Did This Plane Crash Because the Pilots Read a Chart Wrong? | Air Disasters | Smithsonian Channel

- The data from PIA Flight 268, which crashed on September 28, 1992, reveals that the pilots didn’t follow Kathmandu Airport’s meticulous descent chart. But how did an experienced crew miss such a basic step?

it seems so unfair that the pilots bare all the responsibility and their chart was so crappy and cheap.
Cost cutting. Pressure on all for on time performance. They just want to get the airplane in air. Sometimes windshield are also not cleaned properly because no ladder is present. I have myself cleaned the windshield because to get the tressle will make the flight delayed. Landing lights not working. Pathetic state of working and finally everything will be blamed on pilots.
to be fair, pilot should've known better. making clumsy mistakes like that is just inexcusable
Many navigation systems will control the autopilot and fly the plane down to the inner marker. You can also blame the investigators and or Smithsonian youtube channel for not knowing what they are putting out. But it protects any navigation equipment manufacturer.
The maps may need to be reviewed to be more user friendly but at the end of the day he is a pilot and he should take precautions and use more critical thinking when flying over known-to-be-dangerous mountainous terrain

I always love seeing this series recycling the actors.
The blue-shirt investigator apparently had survived a lost 737 (DHI574), a 737 overshooting a runway (GIA200), a 737 landed in a river (GIA421) and after all of those circumstances, now a seasoned air crash investigator
Actually he was the first officer in DHI574 episode, he died there so apparently hes god.
In the Thai airways episode he was both playing the captain and lead investigator in the same episode.
You gotta wonder what the budget really is for this show sometimes.
@@farhanatashiga3721 in the thai airways episode, the captain was a different actor and both accidents happened around the same time so its make sense to use the same investigator.
🤣🤣🤣

People should remember this. PIA were once the shining example of how to operate a national carrier. They gave a lot of help to Emirates when the latter was gaining their feet in commerical aviation. Emirates today learned a lot of their operating style from PIA. Its shocking to see how much PIA have declined.
Because of politics bro
and the whole pilot licenses scam and cheating in Pakistan (involving some PIA pilots). Decline indeed... :(
Interesting fact: Emirate’s flight code, EK, come’s from it’s debut flight from Emirate (Dubai) to Karachi in 1985.
Sad to say, Pakistani politicians destroyed PIA
I am from Pakistan and political issues has destroyed PIA.
Well my friend Pakistan as a nation has declined so what can we expect from it's state flag carrier?

True I do that by skipping one stairs step lol
After the crash PIA Airways installed landing checklist onto control columns
thats good
the chart itself causes ambiguity. look at the descent line path...
It's called the glideslope, and it's made that way to reduce risk of ground terrain collisions
@@harryroberts388 Well... I mean it kinda didn't do it that time did it?
@@9999AWC The chart assumes that the pilots are competent. In this case they weren't.
@@battlec43 No, the people who designed the chart assumed it was fail-proof, which it wasn't. The pilots were experienced. If the investigators found that the chart and cockpit was designed in such a way that it could lead to missing a detail easily, then you can't simply blame "pilot incompetence". Furthermore, it's easy for you to sit at home all relaxed and criticize others; I'd love to see you be in the hot seat. I have; during an IFR approach you're satires with workload (flying the plane, briefing the approach, performing pre-landing checks, communicating with ATC, etc). Therefore, if pilots are already saturated in that phase of flight, then the cockpit ergonomics and the IFR approach charts should be made as to minimize the possibility of misunderstanding (like what happened in this case).
Aviation occurences don't happen merely because of one thing; it is a string of events that enable such an outcome to occur. Look up the Swiss Cheese model. If ONE factor happened differently or did not occur, it is very likely this crash would not have happened. So don't disrespect a deceased crew for an honest mistake when they did their duty to the best of their abilities, because NO ONE is infallible to mistakes, no matter how experienced they are.
I love how the one thing that brought down the plane was the captain’s thumb
Should this be a joke?

