.

Not really. Pitchers tend to watch runners on 2nd & 3rd closely. Stealing home is so rare it's common to completely ignore runners on 3rd, as happened here.
