If you have 4 devices on the mux, how do you individually address each mux? Do you send a command to the mux to talk to a particular device or does the mux do the polling, and then offset the fixed address with a port+fixed address?

I haven't used this breakout board, but a traditional multiplexer works with a series of select bits to select which input to transfer to the output, so for an 8-1 multiplexer, three bits are required for the select bits. So say you wanted to select the first port, the binary command to select this port would be the value 000 (001 for the second, 010 for the third, and so on) and with SparkFun utilities, this port would probably have a label that you can specifically reference in the source code. Throw this all together in a loop to cycle through each port and you can transfer data to/from each port "simultaneously".