Chart creator should have been held responsible to a degree
Yeah, it heavily points at pilot error in the official findings, but it's more of an inadequate cockpit layout and confusing charts. The pilots probably did the best they could with the severe limitations.

Not too familiar with how planes are configured for airlines....but isn't it usually the manufacturer that decides where a clipboard would go in a cockpit lol? Seems like a bit of an odd thing for a airline to decide
Clear case of pilot error.. so scary and sad . This can happen at any time
Mayday, I love that show.
will you guys ever make an episode on Wayfarer Flight 515?
An excellent video. 💙 T.E.N.
Please make one about the airctaft in denver I really wanna see one
Crazy to think in the days of VNAV and ILS, that just a couple of decades ago, they were landing airliners with just charts
there is still no ils at Kathmandu. Only RNAV or DME.
@@naumansajid8630 point is ILS is common now, whereas back then this was common procedure
So they made the chart only 1,000 ft above the mountain, what a brilliant decision
Does smithsonian show full episodes
2:33 has anyone paused this and looked at the chart? If you look at the line, 10,500 ft is at 13.0 miles, not 16.0. At 16.0 miles, it’s above 10,500 ft (his thumb is blocking the altitude).
That, I believe, was literally the point of the episode.
Why are these so addicting ?

I think this is a summary of episode Kathmandu Descent that is similar to but not the same as The Lost Plane (Thai Airways Flight 311). They both crash head-on into a mountain in Kathmandu killing all on board. I can't find the former, Kathmandu Decent, full episode that is a Pakistan Airline.
What is the show name and which episode is it?
It takes one finger to change a fate.
I've heard my whole life how safe flying is, but I could literally watch a new video 24/7 for weeks, so how safe is flying?
Are u being serious? Considering the insane amount of flights that happen every day, the percentage of flights that result in a crash are extremely low meaning yes, flying is very safe
some of these are older, less new technology, some of these are just poorly trained pilots, some yes, are errors, but very very rare and all these videos aren't just in one year of occurence; over many decades
there are tens of millions of flights every year and there are less than 100 accidents every year as well. its pretty safe all things considering,

The head investigator who is wearing blue cyan clothes is the same actor who play pilot in “The Plane that Vanished,” Adam Air, right?

Just a 1000 feet between normal decent and a head on collision?🤔
But why the terrain waring didint sound ?
Hard to believe this to be the cause...there are route checks
Approach control should of spotted the mistake and corrected their approach.

Yep, the captain misplaced his thumb finger and that is how it went
In short, it wasn't pilot error, but it was paper error.
@@amnplays3211 it was human error
It was both. The approach chart was very small and there were no clips that the pilots could use to clip their approach charts.

Why did the ATC not think they did something wrong? Not hearing that they ascended to 11,500 ft?
Yeah cause the pilot didn't listen to ATC. They said report one, six miles, but no change in altitude.

Seriously 4 inch chart? How cheap was their employer.
Man back then Pia was like Etihad airlines a big one of course they are not going to use a cheap one
It's not pilot's fault it's the chart 📉 and no redder in airport and all.
Cause : one step ahead is not correct step in Kathmandu
Hmmm...
This is like a mystery channel. They always cut the movie in the middle. I don’t understand the point. What a waste of time!!
Hmmm weird
everybody thinking it was pilot mistake and no body thinking about why no margin or error is given in charts ? 1000 feet clearance above the top of the world or Pilots are supposed to be 100% correct or be dead
Khona into e off
This guy doing the investigation isn’t he dead 😭😭😭. I’ve seen him before in one of the episodes and all in board the flight were declared dead😭😭😭
He was a pilot 😭😭😭
I think his eye glass was the cause
It's actually Airbus 300
its airbus a300

In heaven his friend ask,
How can u die ?
pilot : dont ask
a system is strong as its weakest link..a thumb caused a plane crash..nearly as good as air france with tape over the speed sensor causing a wrong input in flight computer confusing the silly pilot, stalling the craft..tape is used when washing the plane...gaffer tape 4 in square ...total loss....
All because a pilot’s thumb obscured part of the chart.
Gpws error
Omg
Never fly on an Airbus.
